MR spectroscopic imaging and diffusion-weighted MRI for early detection of kainate-induced status epilepticus in the rat.

Previous studies have shown that reduction of N-acetyl-aspartate (NAA) is correlated with the degree of neuronal loss at 3 days after kainate-induced status epilepticus in the rat. In this study, magnetic resonance spectroscopic imaging (MRSI), measurement of NAA and lactate, T2-weighted MRI, and diffusion-weighted MRI were used to study early alterations in rat piriform cortex at 12 and 26 h after kainate administration. The major findings are that decreased NAA signal, increased lactate signal, and decreased apparent diffusion coefficient (ADC) were observed at 12 h, with little evidence of histological and T2-weighted MRI changes. These results support the hypothesis that NAA, lactate signals, and ADC provide sensitive methods for detection of early and minimal brain damage in vivo.

Association of anophthalmia and esophageal atresia.

We report on a boy and a girl with bilateral anophthalmia and proximal esophageal atresia. In addition to vestigial optic nerves and chiasma, MRI studies showed other central nervous system abnormalities; one had ectopic tissue in the hypothalamic region, and the other hand generalized ventriculomegaly associated with atrophy. Two other cases, both males, have been reported with anophthalmia and esophageal atresia as their only malformations. These 4 cases are reviewed in light of recent advances in the understanding of ocular embryogenesis and of the midbrain as a development field. Concurrence of these defects appears to be non-random.

Correlation of early reduction in the apparent diffusion coefficient of water with blood flow reduction during middle cerebral artery occlusion in rats.

To determine the relationship between reductions in the apparent diffusion coefficient of water (ADC) and in cerebral blood flow (CBF) during focal ischemia, we used diffusion-weighted magnetic resonance (D-MR) imaging and autoradiographic CBF analysis to examine rats subjected to 30 or 90 min of permanent middle cerebral artery (MCA) occlusion. In the 30-min occlusion group (n = 10), the area with substantially reduced ADC (15% or more below the contralateral level [ADC15]) corresponded best to the area with CBF below 25 ml/100 g/min and was significantly smaller than the area with CBF below 50 ml/100 g/min (CBF50), a level associated with reduced protein synthesis and delayed necrosis (40 +/- 13% versus 74 +/- 8% of the ischemic hemisphere; P < 0.0001). In the 90-min occlusion group (n = 6), the ADC15 area corresponded best to the CBF30 to CBF35 area and was again significantly smaller than the CBF50 area (54 +/- 13% versus 73 +/- 20%, P < 0.05). Thus, the area of substantially reduced ADC at 30 and 90 min represents only 53% and 74%, respectively, of the tissue at risk for infarction. These findings indicate a potential limitation in using early D-MR imaging to predict stroke outcome.

Anatomo-clinical considerations on the ovarian fibroma.

The Authors analysed from an anatomo-clinical point of view, 17 cases of ovarian fibroma, observed during the period between 1968 and 1992. The incidence of ovarian fibroma in this study was 2.52% and it affected women in different age groups, though with greater frequency those above 50 years. Even the symptomatology was variable and was characterized by pain in the hypogastric quadrant (47.1%); alteration of the menstrual cycle (17.6%); metrorrhagic episodes (17.6%); sterility (5.9%). Due to the fact that the ovarian fibroma can appear at any age, this suggests different surgical approaches according to age, general conditions of the patient and fertility.

[Variations in fibronectin plasma levels after surgical stress].

Depletion of reticuloendothelial system following major operations or trauma closely correlates with a decrease of plasmatic levels of fibronectin. Aim of the study is to evaluate the changes in plasma fibronectin levels after a surgical stress. Twenty patients who underwent elective gynecological surgery for a variety of pathological condition were the subject of our study. Circulating levels of fibronectin showed a significative decrease, as compared to preoperative values, and returned to normal during the second postoperative day. The recovery to normal values of this plasma glycoprotein was associated, in our patients, with a normal postoperative course.

Seasonal changes of plasma prolactin concentration in the reproduction of the crested newt (Triturus carnifex Laur).

Plasma prolactin (PRL) and vitellogenin (VTG) concentrations were determined during the annual reproductive cycle of the newt Triturus carnifex Laur. Plasma PRL was measured by a radioimmunoassay using purified newt (Cynops pyrrogaster) PRL as standard and radioligand and its antibodies. Using ELISA, plasma VTG titers were related to ovarian weight. The trends in PRL differed in the two sexes: the PRL changes seemed to be related with vitellogenic processes in the female, whereas they were more related to reproductive behavior in the male.

Effects of 1-year treatment with ipriflavone on bone in postmenopausal women with low bone mass.

Ipriflavone (IP) (7-isopropoxyisoflavone), a synthetic isoflavone derivative, is active in both inhibiting bone resorption and enhancing osteoblast function. This property suggested its clinical use in the treatment of involutional osteoporosis, and in the prevention of postmenopausal bone mass loss. Forty postmenopausal women with low bone mineral content were enrolled and randomly treated for 12 months with IP 600 mg/day or placebo (PL), according to a double-blind, parallel group design. All patients wee also given an oral calcium supplementation (1 g/day). Bone mineral density (BMD) was measured at the spine (L2-L4) by dual-energy X-ray absorptiometry and at the distal radius by single-photon absorptiometry. Bone metabolism markers (serum calcium, phosphate, osteocalcin, and alkaline phosphatase, and urinary calcium, phosphate, and hydroxyproline) were assessed at the same times. After 12 months, a reduction of BMD was evidenced in the PL-treated group, at both the spine (-2.2%, P < 0.01 vs baseline) and the forearm (-1.2%). In the IP-treated group, an increase of BMD was obtained (+1.2%, P < 0.01 vs placebo, at the spine; +3%, not significant, at the forearm). Bone markers were in the normal range for postmenopausal women; no statistically significant modifications were observed during the treatment period. Three patients were withdrawn from the treatment in the IP-treated group, and two in the PL-treated group for gastrointestinal disturbances. In the other women, the tolerance of the drug was good and the compliance with the oral treatment was excellent.

Conjugated deferoxamine reduces blood-brain barrier disruption in experimental optic neuritis.

The purpose of this paper was to investigate the role of deferoxamine (DFO) scavenging of hydroxyl radical (.OH) on disruption of the blood-brain barrier (BBB) and demyelination in experimental optic neuritis. Eighteen strain-13 guinea pigs were sensitized for experimental allergic encephalomyelitis. Nine animals received 100 mg/kg of hydroxyethyl starch-conjugated (HES) DFO by daily intraperitoneal injection commencing the day of antigenic sensitization. Nine paired litter mates received daily IP injections of HES. Serial fat-suppressed magnetic resonance imaging of the optic nerves was obtained with a T2 weighting (T2w) to evaluate demyelination and after intravascular administration of Gd-DTPA to evaluate BBB disruption. The intensity of Gd-DTPA enhancement and T2w signal of the optic nerves was quantitated 3, 7, 10 and 14 days after antigenic sensitization. Animals were then sacrificed and the optic nerves processed for light and transmission electron microscopy with ultracytochemical localization of endogenous hydrogen peroxide (H2O2) and immunogold colocalization of extravasated serum albumin. The area of the optic nerve head, intensity of toluidine blue staining, and the cellular infiltrate were digitized and quantitated. Administration of HES-DFO significantly reduced the intensity of Gd-DTPA enhancement in the optic nerves of HES-DFO-treated animals compared to paired control HES animals (p = 0.0236), with the mean difference between control and treated animals of 19.39. The difference in T2w signal was not significant (p = 0.39), with a mean difference between control and treated animals of -5.51. The intensity of toluidine blue staining of optic nerve specimens was slightly less with HES-DFO compared to untreated animals (mean pair difference 2.48), and the inflammatory infiltrate was reduced with HES-DFO compared to untreated animals (mean pair difference = 61.57); these differences were not statistically significant. In the optic nerve specimens of both groups cerium perhydroxide-derived H2O2 reaction product was evident in a predominantly perivascular and perineural distribution. Immunogold-labeled serum albumin showed extravasation at foci of perivascular inflammation in both the presence and absence of H2O2-derived reaction product. Conjugated DFO reduces disruption of the BBB, as measured by Gd-DTPA enhancement, suggesting the .OH radical generated from perivascular H2O2 may play a role in alterations of vascular permeability in experimental optic neuritis.

Reversals of blood-brain barrier disruption by catalase: a serial magnetic resonance imaging study of experimental optic neuritis.

PURPOSE: To investigate serially the role of catalase detoxification of endogenous H2O2 in the disruption of the blood-brain barrier (BBB) and demyelination of experimental optic neuritis. METHODS: Serial contrast-enhanced magnetic resonance imaging (MRI) of the optic nerves (T1 weighted) and T2 weighted MRI without contrast were performed on 18 guinea pigs 3 to 14 days after sensitization with central myelin for experimental allergic encephalomyelitis. Sex and age-matched littermates were paired and sensitized with the identical antigenic emulsion. To detoxify endogenous hydrogen peroxide (H2O2), animals received daily intraperitoneal injections of polyethylene glycol (PEG)-catalase at a dose of 12,000 U/kg per day for 3 days, then 1,200 U/kg daily for the next week, commencing 3 days after antigenic sensitization. Littermates received an equal volume of preservative-free saline. The intensity of gadolinium-DTPA (Gd-DTPA) enhancement was quantitated by obtaining the value for a region of interest (ROI) of the right optic nerve and the left optic nerve. The effect of H2O2 detoxification by catalase was evaluated by differences in the intensity of Gd-DTPA enhancement and T2 weighted signal in the ROI of the right and the left optic nerves at 7, 10, and 14 days after antigenic sensitization, from the pretreatment value obtained at day 3. The effectiveness of catalase detoxification of H2O2 was assessed with quantitative ultracytochemical localization of electron-dense, H2O2-derived cerium perhydroxide in the optic nerves. RESULTS: With PEG-catalase treatment, mean differences for Gd-DTPA enhancement in the ROI at 7, 10, and 14 days after antigenic sensitization were significantly reduced from the pretreatment values obtained 3 days after antigenic sensitization compared with the comparable interval values for untreated littermates. For T2 weighted signal intensity, only the 7- and 14-day values were significantly less with PEG-catalase compared with values for littermates obtained at comparable intervals. Quantitative ultracytochemical localization of H2O2-derived cerium perhydroxide reaction product revealed significant reductions in the medium number of cerium particle counts of the optic nerve head, sheath, and myelinated retrobulbar nerve. CONCLUSIONS: PEG-catalase reduced H2O2-derived cerium perhydroxide reaction product in the optic nerve but did not eliminate it, reversed disruption of the BBB as measured by Gd-DTPA enhancement, and reduced demyelination and edema as measured by T2 weighted signal intensity, suggesting detoxification of H2O2 as a new treatment strategy for disorders of primary demyelination of the central nervous system.

[Pregnancy in a sample of immigrants from Southeast Asia living in the Messina region].

Hundred-forty-eight pregnancies in immigrants from South-east Asia were analyzed. A high number of such patients underwent legal abortion; in the women who carried on pregnancy, the authors considered the course of pregnancy, delivery and puerperium and some neonatal parameters. The results have shown that pregnancy in immigrants presents a certain amount of risk.

Fat tissue and fat suppression.

Fat tissues consist of fat cells, capillaries, and collagen fibers. In order to completely suppress the signals from fat tissues in clinical magnetic resonance imaging, the signal from capillaries and collagen fibers as well as from fat cells should all be suppressed. We have previously reported that fat signal can be uniformly suppressed by applying an optimized presaturation pulse. The inhomogeneously broadened fat peak of tissue spectrum is excited by the optimized pulse and dephased by a subsequent field gradient. The broadened water peak is not affected. In this paper we discuss a technique that suppresses signals from fat tissues completely as well as uniformly. This technique is based on the cancellation of fat and water signals in the same image voxel by combining the optimized selective excitation with the opposite phase imaging technique. Experimental and clinical images demonstrate that the new technique improves the delineation and depiction of anatomy in clinical fat suppression imaging.

Enhanced iodine concentrating capacity by the mammary gland in iodine deficient lactating women of an endemic goiter region in Sicily.

Iodine balance during pregnancy and lactation was investigated by measuring iodine concentration in the urine of 11 pregnant women, born and living in a moderately iodine deficient endemic goiter area in Northeastern Sicily, collected during the last week of pregnancy, and between the 5th and 7th day after delivery, and in their milk sampled simultaneously with the urine of their newborns. The results were compared with those obtained on similar samples from 16 euthyroid age-matched nongoitrous women and their offspring from an iodine sufficient area. Urinary iodine concentration in pregnant women from the endemic area (1.28 +/- 0.13 micrograms/dl, mean +/- SE) was significantly lower than that of pregnant women from the iodine sufficient area (3.77 +/- 0.57 micrograms/dl) (t = 3.56, p less than 0.005). The longitudinal measurement of iodine concentration in each nursing woman showed a marked increase (approximately 90%) when compared with the values obtained during pregnancy in both endemic and control groups (2.32 +/- 0.36 and 7.76 +/- 2.08 micrograms/dl; t = 2.13 p less than 0.05, respectively). The slight difference in milk iodine concentration between the endemic (3.25 +/- 0.77 micrograms/dl) and the control (4.33 +/- 0.57 micrograms/dl) group was not statistically significant (t = 1.14; p less than 0.5, NS). Similarly no difference was found in urinary iodine excretion between the endemic and the control newborn groups (3.41 +/- 0.76 and 4.30 +/- 0.65 micrograms/dl, respectively, t = 0.88 p less than 0.1, NS).(ABSTRACT TRUNCATED AT 250 WORDS)

Enhancement and demyelination of the intraorbital optic nerve. Fat suppression magnetic resonance imaging.

Conventional spin-echo magnetic resonance imaging (MRI) of intraorbital optic neuritis is hampered by the adjacent high signal and chemical shift artifact of orbital fat. Frequency-selective saturation pulse MRI reduces these problems and was used to determine its utility in evaluation of intraorbital optic neuritis. Eight consecutive patients with optic neuritis underwent MRI within 1 week of the onset of visual loss. Conventional MRI with T1, proton density, and T2 weighting and frequency-selective saturation pulse MRI with T1, proton density, and T2 weighting were performed. After administration of intravenous gadopentetate dimeglumine, T1-weighted conventional and frequency-selective saturation pulse MRI were performed. Frequency-selective saturation pulse MRI showed gadopentetate dimeglumine enhancement in the intraorbital optic nerve in 7 patients and the intracranial optic nerve in 3 patients. Conventional MRI failed to show optic nerve gadopentetate dimeglumine enhancement in patients with intraorbital lesions, but did show intracranial lesions. Frequency-selective saturation pulse MRI showed bilateral optic nerve enhancement in 3 patients with unilateral visual signs and symptoms. Proton density and T2-weighted conventional MRI of the brain showed no convincing signal aberrations in the optic nerves. In the MRI evaluation of intraorbital optic neuritis: (1) frequency-selective saturation pulse fat suppression MRI is superior to T1-weighted conventional MRI in the detection of gadopentetate dimeglumine enhancement; (2) frequency-selective saturation pulse proton density and T2-weighted MRI is superior to proton density and T2-weighted conventional MRI; (3) frequency-selective saturation pulse MRI showed gadopentetate dimeglumine enhancement as well as proton density/T2-weighted signal aberration in exactly the same portion of the intraorbital optic nerve.

Intraorbital optic nerve and experimental optic neuritis. Correlation of fat suppression magnetic resonance imaging and electron microscopy.

To provide magnetic resonance imaging (MRI)-ultrastructural correlations of demyelinating lesions of the intraorbital optic nerve, the authors performed gadolinium-enhanced/T2-weighted fat-suppressed MRI and transmission electron microscopy of the optic nerves of animals with experimental allergic encephalomyelitis. Gadolinium enhancement of the optic nerve adjacent to the globe was seen on fat-suppressed T1-weighted MRI as early as 3 days after antigenic sensitization, increased in severity involving longer segments of nerve at 10 to 14 days, and persisted at 30 days. Gadolinium enhancement preceded T2-weighted signal aberrations. Ultrastructural evaluation of the intraorbital nerve revealed: (1) expansion of the extracellular space and inflammatory infiltrate that correlated with the intensity of gadolinium enhancement; (2) the degree of demyelination correlated with T2-weighted signal aberrations; (3) as deduced from gadolinium enhancement and T2 signal aberrations, breakdown of the blood-brain barrier preceded widespread demyelination; (4) lesions appeared to start at the optic nerve insertion into the globe and then progress toward the orbital apex.

[Antihypertensive drugs during pregnancy. Effects on the fetus].

After reviewing the literature on this topic, the paper examines fetal effects during maternal anti-hypertensive treatment, assessing both possible teratogenic results and fetal outcome. All anti-hypertensive agents cross the placental barrier and are present in varying concentrations in the fetal circulation, thus provoking varying effects on the cardiocirculatory apparatus and fetal metabolism. Some agents, such as alpha-metildopa, may be considered reliable, whereas others, like ACE-inhibitors, are still used with considerable caution, an attitude which will only change following extensive clinical trials and through post-natal checks.

Pregnancy in the obese patient.

The authors considered the course of pregnancy, delivery and puerperium in 70 obese women (pre-pregnancy body mass index greater than 30) and some of the neonatal effects of obesity (weight and morbidity). The results have shown that pregnancy in obese woman presents a certain amount of risk which, however, seem inferior to what it has been believed to be so far. Moreover, the authors underline that in order to further reduce this risk it is necessary for the obese patient to be carefully counselled before pregnancy and controlled during the gestation.

A clinically viable technique of fat suppression for abdomen and pelvis.

Chemical-shift selective imaging with an improved selective presaturation pulse can be used to suppress the fat signal uniformly across the entire abdominal cavity. The required magnetic field homogeneity is no higher than for a routine diagnostic scan. Therefore, no extra setup time for shimming is needed. The technique has been implemented on our clinical system for routine fat suppression scans.
