Venous digital subtraction angiography in the evaluation of renovascular hypertension.

The study of renovascular hypertension (R.V.H.) presently requires multiple non invasive examinations in order to select between patients with R.V.H. or other kind of hypertension, before resorting to angiography. The use of venous digital subtraction angiography (V.D.S.A.) may change this diagnostic flow-chart. For this purpose, 100 patients with clinical and laboratory data suspect of R.V.H. underwent V.D.S.A. Compared to angiography, V.D.S.A. showed a 100% sensitivity and 93% specificity. Since the sensitivity and specificity of the non invasive techniques vs. V.D.S.A. in the same series was always lower, V.D.S.A. may be proposed as the first examination in the study of R.V.H. The arterial route for D.S.A. is generally not required for diagnostic purposes.

[Differential spirometry and computerized pulmonary ventilation- perfusion scintiphotography as preoperative functional studies in thoracic surgery].

With purposes of a better knowledge of the respiratory function, and for a more and more exact postoperative functional prognosis, the authors considered the results obtained by the differential bronchospirometry and the pulmonary ventilation-perfusion imaging. Through the study of 10 patients, susceptible of operations for lung extirpation, they could so evaluate the reliability and usefulness of said two functional investigations, which can integrate and complete each other.

Serum concentrations of PRL, GH, LH, FSH, TSH and cortisol after single administration to man of a new synthetic narcotic analgesic butorphanol.

The i.m. administration to 6 healthy adult male volunteers of 2 mg butorphanol, a potent synthetic opiate analgesic, resulted in a significant rise in serum PRL level, without affecting GH, LH, FSH, TSH or cortisol secretion. These effects indicate that in man butorphanol exerts an opiate agonist rather than an opiate antagonist effect at the hypothalamic and/or pituitary level.

Biochemical and morphological comparison of microsomal preparations from rat, quail, trout, mussel, and water flea.

Differential centrifugation methods already in use were applied to purify rat, quail, and trout liver microsomes and modified as necessary to purify microsomes from mussel digestive gland and whole water flea. All these microsomal preparations were comparatively characterized with respect to protein and RNA content, levels of markers of subcellular contaminants, ultrastructural morphology, differential spectra of cytochromes P-450 and b5, monoxygenase activity, and in vitro metabolism of p-dichlorobenzene. Yields of microsomal proteins of the tested organisms differed widely, with mussel showing the lowest yield. Very low levels of nuclear and mitochondrial contaminants were detected in all microsomal preparations, but cell membrane contaminants were clearly present in most preparations. Daphnia microsomes were significantly contaminated by plasma membranes, and hepatopancreas microsomes contained significant amounts of partially disrupted secretory granules and plasma membrane. From a qualitative standpoint differential spectra of cytochrome b5 were very similar for all the preparations, whereas cytochrome P-450 spectra were largely dependent on the microsomal preparation as well as on the assay method used. The content of cytochrome P-450 was highest for rat liver microsomes and very low or absent in Daphnia and mussel preparations; the range of cytochrome b5 contents was much narrower. Significant differences were observed among monoxygenase activities of the different preparations. In Daphnia and mussel microsomes, aniline hydroxylase was absent and benzo[a]pyrene hydroxylase activity was much lower than in rat and quail microsomes. Benzo[a]pyrene hydroxylase activity of trout liver microsomes was similar to that of rat and quail microsomes, whereas hydroxylation of substrates which in rat liver are preferentially metabolized by phenobarbital-inducible forms of cytochrome P-450 was much lower in trout microsomes.

Pituitary secretion after administration of a new cerebroactive drug, fipexide.

The effect of a single oral dose of 400 mg fipexide on pituitary secretion was investigated in 10 elderly non-endocrine patients. Fipexide induced significant decrease (P less than 0.05) in serum prolactin (PRL) values at 90 and 120 min after drug administration, without affecting serum growth hormone (GH), gonadotropin (LH and FSH), thyrotropin (TSH) and cortisol values. Fipexide was unable to modify metoclopramide-induced PRL release in five of these patients. Our results show that this drug acts as a mild dopamine (DA) agonist, probably not directly affecting hypothalamic and/or pituitary DA receptors but indirectly via a reduced DA re-uptake at the pre-synaptic level.

Changes in pituitary secretion induced by an agonist-antagonist opioid drug, buprenorphine.

The im administration in 6 healthy subjects of buprenorphine, a derivative of the morphine alkaloid thebaine with agonist-antagonist properties, resulted in a significant rise of serum Prl within 45-240 min, a small and transient increase in serum GH and a significant fall in serum cortisol values. Conversely, no appreciable changes in the serum levels of the other hormones studied were found. The observed hormonal effects indicate that, in man, buprenorphine exerts an opiate agonist rather than an opiate antagonist effect.

Influence of low doses of naloxone on pituitary secretion in man.

Naloxone 0.8 mg im administered to eight healthy subjects did not affect the serum levels of GH, LH, FSH, PRL, TSH and cortisol. Pretreatment with naloxone 0.8 mg increased TRH-induced TSH and PRL release in six healthy subjects. The same pretreatment caused an enhancement of haloperidol-induced PRL secretion in further other group of six subjects.

[Aseptic osseous necrosis after renal transplantation].

Of 165 renal Transplantated patients, 12 developed aseptic bone necrosis in the femoral head (6 patients), in the femoral condyle (5 patients), in the astragalus (1 patient). The onset of symptoms was 6 to 23 months after transplantation. 99mTc-O4-MDP bone scintigraphy and radiological examination associated with clinical signs confirmed the diagnosis. Unresolved hyperparathyroidism, phosforus depletion, ponderal increase, total i.v. prednisolone-boluses and trauma represented conditions which might predispose to the development of lesion. 8 patients were managed with conservative treatment. 4 patients required a total of 8 operations: head replacement arthroplasty and articular cartilage reimplant in two patients with disease involving femoral head; articular cartilage reimplant and condyle replacement arthroplasty in two patients with disease involving femoral condyle.

[Supervision of the postoperative period in biliary shunt operations: validity of sequential hepato-biliary scintiphotography with 99mTc-HIDA].

The Authors' aim was to assess the effectiveness of Sequential Scintiphotography of the Liver with 99mTc-HIDA (SSFH) in the evaluation of post operative jaundice, in operations of biliary derivation (43 observations in 28 patients). SSFH proved to be highly reliable even in comparison with more laborious procedures (such as Transhepatic Cholangiography), and superior to equally simple procedures such as Echo Tomography and Computerised Tomography, evidencing itself as the investigation of choice for the study of this pathology, especially in long-term follow-ups.

Evaluation of the scintigraphic technique for detecting bone metastases in 100 breast cancer patients.

In an attempt to evaluate the utility of bone scintigraphy for the diagnosis and follow-up of skeletal metastases, radioisotopic scan (total body) by 99mTc has been performed on 100 patients suffering from breast cancer in various stages. The radiographic control followed the scan on the basis of clinical or scintigraphic suspicion of bone lesions. Twenty-one patients showed metastases by scan, and in all but 2 the radiography was concordantly positive; in 22 subjects scintigraphy and radiology demonstrated non-malignant lesions. Both scan and radiology were used to explore 169 metastatic localizations, with concordant results in 45% of the sites; however, 7% of the sites with discordant results became positive for metastasis by radiology some months after the scan positivity. The false-negative results were more frequent in the radiographic study (27%) than with the scintigraphic scan (21%). The latter technique was unable to detect osteoblastic lesions in particular. The scintigraphic test also gave reliable results in the follow-up procedure, frequently anticipating the radiological modifications of metastatic lesions. In conclusion, bone scan often appears to be more accurate and preocious than the roentgengraphic test in the detection and the follow-up of skeletal metastatic lesions; it suggests the utility of the bone scan as a preliminary test, which allows a more rational use of the radiology.

[Surgery in radioactive patients (author's transl)].

The authors discuss some legal aspects and protection problems relative to surgical procedures involving radioactive patients, also in the light of recent European guidelines. They analyse in particular the radiation risk associated with the more common cases concerning radioactive substances that are administered for diagnostic purposes. This study affords a preliminary evaluation of the risk inherent in such surgery, in the light of experimental measurements.

Angioscintiphotography with 99mTc in 310 cases of space-occupying kidney lesions.

Angioscintiphotography with 99mTc has been performed in 310 cases of space-occupying kidney lesions with the purpose of establishing the diagnostic value of the kidney perfusion. In the presence of a neoplasm, a more or less marked blood flow in the lesion generally appears as a hot area. This pattern is not observed in poor vascularized carcinomas or in Wilms' tumors. The percentage of poorly vascularized tumors is moderate and does not affect the value of the method. Cystic lesions appear usually to be cold. Large cysts exhibit no uptake; on the contrary, the degree of vascularization in the small cysts is more difficult to evaluate, as the normally supplied surrounding parenchyma overlaps the cold area caused by the cyst. In the evaluation of the angioscintiphotographic picture, we must take into account the concomitant hepatic or splenic vascularization, which may cause some doubts of interpretation because of projection interferences. The renal angioscintiphotography has an important rule in specifying the nature of a space-occupying kidney lesion and may be regarded as a useful screening test.
