[Ureterocele: study of 66 cases observed in 60 children].

Ureterocele is an unfrequent urinary malformation in pediatric age, but frequently it is clinically severe. Ureterocele in a double pyelo-ureteral system often presents very difficult treatment problems not uniformely dealt with. Authors report their experience with 66 ureteroceles observed in 60 children; more than a half of which were less than one year old. In the authors experience simple ureterocele does'nt give any surgical problem. On the contrary ectopic ureterocele, almost always associated with a pyeloureteral duplication, frequently gives some problems of surgical treatment. Treatment is always individualized but in our experience the best results have been achieved with unroofing of the ureterocele and upper heminephoureterectomy in one stage. This treatment can be used successfully in the majority of ectopic ureteroceles in pediatric age.

[Fetal macrosomia. Maternal and fetal implications].

Ninety-six cases of fetal macrosomia have been tested to focus on the factors necessary to select women running the risk of delivering macrosomic fetuses. The results of these studies are the following ones: a) pregnant women with pathology are multiparas; b) they are usually characterized by peculiar weight increase in pregnancy; c) they have gone beyond pregnancy term; d) they have previously delivered macrosomic fetuses. The Authors focus on the necessity to anticipate diagnosis and to plan caesarean section in order to avoid the traumatic lesions this kind of babies are frequently affected by.

[Repeated cesarean section].

Pregnancies after repeated cesarean sections are often considered to carry high maternal and fetal risks. The pregnancy course, intraoperative and postoperative complications and conditions of newborns were compared between 53 patients who had two or more previous cesarean sections and 58 women sectioned for the second time. No statistical difference was found between the two groups except for intraoperative complications.

Epithelial-myoepithelial carcinoma of the salivary glands: a clinico-pathologic, immunohistochemical and DNA flow cytometric study of three cases.

Epithelial-myoepithelial carcinoma is a rare neoplasia of the salivary glands. Histologic, immunohistochemical and flow cytometry analysis were used to study three new cases involving the major salivary glands of two women and one man. The mean age was 66.7 years. Immunohistochemistry confirmed the epithelial-myoepithelial dualism of the tumors: the predominant growth pattern showed ducts made up of an external myoepithelial layer and an internal epithelial layer. Proliferative activity, assessed by means of the PCNA and Ki-67 immunostaining and the study of the S-phase with flow cytometric analysis, proved to be higher in a recurrent case. All three cases were diploid. The use of immunohistochemical and flow cytometric techniques for the assessment of proliferative activity would seem to be an efficient method for the selection of a subgroup of epithelial-myoepithelial carcinomas with unfavorable prognosis.

Malignant myoepithelioma of the minor salivary glands arising in a pleomorphic adenoma.

This paper reports a case of carcinoma expleomorphic adenoma of the minor salivary glands, where the carcinomatous component was a malignant myoepithelioma (myoepithelial carcinoma). Immunohistochemical investigation showed that the carcinoma cells were positive for cytokeratin, S-100 protein and vimentin, and focally positive for actin and desmin. A moderate level of positivity was found fo laminin and collagen IV around the neoplastic islands. These data are the confirmation that the carcinoma cells were myoepithelial. The number of mitoses, the istotype, the minimal extent of extracapsular infiltration and the absence of vascular invasion made it possible to include this particular case among those with a better prognosis.
