Abnormal eye movements in Creutzfeldt-Jakob disease.

We report 3 patients with autopsy-proven Creutzfeldt-Jakob disease who, early in their course, developed abnormal eye movements that included periodic alternating nystagmus and slow vertical saccades. These findings suggested involvement of the cerebellar nodulus and uvula, and the brainstem reticular formation, respectively. Cerebellar ataxia was also an early manifestation and, in 1 patient, a frontal lobe brain biopsy was normal at a time when ocular motor and cerebellar signs were conspicuous. As the disease progressed, all saccades and quick phases of nystagmus were lost, but periodic alternating gaze deviation persisted. At autopsy, 2 of the 3 patients had pronounced involvement of the cerebellum, especially of the midline structures. Creutzfeldt-Jakob disease should be considered in patients with subacute progressive neurological disease when cognitive changes are overshadowed by ocular motor findings or ataxia.

Thimerosal reveals calcium-induced calcium release in unfertilised sea urchin eggs.

The fertilisation calcium wave in sea urchin eggs triggers the onset of development. The wave is an explosive increase in intracellular free calcium concentration ([Ca2+i]) that begins at the point of sperm entry and crosses the egg in about 20 s. Thimerosal is a sulphydryl reagent that sensitises calcium release from intracellular stores in a variety of cell types. Treatment of unfertilised eggs with thimerosal causes a slow increase [Ca2+i] that results eventually in a large, spontaneous calcium transient and egg activation. At shorter times after thimerosal treatment, egg activation and the calcium transient can be triggered by calcium influx through voltage-gated calcium channels, a form of calcium-induced/calcium release (CICR). Thimerosal treatment also reduces the latency of the fertilisation calcium response and increases the velocity of the fertilisation wave. These results indicate that thimerosal can unmask CICR in sea urchin eggs and suggest that the ryanodine receptor channel based CICR may contribute to explosive calcium release during the fertilisation wave.

Sperm-induced currents at fertilization in sea urchin eggs injected with EGTA and neomycin.

Membrane currents were measured in single voltage-clamped sea urchin eggs (Lytechinus pictus and Lytechinus variegatus) that were injected with either EGTA or neomycin and inseminated. Although egg activation and the fertilization calcium wave were prevented by injection of either of these compounds, sperm attached and still elicited inward currents. Sperm-induced currents in EGTA-injected eggs had an abrupt onset, quickly reached a maximum, and then slowly declined in amplitude. Sperm incorporation occurred readily in EGTA-injected eggs. Similar results were obtained with another calcium chelator, BAPTA. In neomycin-injected eggs, sperm-induced currents generally had an abrupt onset and, in contrast to EGTA-injected eggs, the currents usually cut off rapidly. Sperm failed to enter the neomycin-injected eggs and the duration of sperm-induced currents in neomycin-injected eggs was markedly dependent upon the voltage-clamp holding potential, with shorter duration currents occurring at -70 than at -20 mV. The lability of the initial interaction between sperm and egg at negative holding potentials may explain why activation often fails when the egg membrane is voltage clamped at these potentials (Lynn et al., Dev. Biol. 128, 305-323, 1988).

Internal calcium release and activation of sea urchin eggs by cGMP are independent of the phosphoinositide signaling pathway.

We show that microinjecting cyclic GMP (cGMP) into unfertilized sea urchin eggs activates them by stimulating a rise in the intracellular free calcium ion concentration ([Ca2+]i). The increase in [Ca2+]i is similar in both magnitude and duration to the transient that activates the egg at fertilization. It is due to mobilization of calcium from intracellular stores but is not prevented by the inositol trisphosphate (InsP3) antagonist heparin. Furthermore, cGMP does not stimulate the eggs Na+/H+ antiport when the [Ca2+]i transient is blocked by the calcium chelator bis-(O-aminophenoxy)-N,N,N',N'-tetraacetic acid (BAPTA), suggesting that cGMP does not activate eggs by interacting with the their phosphoinositide signaling pathway. However, the [Ca2+]i increase and activation are prevented in eggs in which the InsP3-sensitive calcium stores have been emptied by the prior microinjection of the InsP3 analogue inositol 1,4,5-trisphosphorothioate. These data indicate that cGMP activates eggs by stimulating the release of calcium from an InsP3-sensitive calcium store via a novel, though unidentified, route independent of the InsP3 receptor.

Risks and benefits of paracetamol antipyresis in young children with fever of presumed viral origin.

To examine whether antipyretic therapy in young children is associated with potential risks (interference with enhanced host defences at febrile temperatures) or benefits (improved comfort and behaviour), a randomised, double-blind, placebo-controlled trial of paracetamol was conducted among 225 children 6 months to 6 years of age who presented with acute (less than or equal to 4 days) fever (greater than or equal to 38 degrees C per rectum) without evident bacterial focus of infection. Parents were asked to give paracetamol liquid 10-15 mg/kg or placebo every 4 h as needed for fever and to avoid bathing, sponging, or other pharmacological agents. Parents kept temperature and symptom diaries and recorded changes in child comfort and behaviour according to a pretested, 5-category Likert-type questionnaire 1-2 h after every dose. There were no significant differences between treated and placebo groups in mean duration of subsequent fever (34.7 vs 36.1 h) or other symptoms (72.9 vs 71.7 h). Paracetamol-treated children were more likely to be rated by their parents as having at least a 1-category improvement in activity (38 vs 11%; p = 0.005) and alertness (33 vs 12%; p = 0.036) but no significant differences were noted in mood, comfort, appetite, or fluid intake. That overall improvement in behaviour and comfort with paracetamol was not impressive is underscored by the inaccuracy of parents' "guess" at the end of the trial as to which agent their child had received-45% correct guesses for paracetamol and 52% for placebo. The data suggest that the clinically relevant hazards and benefits of paracetamol antipyresis have been exaggerated.

Molecular cloning and characterization of a defective recombinant feline leukaemia virus associated with myeloid leukaemia.

The GM1 strain of feline leukaemia virus (FeLV) was isolated from a naturally occurring case of myeloid leukaemia and induces severe haematopoietic abnormalities, including myeloblastic leukaemia, on inoculation into cats. Molecular clones of FeLV-GM1 proviruses were obtained and studied by restriction enzyme mapping, blot hybridization and partial DNA sequence analysis. Two types of clone were isolated; the first was a replication-competent FeLV of subgroup A, resembling other low or minimally pathogenic FeLV-A isolates; the second was replication-defective with extensive deletions and mutations in gag and pol, although it has an intact env gene of subgroup B phenotype. Large segments of the defective proviruses, from the 5' leader sequence upstream of the gag gene to the 5' half of the env gene, show structural hallmarks of endogenous FeLV-related proviruses. Infectious FeLV-GM1 viruses recovered after transfection were tested for their leukaemogenic potential in newborn cats. Early polyclonal myeloproliferative changes were observed in cats inoculated with FeLV-A/GM1 alone, although these were more pronounced in animals receiving the full FeLV-AB/GM1 complex reconstituted by cotransfection of the defective virus FeLV-B with its FeLV-A helper. Analysis of viruses in the bone marrow showed that replication of the subgroup B component is delayed and restricted to a proportion of cats. Most of the infected cats developed persistent abnormalities of haematopoiesis and one progressed to disseminated myeloid leukaemia. The defective recombinant FeLV-B/GM1 appears to play an indirect but important role in myeloid leukaemogenesis.

Immunoassays and nucleic acid detection with a biosensor based on surface plasmon resonance.

A technique based on surface plasmon resonance is described which can be used to detect changes of refractive index that occur when one partner of a molecular binding pair diffuses from solution to bind the other partner which is immobilised on a silver surface. Results for the molecular binding pairs; protein-antibody, hapten-antibody and DNA-DNA are described. Instrumentation necessary for implementation of the technique is detailed. Immunoassay of proteins and haptens is possible in less than one minute with a sensitivity of 10(-9) mol/l. Hybridisation of 10 fmoles of a 97 base target sequence on the 1 mm2 area of detection to an immobilised oligonucleotide probe can be detected in less than five minutes. Advantages of the technique include the ability to record the kinetics of binding reactions in "real time" and the lack of labels in this simple assay format. Methods of improving the sensitivity are discussed.

Hemodialysis-related porphyria cutanea tarda--treatment failure with charcoal hemoperfusion.

A 56-yr-old man presented with a photosensitive bullous dermatosis 1 yr after commencing hemodialysis. Urinary porphyrin levels were normal and fecal porphyrin levels only minimally increased, but the plasma uroporphyrin concentration was markedly elevated, being higher than that normally occurring in porphyria cutanea tarda. The high plasma uroporphyrin level was thought to result from the combined effects of increased porphyrin production and failure of adequate clearance by hemodialysis. Chronic alcohol ingestion, methyldopa therapy and elevated iron and aluminum levels may have contributed to the porphyrin overproduction. Charcoal hemoperfusion proved unhelpful in lowering the plasma uroporphyrin concentration.

Cardiovascular components of the defence reaction evoked by excitation of neuronal cell bodies in the midbrain periaqueductal grey of the cat.

In the anaesthetized and paralyzed cat the cardiovascular pattern characteristic of the defence reaction, i.e. skeletal muscle vasodilatation, tachycardia and increased arterial pressure, was elicited by microinjections (0.40 microliter) of the excitatory amino acid, DL-homocysteic acid (DLH), into the midbrain periaqueductal grey (PAG) matter. In contrast, injections of DLH into the midbrain tegmentum failed to elicit the cardiovascular components of the defence reaction. As injections of DLH depolarize cell bodies but not axons, the results indicate that the midbrain PAG, but not the adjacent tegmentum, contains a population of neurones whose excitation elicits these reactions. Further, the data suggested that such neurones may be localized preferentially within the caudal half of the midbrain PAG.

Road traffic injuries in Sharjah.

Hospital data of all patients seen as a result of road traffic accident injuries are presented for the year 1977. A follow-up study was carried out in December 1978 of those patients admitted with fractures of the skull and spine. The road traffic accident rate was 810 per 100 000 and the death rate was 25 per 100 000. All the deaths in hospitals were due to skull fractures. A third of those with cervical spine fractures were discharged with serious permanent disabilities. The problems of planning health services for this most unusual population are discussed.

Extra-abdominal desmoid tumours.

Desmoid tumours are not common but have a distinct resemblance to fibrosarcomata. Their clinical appearance and progress should be recognised since failure to distinguish them from sarcomata could result in extensive and unnecessarily mutilating operations. Three cases of extra-abdominal desmoid tumours are reported. Two of the patients had tumours arising from multicentric foci in the same limb. The disappearance of the tumours in two patients after the menopause, and the variation in the tumours during the menstrual cycle in the third patient, add weight to the theories about endocrine control.

The preformed socket and modular assembly for primary amputees.

A method is described by which an artificial limb can be fitted as soon as an above-knee or below-knee amputation wound is healed, by using preformed sockets which enable the prosthetist to fit the limb within the hour. The method has been found safe, and better than other methods previously described to facilitate the early mobilisation and rehabilitation of the amputee.
