Pulmonary infection by Mycobacterium gordonae in an immunocompromised patient.

Mycobacterium gordonae, a scotochromogenic organism, is considered a saprophyte and isolation of this organism in sputum cultures is not generally considered clinically significant. We report the case of 70-yr-old man with Hodgkin's disease and pulmonary infection caused by Mycobacterium gordonae. The clinical and radiographic findings in this case and the course are outlined. Mycobacterium gordonae isolated from patients with debilitating diseases should not automatically be rejected as a contaminant.

A new technique for desorbing substances tightly bound to affinity gels: flat-bed electrophoretic desorption in Sephadex via isoelectric focusing (FEDS-IEF).

In some cases, proteins and other molecules which are tightly bound to affinity gels can be recovered under mild conditions by electrophoresis. We have extended this technique by running electrophoretic desorption in flat-beds of Sephadex in the presence of ampholytes (FEDS-IEF). A number of advantages of this technique are noted: due to the geometry of the apparatus, high voltages can be used which result in short running times; there are no physical barriers to the migration of the protein and no abrupt conductivity drops; desorbed samples are easily located and recovered; and relatively large sample loads can be readily accommodated. Running times are very sensitive to the experimental conditions. Affinity gels should be applied as a narrow zone, distant from the anticipated banding position of the desorbed species. A wide ampholyte interval is generally recommended. The system appears to be gentle and flexible enough to allow investigators to optimize the conditions for desorption of various affinity gel systems.

Comparison of two three-dimensional gated SPECT methods with thallium in patients with large myocardial infarction.

BACKGROUND: Two different commercially available gated single photon emission computed tomography (GSPECT) methods were compared in a population of patients with a major myocardial infarction. METHODS: Rest thallium GSPECT was performed with a 90-degree dual-detector camera, 4 hours after injection of thallium-201 (Tl-201; 185 MBq) in 43 patients (mean age, 62+/-12 years) with a large myocardial infarction (mean defect size, 33%+/-16%). End-diastolic volume (EDV), end-systolic volume (ESV), and left ventricular ejection fraction (LVEF) were calculated by using QGS (Cedars Sinai) and MultiDim (Sopha Medical Vision International, Buc, France). Images were reconstructed by using a 2.5 zoom and a Butterworth filter (order, 5; cut-off frequency, 0.20). LVEF was calculated in all patients by using equilibrium radionuclide angiocardiography (ERNA). EDV, ESV, and LVEF were also measured by using left ventriculography (LVG). RESULTS: Compared with LVG, QGS underestimated LVEF by means of an underestimation of mean EDV. MultiDim overestimated EDV and ESV. GSPECT EDV and ESV overestimation was demonstrated by means of Bland-Altman analysis to increase with left ventricular volume size (P<.05). The difference between LVG and GSPECT volumes was demonstrated by means of regression analysis to be correlated with infarction size. This effect was particularly important with MultiDim (P<.0001). CONCLUSION: In Tl-201 GSPECT, LVEF and volume measurements will vary according to the type of software used.

Diagnostic value of Tl-201 lung uptake is dependent on measurement method.

BACKGROUND: Increased lung uptake during exercise thallium 201 single photon emission computed tomography is related to left ventricular dysfunction and extent of coronary artery disease (CAD). We studied the influence of the lung region of interest (ROI), used to quantify Tl-201 lung uptake, on the diagnostic value of the lung-to-heart uptake ratio (LHR) in detecting CAD with Tl-201 myocardial single photon emission computed tomography. METHODS AND RESULTS: We retrospectively studied 152 consecutive patients referred to our center for stress Tl-201 scanning. Of these, 116 had proven multivessel CAD and 36 had either normal findings on coronary angiogram or a low likelihood (<5%) of CAD. Poststress quantitative analysis was performed from a 4 x 4-pixel ROI over the hottest myocardial region, an 8 x 8-pixel left lung ROI, an 8 x 8-pixel right lung ROI, and a manual ROI encompassing the whole right lung. The LHR was calculated for each lung ROI. Right LHR (R-LHR) provided the best interobserver and intraobserver reproducibility. R-LHR and total R-LHR values were significantly higher in patients with CAD. Only history of myocardial infarction significantly influenced the R-LHR measurement. CONCLUSIONS: The methodology of LHR measurement significantly influences the clinical contribution of Tl-201 lung uptake evaluation. Optimal reproducibility and diagnostic accuracy are provided by a right lung ROI.

[Myocardial Infarction in the population aged 25-74 in Navarra. Incidence, lethality and treatment in the period 1997-98. IBERICA study].

The principal aim of this study was to determine the incidence of Myocardial Infarction in the population aged 25-74 in Navarra, lethality after 28 days, and the characteristics of the treatment applied in the hospitals during the years 1997-1998. A search was conducted for possible cases of heart attack in the hospital services responsible for attending to them, by means of a review of clinical histories. The lethal cases that did not reach the hospitals were researched through the statistical bulletins of deaths, with the information on these deaths completed through the family doctors. 1,059 coronary events were registered, which corresponded to a crude rate of attack of 238 cases per 100,000 amongst men and 59 cases per 100,000 amongst women. Amongst the male patients who reached hospital alive, 50% were smokers, 41% showed hypercholesterolaemia, 35.9% were hypertense and 16% were diabetic. Amongst the women, who showed a higher average age, arterial hypertension was the most frequent risk factor, with 66%. During hospitalisation thrombolysis was carried out on 39.2% of the patients, primary or rescue angioplasty on 4.6%; aspirin or other anticoagulants were administered to 94.2%, beta-blockers to 60.6% and IECA to 37.4%. This study has shown that the frequency of Myocardial Infarction in Navarra is lower than that registered in many European and North American countries, and is concordant with the low rates of mortality due to this cause. Global analysis of the data shows that a high implementation of the care guidelines for this disease with respect to the use of efficient therapies could be observed in the years 1997 and 1998. Nonetheless, with the exception of the use of anticoagulants, hospital variability has been observed in the use of thrombolytic therapy and adjuvant therapies such as the beta-blockers or ACE Inhibitor, and in the use of diagnostic techniques such as arteriography.

[Isolation of Mycobacterium tuberculosis with primary resistance to chemotherapeutic agents in patients with HIV infection].

BACKGROUND: To evaluate the resistance of Mycobacterium tuberculosis in subjects with simultaneous infection by the human immunodeficiency virus. METHODS: Isolation of Mycobacterium tuberculosis was carried out in a University hospital over a period of 7 years with selection of those corresponding to subjects with HIV infection. The isolation of Mycobacterium tuberculosis with resistance to one or more chemotherapeutic drugs was identified and is described herein. RESULTS: A total of 287 subjects were identified having tuberculous disease, 39 (13.59%) of them presented a simultaneous infection by the human immunodeficiency virus. In 4 of the latter (10.26%) Mycobacterium tuberculosis resistant to one or more chemotherapeutic drugs was isolated with the most frequent resistance being to isoniazide (3/4). CONCLUSIONS: A high degree of diagnostic suspicion must be maintained in patients with the human immunodeficiency virus with respect to tuberculosis, not only because of frequent endogenous activation but also because of the danger of in-hospital transmission. The possibility of resistance makes systematic antibiograms in these cases advisable. In these patients recommendations regarding treatment should be followed avoiding shortened or simplified therapeutic periods which have occasionally been demonstrated as ineffective.

[Longitudinal study of mycobacteriosis, 1983-1987].

The results obtained in the CNMVIS from 1983 to 1987 are reported for the different mycobacterial species identified, the susceptibility of M. tuberculosis to drugs, and the age and sex distribution of the infected patients, to provide additional data regarding the status of mycobacteriosis in this country. 3682 of the 4,015 isolated mycobacteria were identified as tuberculous; among these, the vast majority (99.4%) corresponded to M. tuberculosis. The other mycobacterial species amounted to 25 varieties. On the other hand, no significant changes developed during the five year period regarding predominance of the different species. Primary resistance of M. tuberculosis to chemotherapeutic agents was observed in 7.5% of instances, and acquired resistance in 25.4%; isoniazid, streptomycin and rifampin were the drugs to which resistance more commonly developed. The sex distribution of the affected individuals disclosed a mean males/females ratio of 3.7:1, as shown in similar studies. However, the age distribution did not fit with the reported pattern for other european countries, as in our study there was a greater frequency of tuberculosis in young adults aged 20-39 years.
