Ultrasound in abdominal tuberculosis.

BACKGROUND: The present study was done to emphasize the importance of ultrasound (US) and US-guided fine-needle aspiration biopsy (FNAB) in the diagnosis of abdominal tuberculosis. METHODS: Sixty-six proven cases of abdominal tuberculosis were selected for this study. The diagnosis was based on clinical features, US observations, FNAB, operative findings, and responses to appropriate antitubercular treatment. The US findings were interpreted with regard to the involvement of lymph nodes, intestine, peritoneum, solid viscera, and abdominal abscesses. Patients with disease limited to the musculoskeletal and genitourinary systems were not included in the study. RESULTS: Peritoneal tuberculosis was the most common form, of the "wet" ascitic type. Ascites was clear in 19 patients and complex in 17. Tuberculous lymphadenopathy was seen in 37 patients. There was a predilection of periportal, peripancreatic, and mesenteric locations compared with the degree of retroperitoneal involvement. Calcification and heterogeneous echotexture were seen in seven cases. FNAB confirmed the diagnosis of tubercular lymphadenopathy in 19 patients. Intestinal disease was seen in 14 patients. Hepatic or splenic involvement was seen as diffuse organomegaly; less commonly, focal lesions were seen. CONCLUSION: This combination of US findings in proper clinical settings are diagnostic of tuberculosis. FNAB confirms the diagnosis in lymphadenopathy, abscesses, and focal lesions of the viscera.

Prognostic value of placental ultrasound in pregnancies complicated by absent end-diastolic flow velocity in the umbilical arteries.

Our objective was to evaluate the utility of gray-scale placental ultrasound for the detection of pathological lesions in the placentas of preterm pregnancies with abnormal fetoplacental blood flow (defined by absent or reversed end-diastolic flow velocities [ARED] in the umbilical arteries) before 32 weeks of gestation. Sixty consecutive structurally and chromosomally normal singleton pregnancies were evaluated. Pre-defined criteria were used to describe placental appearances using gray-scale real-time ultrasound. Proximal uterine artery Doppler waveforms were recorded using pulsed and color Doppler ultrasound. Each patient had a thrombophilia profile. Following delivery, a single perinatal pathologist reviewed each placenta at a gross and microscopic level blinded to the placental ultrasound findings. Placental shape or texture was abnormal on gray-scale ultrasound in 43/59 (73%) and echogenic cystic lesions (ECL) were found in 16 (27%). Uterine artery Doppler was abnormal in 47/60 (78%) cases. Thirty-eight pregnancies were subsequently delivered by planned Caesarean section in the fetal and/or maternal interest (birthweights 540-2300 g, mean gestational age 30.6 weeks) and 21 pregnancies resulted in the vaginal delivery of a stillborn fetus where fetal weight and/or gestational age did not justify Caesarean section (birthweights 85-600 g, mean gestational age 24.9 weeks). ECL had a low positive predictive value for both villous infarcts (63%) and for focal/massive perivillous fibrin deposition (40%). Nevertheless, the combination of abnormal uterine artery Doppler and abnormal gray-scale findings (abnormal placental morphology or ECL) was strongly predictive of stillbirth (17/21; sensitivity 81%, PPV 52%, p = 0.006 Fisher's exact test). Pregnancies with ARED in the umbilical arteries have a high perinatal mortality associated with pathology of the placental villi. Ultrasound examination of the placenta and its maternal blood supply may contribute to the perinatal management of these pregnancies.

Fine needle aspiration cytology of reactions in leprosy.

OBJECTIVE: To define diagnostic cytomorphologic features of reactions in leprosy. STUDY DESIGN: Part-retrospective, part-prospective, single-blind, controlled study of the applicability of fine needle aspiration cytology in the diagnosis of reactions in leprosy. Cytomorphologic features were compared in 42 clinically diagnosed patients with reactions in leprosy with those in a control group of patients with nonreactional leprosy. The study groups included type 1 and type 2 reactions in 35 and 9 patients, respectively. May-Grünwald-Giemsa and Ziehl-Neelsen staining methods were employed. RESULTS: Statistically significant (P < .01) cytomorphologic features of type 1 reaction were the presence of fragments of collagen and elastin; giant cells; giant cells exhibiting elastin phagocytosis; loose, epithelioid cell granulomas; and fibroblasts. Type 2 reaction was characterized in aspirates by the presence of an abundance of neutrophils in a background of lepromatous leprosy (P < .01). CONCLUSION: Criteria that are used in histopathology for the diagnosis of leprosy reactions can be applied satisfactorily to cytologic smears. A good correlation between clinical diagnosis and cytomorphology can be achieved. Multiple-site aspirates from the skin, nerve and lymph nodes are helpful in substantiating the diagnosis.

[Physicochemical examination of materials used for dentures for elaboration of individual composition of acrylic plastics].

It has been found that materials for dentures are obtained from poly (methyl methacrylate) as the basic component. Basing on the identification analysis of foreign acrylic plastics used for performing denture plates and literature and patent investigations a production technology of individual material calle Akpol S has been elaborated. Series of physicochemical comparative examinations of this composition and the foreign materials with regard to the requirements of ISO 1567 international standard have been carried out. It has been found that this material is comparable with the plastics of this type commonly used with regard to physicochemical features; a it also meets the requirements of the standard.

Effect of protein deprivation and subsequent rehabilitation on the intestinal transport of L-methionine in vivo.

The effect of protein deprivation and subsequent rehabilitation on the intestinal transport of L-methionine was studied in albino rats of both sexes. The rats given diet containing no protein or 3 per cent maize protein for 28 days, lost their intestinal cell population by 50 and 20 per cent respectively. The net absorption rate of L-methionine was little affected, while absorptive capacity of intestinal cells was considerably enhanced in protein-deficient rats. The increase in absorptive capacity of intestinal cells was much higher in rats given protein-free diet than in those given maize diet. The augmentation is absorptive capacity of intestinal cells of protein-deprived rats was a temporary adaptation to the conditions that prevent the formation of new cells. Rehabilitation of malnourished rat on diet containing 17 per cent casein, resulted in a rapid increase in intestinal cell population, return of the absorptive capacity of intestinal cells to normal, and augmentation in net absorption rates.

Mild-to-moderate malnutrition and small intestine of young rhesus monkeys.

The effect of severe malnutrition and protein deficiency on small intestine has been documented, but the literature on the effect of mild-to-moderate protein-energy malnutrition (PEM) on small intestine is scant. Mild-to-moderate PEM is most prevalent in India. Twenty-four young rhesus monkeys weighing 1.5-2.0 kg were divided into two groups, control and experimental. Mild-to-moderate PEM was induced in the experimental group by giving half of the required normal diet providing 2.42 g protein.kg-1.day-1 and 55 kcal.kg-1.day-1. Body weight, serum protein, and D-xylose were measured before starting the experiment, at PEM stage, and after rehabilitation. Experimental monkeys representing group I were killed after a 25-30% reduction in body weight along with control group 1 animals at 12 wk. The rest of the experimental animals were rehabilitated for another 10-12 wk and killed along with their respective controls (control group 2). Brush-border membrane vesicles were prepared from three parts of the small intestine. Viable vesicles were used for the uptake of [U-14C]L-proline. Alkaline phosphatase and enterokinase were also measured. Uptake of L-proline amino acid and the activity of both enzymes were found to be decreased significantly in the PEM group; a D-xylose test was abnormal. All animals recovered after rehabilitation. These results indicate that even mild-to-moderate malnutrition affects the absorptive and digestive capacity of the brush border of the small intestine, which reversed back on rehabilitation.
