Inducement test in cow's milk allergy.

Results of practical tests with whole milk and the three most allergenic proteins were compared in 32 children who were suspected of showing allergy to milk. Diagnosis was composed of a suggestive clinical history, familial atopy, positive in vitro and in vivo tests and regression of symptoms under an exclusion diet. Tests of re-introduction were done with 11 children. Better agreement between the provocation tests and the other elements of the diagnosis was found when the tests were made with the fractions, rather than whole milk. Furthermore, the finding of reduced allergy to casein allows consideration of re-introduction of milk in the form of cheese.

Bronchoalveolar lavage cellular analysis and gallium lung scan in the assessment of patients with amiodarone-induced pneumonitis.

Seven patients suffering from amiodarone-induced pneumonitis were followed after diagnosis over a period of 16 +/- 6 months. All of them showed clinical, radiographic and functional manifestations of interstitial lung disease. Bronchoalveolar lavage was performed on 6 of them and disclosed lymphocytosis in 2 cases (associated to eosinophilia in 1 of them), neutrophilia in 1 and normal differential count in 2. In 1 case there were abundant hemosiderin-laden macrophages. In addition, all patients evidenced an increased 67Ga lung scan uptake. After discontinuation of amiodarone therapy (with or without corticosteroid therapy), 67Ga lung uptake normalized in 3 cases, but remained increased in 3. Bronchoalveolar lavage was repeated only in 2 cases, showing disappearance of lymphocytosis in one and abundant hemosiderin-laden macrophages in the other. Our results suggest that (1) hemosiderin-laden macrophages can be found in bronchoalveolar lavage in patients with amiodarone-induced pneumonitis, probably related to subclinical chronic left-sided heart failure; (2) 67Ga lung scan appears to be a sensitive marker of amiodarone pneumonitis, although it can remain abnormal after pulmonary clinical, radiographic and functional normalization.

Commodity, the bias of the world.

This paper is a survey primarily for non-specialists of the "value of life" controversy, and its relevance to the environment. Many projects which affect the environment also cause or prevent human deaths. For explicit inclusion of the effects on mortality in the cost-benefit assessment, and for consistency within and between projects, monetary valuation is required. There are objections to "pricing life" and to the various methods, but the willingness to pay for risk reduction seems the best approach available. Some recent UK studies suggest a minimum value of 2 million pounds per death.

Aspirin-induced recurrent urticaria and recurrent angioedema in non-atopic children.

Recurrent urticaria and recurrent angioedema are frequent adverse reactions in non-atopic patients who do not tolerate aspirin. Of 1,632 infants and children who visited our allergy department for the first time, nine children (0.55%), five males and four females, presented recurrent urticaria or recurrent angioedema after taking aspirin. No evidence of atopy was disclosed either by clinical history (personal and family), skin testing, eosinophil levels or total IgE. Aspirin intolerance was established by oral challenge. Only three of the nine patients reported aspirin intolerance in their histories.

Blood group, protein, and red cell enzyme polymorphisms of the Hadza of Tanzania.

Three subpopulations of the Hadza were examined for the following antigens and proteins including enzymes A1ABH, MNS Henshaw, CcCwDDuEeVCe, Lua, KJsa, Fy1Fy2, JkaJkb, Dia, Wra, haemoglobin, haptoglobin, transferrin, acid phosphatase, glucose-6-phosphate dehydrogenase, 6-phosphogluconate dehydrogenase, phosphoglucomutase, adenylate kinase, lactate dehydrogenase, and malate dehydrogenase. The results are discussed in relation to other African populations including the Sandawe, Nyaturu, Pygmies, San, and Khoikhoi.

[Clinical and biochemical considerations on depressive states occuring during parkinsonian syndrome treated by L-Dopa].

From 17 observations of depressive illness occuring to parkinsonians treated with L. Dopa alone or associated with an inhibitor of peripheral decarboxylase, several types of depressive illness can be distinguished according to their date of appearance, their evolution with or without tricyclic anti-depressant drugs tertiary amine. According to the characteristics of these depressive illness, to the related locomotive state and to various monoamingeric hypothesises that are suggested both in the genesis of affective disorders and the mechanism of action of two biochemically related depressive aspects, a distinction already considered by other authors for endogeneous depressions.
