[Disability in childhood. Care and professional training regarding particularly the problems currently seen in long-term control of newborns at risk].

Children with disability are a heterogeneous population distributed between complex conditions including physical disorders, psychomotor and neurodevelopment disorders, social role and school activities limitations. Prenatal, perinatal and neonatal diseases together with disabling chronic conditions may cause childhood disability. Data from literature show a prevalence of childhood disability ranging from 5 to 20% with a wide range of severity level. The social impact of childhood disability is extremely relevant. Children with disabilities frequently require health care services and special education. VLBW (very low birth weight infants, birth weight <1500 gr) have major disabilities (10-15%) while minor disabilities can be found in 20-25% of these subjects. The first step stays in medical prevention but also an oriented training toward these problems for doctors both in pre and post-graduate formation can play a significant role. Support of society in order to understand the complexity and heterogeneity of childhood disability, through social care services, is mandatory. These measures, apparently costly, if correctly done, will be helpful for both the individual subject and the whole society, through a reduction of the disability problems and the final economy cost impact.

A case of B-CLL expressing P-glycoprotein in peripheral leukocytes. Methodological considerations.

P-glycoprotein, the molecular marker of multidrug resistance, was investigated by means of the specific monoclonal antibody C219 in circulating lymphocytes obtained from a patient affected by B-CLL, who had undergone chemotherapy with chlorambucil and CVP. Dot-immunobinding and immunoblotting revealed the presence of P-glycoprotein in the cell lysate, but the immunoperoxidase method and indirect immunofluorescence carried out with a fluorescence microscope and a cytofluorimeter failed to detect the protein. Some methodologic problems are discussed.

P-glycoprotein expression in multiple myeloma.

P-glycoprotein expression was investigated in ten drug resistant patients suffering from multiple myeloma. The immunoperoxidase method on bone marrow plasma cells and immunoblotting were performed by using the monoclonal antibody C219. Five patients resistant to protocols including vincristine, melphalan and doxorubicin were found to express P-glycoprotein. The immunoperoxidase method appeared to be sensitive and suitable for P-glycoprotein detection in bone marrow samples.

P-glycoprotein and drug resistance in acute leukemias and in the blastic crisis of chronic myeloid leukemia.

The aim of this work is to evaluate the relationship between P-glycoprotein expression in circulating blasts and clinical response in patients suffering from acute lymphoblastic leukemia, acute non-lymphoblastic leukemia, and chronic myeloid leukemia in either lymphoid or myeloid blastic crisis. The results obtained show that: a) patients whose blasts express P-glycoprotein are resistant towards protocols including Doxorubicin, Daunorubicin, Etoposide, Mithramycin, Vincristine; b) P-glycoprotein can be expressed constitutively in some cases; c) P-glycoprotein does not appear to be the only mechanism responsible for resistance towards anthracyclines and Etoposide.

Clinical value of radioimmunoscintigraphy in the follow-up of ovarian carcinoma: a prospective study.

Twenty-five patients treated with debulking surgery and chemotherapy for ovarian cancer were prospectively studied to evaluate the efficacy of radioimmunoscintigraphy (RIS) in detecting residual tumor before second-look surgery. RIS was performed with the monoclonal antibody OC125 F(ab')2 labelled with I-131 without knowledge of clinical data and compared with subsequent surgical results. Second look showed tumor persistence in 12 patients, mostly characterized by small lesions. The overall diagnostic sensitivity of RIS was 50% and the specificity was 85%. In particular, RIS showed better sensitivity for pelvic tumor localizations than for abdominal sites (73% vs 33%); this was due to the inability of RIS to detect upper abdominal lesions. Therefore, our conclusion is that, at present, RIS cannot substitute surgical second-look in the management of ovarian cancer, however, considering that also ultrasonography, computer tomography and magnetic resonance are not always able to give definite diagnostic evidence in the follow-up of ovarian carcinoma, RIS could be added to these procedures to balance the limitations of each method. In this regard, the best application of RIS could be in the follow-up of patients with marker elevation without clinical evidence of disease, especially in the case of pelvic fibrosis or adhesions due to previous therapy, where the other non-invasive tools can give doubtful diagnostic results.

[P-glycoprotein and treatment with plicamycin + hydroxyurea in myeloid blastic crisis of chronic myeloid leukemia].

The authors studied P-glycoprotein (P-gp) expression in three patients affected by myeloid blast crisis of chronic myeloid leukemia undergoing chemotherapy with plicamycin + hydroxyurea. P-gp is the molecular marker of multidrug resistance (MDR), and is able to promote the efflux of various drugs (including plicamycin) from the neoplastic cell. When resistance towards plicamycin and hydroxyurea association was observed, the blasts obtained from the three patients were found to express P-gp. Although P-gp may be not the unique mechanism responsible for resistance towards plicamycin and hydroxyurea association, the MDR phenotype may play an important role in the resistance towards this treatment protocol in the blast crisis of chronic myeloid leukemia.

[Aggregation of human neutrophils induced by phytohemagglutinin].

Phytohemagglutinin-induced aggregation of circulating neutrophils was studied in 13 normal subjects. The interference caused by various drugs (vinblastine, cytochalasin B, indomethacin, nicardipine and flunarizine) was tested. The modifications in the aggregation values induced by these drugs are in favour of an evident dependence of the phenomenon upon the trans-membrane flow of calcium ions and the calmodulin function and of a limited dependence upon the microtubular function.

[Study on the antihemorrhagic action of aminaphtone].

This research is the continuation of a previous published paper. The blood losses occurring during multiple dental extractions under general anesthesia in pretreated patients (not with a recent new capillary protector) have been measured. The results obtained have been processed statistically and confirm the effectiveness of the product.

Sugar absorption in healthy preterm and full-term infants.

We have studied carbohydrate absorption in 40 healthy term infants and 10 preterm neonates (31-35 weeks gestation) by respiratory H2, fecal pH, and chromatographic analysis of stools. Sequential studies of H2 excretion (24-h collection) in response to breast feeding were carried out in premature infants during the first 8 weeks of life. Five expired H2 during the first 2 weeks, and two continued to do so in the 3rd to 4th weeks. Breath H2 excretion fell below 10 ppm by 8 weeks and was not related to feeding or sleep. In term neonates, the frequency of incomplete carbohydrate absorption (4-h test) at the end of the first week was 36% for 14 breast-fed, 42% for 12 formula-fed, and 64% for 14 mixed-fed neonates (not significant differences). There were no significant differences between the absorbing and malabsorbing subjects in fecal pH. Chromatographic analysis showed only small quantities of sugars. In summary, incomplete carbohydrate absorption occurred in a high percentage of the newborns studied; the 24-h test evaluated better than the 4-h test; and negative breath H2 excretion indicated development of the capacity of the small intestine to hydrolyze carbohydrates. In the majority of the preterm malabsorbing babies, completely functional lactase occurs within the first month of life. The growth modulators in human milk may increase the rate of maturing of the small intestine.
