A 5 year follow-up of Dacron femoropopliteal bypass grafts.

Over a 5 1/2 year period, 66 Dacron femoropopliteal grafts were performed for patients with an absent or unsuitable long saphenous vein. The minimum follow-up has been 6 months and cumulative patency was 50 per cent at 5 years. Twenty-five patients had critical ischaemia (preoperative Doppler ankle pressure less than 40 mmHg) and 41 patients had severe ischaemia (pre-operative ankle pressure greater than 40 mmHg). The procedure significantly improved ankle pressures in both groups and this was maintained at follow-up. In the group of 25 patients with critical ischaemia there were three operative deaths and in 10 the graft subsequently occluded, precipitating an amputation. In the group of forty-one patients with severe ischaemia, there was one operative death and in two patients the graft occluded at 18 and 24 months. In this small series there was no significant difference in patency, whether the graft was placed to the popliteal artery above or below the knee joint, or whether the popliteal had less than three patent branches at its trifurcation.

Assessment of myocardial function before aortic surgery by radionuclide angiocardiography.

A consecutive series of 41 patients undergoing elective aortic surgery had pre-operative assessment of cardiac function by radionuclide angiocardiography. Their subsequent progress was monitored. There were 4 patients whose pre-operative left ventricular ejection fraction was less than 30 per cent and 3 of these patients died of cardiac failure postoperatively. There was only 1 death from cardiac failure amongst the 37 patients whose pre-operative left ventricular ejection fraction was less than 30 per cent. Radionuclide angiocardiography is an accurate indicator of myocardial function of patients before major aortic surgery.

Intestinal function and intestinal blood supply: a 20 year surgical study.

The experience of The Middlesex Hospital in the investigation and management of chronic intestinal arterial occlusion (IAO) over a 20 year period is presented. Of some 100 patients referred only 28 were found to have IAO and these were critically evaluated in terms of insorptive and exsorptive function. No relation was found between intestinal performance and the degree of potential ischaemia suggested by angiography. Twenty two patients underwent reconstruction of the coeliac axis and mesenteric arteries, with three perioperative deaths. The remainder were followed for periods ranging between six months and 10 years. Of these, 15 are asymptomatic, one is unchanged, and one is subjectively worse. The role of elective surgery in relation to the visceral arteries is critically discussed in the light of these results, and in the perspective of the world experience.

A brief test for measuring malingering in schizophrenic individuals.

The M Test, a brief test for measuring malingering of schizophrenic illness, contains true-false items describing actual symptoms of schizophrenia, bizarre attitudes and beliefs, and fake symptoms. Normal subjects rarely endorsed any of the items. The responses of genuinely ill schizophrenic inpatients were compared with those of normal subjects who had been asked to fake malingering after being educated about the symptoms of schizophrenia and forewarned about the purpose of the test. The test significantly discriminated between genuinely ill patients and malingerers. The test correctly identified 87.3% of the schizophrenic subjects and 78.2% of the malingering normal subjects.

The role of percutaneous transluminal angioplasty for atherosclerotic disease of the lower extremities.

A consecutive series of 62 patients (74 symptomatic limbs) were admitted with atherosclerotic disease of the legs in a 6-month period from 1st July 1982. All patients had an arteriogram under local anaesthetic the day after admission and if possible a transluminal balloon dilatation was performed. If dilatation failed an operation was performed the following day. Dilatation was successful in 13 of 23 lesions above the inguinal ligament and significantly improved the Doppler foot pressures from 84 to 120 mmHg. Dilatation was only successful in 17 of 51 lesions below the inguinal ligament and 40% of these had re-occluded within 6 months. Transluminal dilatation is an important advance for stenotic lesions of the distal aorta and iliacs but its place is limited for the more common lesions in the superficial femoral artery.

Seeding Dacron arterial prostheses with peritoneal mesothelial cells: a preliminary morphological study.

A preliminary laboratory study has been undertaken using dogs, in which porous Dacron arterial prostheses have been seeded with autologous peritoneal mesothelial cells before implantation into the arterial system. These cells were harvested from omentum by collagenase digestion and were introduced into the graft at the time of preclotting. Examination of the grafts by scanning and transmission electron microscopy one month after insertion showed no organized cellular lining in the control graft. In three seeded grafts there was a lining of mesothelial cells which extended over the whole surface of the grafts up to 1 cm from the suture lines. Selected areas of the grafts have been analysed morphometrically. In the seeded grafts there is a cellular layer which on average covers 94 per cent of the luminal surface. These results suggest that mesothelial cells, which both secrete prostacyclin and possess fibrinolytic activity, present a possible alternative to endothelium as a cellular lining for prosthetic grafts.

Long term results of 66 femoral-to-femoral by-pass grafts: a 9-year follow-up.

Over an 8-year period, 66 femoral-to-femoral grafts were performed for unilateral iliac artery disease. The grafts were inserted retropubically with an operative mortality of 4.5 per cent. The minimum follow-up has been 12 months and cumulative patency 80 per cent at 6 years. Sixteen patients had critical ischaemia (preoperative Doppler ankle pressure less than 40 mmHg) and 50 patients had severe ischaemia (preoperative ankle pressure greater than 40 mmHg). The graft significantly improved initial ankle pressures and this improvement was maintained at late follow-up (average 45 months). There was no evidence of a detrimental effect on the donor limb. Long term patency was adversely affected if there was preoperative critical ischaemia, a woven Dacron prosthesis was used or the patient continued to smoke.

Inhibition of specific binding of [3H]phorbol-12,13-dipropionate to an epidermal fraction by certain irritants and irritant promoters of mouse skin.

Specific binding of [3H]phorbol-12,13-dipropionate ([3H]PDPr) to a particulate fraction of mouse skin is demonstrated (KD = 35 nM; Rt = 1.2 pmol/mg protein). A series of compounds of the diterpene ester, indole akaloid and polyacetate types with different degrees of activity as skin tumor promoters and/or irritants have been tested for their capacity to inhibit specific [3H]PDPr binding. Three main categories are found: (i) compounds which exhibit a positive correlation between their potency as irritants and promoters in vivo and their inhibition of specific binding in vitro: 12-O-tetradecanoylphorbol-13-acetate, 3-O-tetradecanoylingenol, pimelea factor P2, 'teleocidin', dihydroteleocidin B, and lyngbyatoxin are active in vivo and in vitro, whereas phorbol and ingenol are inactive and 4-O-methyl-12-O-tetradecanoyl-phorbol-13-acetate is weakly active; (ii) compounds which are strong irritants and inhibitors of binding but are weak or practically non-promoters: mezerein, 12-O-retinoylphorbol-13-acetate and milliamine C; (iii) strong irritants which are weak or marginally active inhibitors of binding: debromoaplysiatoxin and resiniferatoxin. Some consequences of these findings with respect to interpretations of the biochemical mechanism(s) of tumor promotion are discussed.

Does 'intestinal angina' exist? A critical study of obstructed visceral arteries.

A study of the mesenteric arteries was carried out in 203 unselected autopsies. Although mild degrees of stenosis were common, the incidence of a 'critical stenosis' was rare and no correlation between degrees of stenosis and previous gastrointestinal symptoms could be found. Radiological demonstration of stenosed or blocked visceral arteries remains therefore of doubtful clinical significance.

Differential effect of benserazide (Ro4-4602) on the concentration of indoleamines in rat pineal and hypothalamus.

1 Low doses (50 and 80 mg/kg) of benserazide (Ro4-4602), an aromatic amino acid decarboxylase inhibitor, markedly reduced 5-hydroxytryptamine and melatonin in the rat pineal gland without affecting hypothalamic 5-hydroxytryptamine. 2 This differential effect shows that inhibition of the pineal gland decarboxylase activity is possible, and confirms that the rat pineal gland is accessible to peripherally acting agents.
