Structural determination of the sialic acid polysaccharide antigens of Neisseria meningitidis serogroups B and C with carbon 13 nuclear magnetic resonance.

The application of 13-C nuclear magnetic resonance to the analysis of some sialic acid-containing meningococcal polysaccharide antigens is described. Complete assignments of the spectra of both the native serogroup B and the de-O-acetylated serogroup C polysaccharides have been made. These assignments were based on the corresponding data for some related monomers (sialic acid and its alpha-and beta-methylglycosides) and on supportive chemical evidence. The data indicate that the serogroup B polysaccharide is a 2 yields 8-alpha-linked homopolymer of sialic acid, identical in structure with colominic acid from Escherichia coli, whereas the de-O-acetylated serogroup C polysaccharide is a 2 yield 9-alpha-linked homopolymer. The native serogroup C polysaccharide is O-acetylated (1.16 mol of O-acetyl per sialic acid residue), all the O-acetyl substituents being located only at C-7 and C-8 of the sialic acid residues, and in addition contains unacetylated residues (24%). The polysaccharide contains di-O-acetylated residues (O-acetyl on C-7 and C-8), and at least one of the possible monoacetylated residues at C-7 or C-8.

The New Patient Index - a method of measuring the activity of day hospitals.

The workload of three Day Hospitals was studied during 1973 and a simple mathematical calculation, the New Patient Index, devised to enable rapid assessment of function and activity to be made. This calculation gives a more accurate assessment of function than the conventional returns of total attendance or of new patient attendance for it takes size and activity into account. It can be measured over any period of time, weekly, monthly or yearly. It is proposed that a calculated New Patient Index should be included in all future reports on Day Hospitals. In addition it is suggested that the calculation should be used to plan Day Hospital development based on the discerned needs of the elderly in an area or district.

Secretion of human chorionic gonadotrophin by the early placenta in Vitro.

The secretion in vitro of HCG and proteins was studied in fragments of placenta from women in the first trimester of pregnancy by a pulse-chase system. A 10-min pulse with [3H]leucine was used. It was concluded that the approximate half-time of release of HCG was 150 min. Proteins precipitable with trichloroacetic acid had a bi-exponential pattern, the half-times of release being 100 and 270 min. These rates of release indicate that the HCG produced by the early placenta was rapidly passed into the circulation rather than stored.

[The Arias-Stella phenomenon and its diagnostic value in ectopic pregnancy. Studies of 86 cases with review of literature].

The role of the pathologist in the diagnostic of an ectopic pregnancy is mostly limited. Nevertheless, out of 59 tubaric pregnancies, in which endometrial curettage previous surgery, in 64.4% an ectopic pregnancy was predicted, on 30,5% there was a typical "Stella Arias" reaction, mean while on 33,9% there was a decidual-like reaction. The authors also make a revision on morphology and pathogenesis of "Stella Arias" reaction.

[Electrogastroenterographic study of the digestive motor effects of prolonged psychotropic treatment].

Functional digestive complaints are frequent in psychiatri patients: simple constipation, which cannot be explained solely by the loss of the sensation of rectal fullness; occlusions, occasionally hemorragies; the late complication of dolichomegacolon (Bourgeois, 1973). In 160 subjects, an attempt to understand the physio-pathology were made by recording diurnal digestive motor activity using skin electrodes placed on the abdomen and extremities (electrogastroenterography or E.G.E.G.). A hypoactive E.G.E.G. was observed in 2/3 of 18 psychotic depressive patients, in 3/4 of 36 schizophrenies. The nocive effect of giving sedative phenothiazine and antiparkinsonian drugs (trihexyphenidyl or ethybenzatropine) during long periods is clear. Whereas non sedative phenothiazine and clotiapine gicen in small doses, do not have an undesirable effect. Sulpiride has been used in gastroduodenal dyskinesia. The dyskinesia noted by the E.G.E.G., sometimes found in the large intestin, were found in 55% of 30 patients with caracter disorders; they coincide with the high frequency of electro-encephalogram dysrythmies. Finally, in hysterical patients, one usually observes normal E.G.E.G., tracings which confirms the clinical observation that hysterical and psychosomatic symptoms, may succeed each other, but do not appear at the same time. In the same categories of patients, no longer treated in a classical psychiatric environment but in a group with institutional objectives, the same clinic results were obtained with fewer digestive disturbances. This tends to show the inutility and nocivity of excessive doses of psychotropic drugs given alone or in complexe association.

Saphenous nerve injury caused by stripping of the long saphenous vein.

Sensory impairment in the cutaneous distribution of the saphenous nerve was measured in 30 patients after bilateral stripping of the long saphenous vein. The vein was stripped upwards in one leg and downwards in the other. Three months after operation a significantly greater incidence of sensory loss was found in those legs in which the vein was stripped upwards (13 out of 26) compared with those which had been stripped downwards (6 out of 26).We recommend that the long saphenous vein should be stripped downwards.
