Sociocultural aspects of areca nut use.

This paper aims to describe the sociocultural context of areca nut use through exploring people's norms and values and the meaning underlying a wide range of different practices. A historical review is followed by an examination of more recent evidence, including quantitative and qualitative research conducted in the United Kingdom. It is concluded that some reports involving the various uses of areca nut and the conclusions drawn are confusing and that future studies need to be more explicit. While a sound scientific background is required, important sociocultural and religious issues around areca nut use must also be understood if health promotion initiatives are to be considered.

Abnormalities of conduction after total correction of Fallot's tetralogy: a prospective study.

OBJECTIVE: To determine the frequency of post-operative conduction abnormalities in Pakistani patients undergoing total correction for Fallot's Tetralogy. PATIENTS AND METHODS: One hundred and fifteen patients of Fallot's Tetralogy underwent definitive repair between January, 1999 and April, 2000. Their mean age was 12.89 years (range 3-30 years). One hundred nine patients (94.78%) had severe cyanosis and 6 patients (5.21%) were moderately cyanotic due to mild right ventricular outflow tract (RVOT) obstruction. Thirty percent patients required frequent hospital admissions within 6 months before the time of operation due to -hyper-cyanotic spells. The mean haematocrit was 50.83 (range 28-71). The majority of patients were in NYHA class-III (57%) and 45% had previous palliative shunt procedures done. Surgical access was through the RVOT in 90% cases and trans-atrial in 10%. RVOT patch was used in 55.9%, Pulmonary artery patch in 13.5% and trans-annular patch in 17.1% of cases. RESULTS: The mean bypass time was 79.15 min and the mean cross clamp time 51.23 min. Average stay in the intensive care unit was 4.48 days. Twenty-three patients required re-exploration for bleeding. Sixty-nine patients required inotropic support. Fifteen patients had transient heart blocks and two had complete heart block requiring permanent pace-maker. Nineteen patients had various transient arrhythmias which were managed medically. Univariate analysis showed that higher age at operation, low preoperative heart rate, prolonged bypass time, prolonged cross clamp time and presence of patch on pulmonary artery were more common in patients who developed various heart blocks. However, none of these factors had statistical significance or definitive cause-effect relationship with heart blocks. CONCLUSION: With careful surgical technique, total correction of Fallot's tetralogy can be conducted in children and young adults, with a very low risk of conduction abnormalities.

Sol-gel open tubular ODS columns with reversed electroosmotic flow for capillary electrochromatography.

Sol-gel chemistry was successfully used for the fabrication of open tubular columns with surface-bonded octadecylsilane (ODS) stationary-phase coating for capillary electrochromatography (OT-CEC). Following column preparations, a series of experiments were performed to investigate the performance of the sol-gel coated ODS columns in OT-CEC. The incorporation of N-octadecyldimethyl[3-(trimethoxysilyl)propyl]ammonium chloride as one of the sol-gel precursors played an important role in the electrochromatographic performance of the prepared columns. This chemical reagent possesses a chromatographically favorable, bonded ODS moiety, in conjunction with three methoxy groups allowing for sol-gel reactivity. In addition, a positively charged nitrogen atom is present in the molecular structure of this reagent and provides a positively charged capillary surface responsible for the reversed electroosmotic flow (EOF) in the columns during CEC operation. Comparative studies involving the EOF within such sol-gel ODS coated and uncoated capillaries were performed using acetonitrile and methanol as the organic modifiers in the mobile phase. The use of a deactivating reagent, phenyldimethylsilane, in the sol-gel solution was evaluated. Efficiency values of over 400,000 theoretical plates per meter were achieved in CEC on a 64 cm x 25 microm i.d. sol-gel ODS open tubular column. Test mixtures of polycyclic aromatic hydrocarbons, benzene derivatives, and aromatic aldehydes and ketones were used to evaluate the CEC performances of both nondeactivated and deactivated open tubular sol-gel columns. The effects of mobile-phase organic modifier contents and pH on EOF in such columns were evaluated. The prepared sol-gel ODS columns are characterized by switchable electroosmotic flow. A pH value of approximately 8.5 was found correspond to the isoelectric point for the prepared sol-gel ODS coatings.

Biodesulphurization of coal: effect of pulse feeding and leachate recycle.

Biodesulphurization of coal was carried out under four modes of operation namely: conventional batch, constant volume pulse feeding (CVPF), increasing volume pulse feeding (IVPF) and leachate recycle. The effects of different pulse feeding strategies and leachate (product) recycle on biological performance were studied and compared with a conventional batch process. The sulphur removal rates for each of the four processes were 0.04 g/day (batch), 0.09 g/day (CVPF), 0.19 g/day (IVPF) and 0.05 g/day (leachate recycle). The values of iron solubilization rate (batch-83 µg/ml/day; CVPF-136 µg/ml/day; IVPF-198 µg/ml/day; leachate recycle-133 µg/ml/day) also followed the same trend. The percentage sulphur removal on the 30th day using batch, CVPF, IVPF and leachate recycle processes was 72%, 93%, 97% and 90%, respectively. IVPF was found to be the best operational strategy for biodesulphurization process at enhanced rates for longer duration.

Impact of endovascular repair for abdominal aortic aneurysms in octogenarians.

A total of 50 consecutive patients (86% male; median age, 82 years) underwent endovascular repair of abdominal aortic aneurysms (AAAs) ranging from 4.0 to 9.0 cm (median, 5.2 cm). Efficacy of aneurysm exclusion was assessed by angiography, duplex scan, and/or contrast-enhanced computed tomography (CT). Acute technical success was 82%. Access failed in one patient, and immediate conversion to open operation was required in two patients. Improper deployment of the endoluminal graft (ELG) across the renal arteries occurred in one patient. The median operation time, estimated blood loss, packed red blood cells received, contrast volume, and length of intensive care and hospital stay were 128 min, 200 mL, 0.1 unit, 297 mL, 0.9 days, and 3 days, respectively. ELG limb thrombosis was seen in one patient. There were 4 (8%) early endoleaks, and 2 endoleaks were discovered in other patients at 3 and 6 months. Local/vascular and remote/systemic postoperative complications were seen in 13 (26%) and 9 (18%) patients, respectively. At a median follow-up of 11 months (range 2 to 36 months), clinical success was 78%. The aneurysm sac diameter (n = 49) decreased from a preoperative median of 5.2 to 4.7 cm (p = 0.0001). Technical success was high, and results at 11 months were satisfactory. Long-term outcomes require further study.

Trianthenol: an antifungal tetraterpenoid from Trianthema portulacastrum (Aizoaceae).

An antifungal tetraterpenoid named trianthenol 1 has been isolated from the chloroform extract of Trianthema portulacastrum. Its structure was established as 15-hydroxymethyl-2,6,10,18,22,26,30-heptamethyl-14-methylene- 17-hentriacontene on the basis of spectroscopic data including high resolution mass and two-dimensional NMR techniques. A benzaldehyde derivative 2, a pentacyclic triterpenoid 3 and benzoic acid derivatives 4-5 are also reported for the first time from Trianthema portulacastrum.

Ursene type nortriterpene from Debregeasia salicifolia.

A new triterpene, 3beta-19alpha-dihydroxy-30-norurs-12-ene (1), has been isolated from the methanolic extract of Debregeasia salicifolia stems. Its structure has been elucidated with the help of one- and two-dimensional NMR and NOE studies. Lupeol, beta-sitosterol, stigmasterol and oleanolic acid are also reported for the first time from this species.

CRB1 mutations may result in retinitis pigmentosa without para-arteriolar RPE preservation.

PURPOSE: To report a new phenotype in retinitis pigmentosa (RP) patients with CRB1 mutations at the RP12 locus. PATIENTS: Thirty-seven patients from two Pakistani families with severe retinitis pigmentosa. METHODS: Samples were screened with single-strand conformation polymorphism analysis followed by DNA sequencing of the coding sequence of the CRB1 gene. RESULTS: Two novel CRB1 mutations were discovered. No patients had evidence of preservation of the para-arteriolar retinal pigment epithelium (PPRPE) that has been previously reported in all cases of RP associated with CRB1 mutations. CONCLUSIONS: Patients with severe autosomal recessive (or simplex) RP who lack the finding of PPRPE should not be excluded from molecular analysis of CRB1 purely because they lack the clinical feature of PPRPE. This report illustrates that RP at the RP12 locus is not clinically uniform. The absence of PPRPE cannot be used to exclude CRB1 as a potential molecular explanation for RP.

Triterpenes from Mimusops elengi.

Pentacyclic triterpenes (1) and (2) have been isolated from Mimusops elengi and assigned structures 3beta,6beta,19alpha,23-tetrahydroxy-urs-12-ene and 1beta-hydroxy-3beta-hexanoyllup-20 (29)-ene-23, 28-dioic acid, respectively, on the basis of spectroscopic studies including 2D-NMR. The compound 1 showed moderate inhibiting activity against beta-glucuronidase enzyme

Daphwazirin, biscoumarin glycopyranoside from Daphne oleoides.

Daphwazirin, a new biscoumarin glycoside (1), along with four known compounds (2), (3), (4) and (5) has been isolated from the roots of Daphne oleoides. Its chemical structure is established as 7-hydroxy-8-[2-oxo-2H-1-benzopyran-7'-(O-alpha-L-rhamnosyl)-6'-yl]-2H-1-benzopyran-2-one through spectroscopic techniques and chemical analysis.

Biodesulphurization of coal: mechanism and rate limiting factors.

The pyrite sulphur removal from coal by Thiobacillus ferrooxidans was studied in batch reactor. A combination of SEM, IR and XRD was used to study the presence of superficial phases and the changes in solid surface during biodesulphurization. Biodesulphurization was found to be a three-step process. In the first step (0-4 days), direct oxidation of pyrite by bacteria brought about 28% pyritic sulphur removal. Both direct and indirect oxidation contributed to the second step (4-10 days) resulting in 51% pyrite removal. The deposition of elemental sulphur, jarosite and ferric sulphate precipitates in the third step reduced the pyrite availability and ferric iron concentration in the leachate and brought the process of biodesulphurization to an end.

Beta blocker eye drops related airway obstruction.

OBJECTIVE: To assess the awareness among chest physicians, ophthalmologists and patients about use of eye drops with particular reference to beta blockers such as timolol as an agent of aggravating breathlessness in predisposed subjects. DESIGN: Cross-sectional study. METHOD: A survey of 20 chest physicians, 20 ophthalmologists and 200 patients suffering from obstructive airway disease was conducted in 2 hospitals and a welfare center of Karachi from August to September 1997. RESULTS: The results indicate that ophthalmologists showed more awareness than chest physicians regarding use of beta blocker eye drops by patients with obstructive airways disease (p = 0.004). Patient awareness was low as well. CONCLUSION: There is a need to update doctors and educate general public about the side effects of beta blocker eye drops in patients with obstructive airway disease.

Sol-gel monolithic columns with reversed electroosmotic flow for capillary electrochromatography

Sol-gel chemistry was used to prepare porous monolithic columns for capillary electrochromatography. The developed sol-gel approach proved invaluable and generates monolithic columns in a simple and rapid manner. Practically any desired column length ranging from a few tens of centimeters to a few meters may be readily obtained. The incorporation of the sol-gel precursor, N-octadecyldimethyl[3-(trimethoxysilyl)propyl]ammonium chloride, into the sol solution proved to be critical as this reagent possesses an octadecyl moiety that allows for chromatographic interactions of analytes with the monolithic stationary phase. Additionally, this reagent served to yield a positively charged surface, thereby providing the relatively strong reversed electroosmotic flow (EOF) in capillary electrochromatography. The enhanced permeability of the monolithic capillaries allowed for the use of such columns without the need for modifications to the commercial CE instrument. There was no need to pressurize both capillary ends during operation or to use high pressures for column rinsing. With the developed procedure, no bubble formation was detected during analysis with the monolithic capillaries when using electric field strengths of up to 300 V cm(-1). The EOF in the monolith columns was found to be dependent on the percentage of organic modifier present in the mobile phase. Separation efficiencies of up to 1.75 x 10(5) plates/m (87,300 plates/column) were achieved on a 50 cm x 50 microm i.d. column using polycyclic aromatic hydrocarbons and aromatic aldehydes and ketones as test solutes.

Normalization strategies for cDNA microarrays.

Multiple Arabidopsis thaliana clones from an experimental series of cDNA microarrays are evaluated in order to identify essential sources of noise in the spotting and hybridization process. Theoretical and experimental strategies for an improved quantitative evaluation of cDNA microarrays are proposed and tested on a series of differently diluted control clones. Several sources of noise are identified from the data. Systematic and stochastic fluctuations in the spotting process are reduced by control spots and statistical techniques. The reliability of slide to slide comparison is critically assessed within the statistical framework of pattern matching and classification.

Absence of scaling for the intermediate scattering function of a hard-sphere suspension: static and dynamic x-ray scattering from concentrated polystyrene latex spheres.

X-ray photon correlation spectroscopy and small-angle scattering measurements are presented of the dynamics and structure of concentrated suspensions of charge-stabilized polystyrene latex spheres dispersed in glycerol, for volume fractions from 3% to 52%. The static structures of the suspensions show essentially hard-sphere behavior, and the short-time dynamics shows good agreement with predictions for the wave-vector-dependent collective diffusion coefficient. However, the intermediate scattering function is found to violate a scaling behavior found previously for a sterically stabilized hard-sphere suspension.

Intellectual psycho-educational and functional status of low birth weight survivors beyond 5 years of age.

To determine the intellectual, psycho-educational and functional status of low birth weight (LBW) survivors (birth weight < or = 2000 gms) beyond 5 years of age and to compare their status with normal birth weight counterparts. This was cross-sectional study. On hospital based cohort on longitudinal follow-up at the High Risk and Well Baby Clinics of a teaching hospital. The demographic data of these children was recorded. A detailed physical examination was performed. The tests of cognition included the Stanford Binet and the Raven's Progressive Matrices. Academic achievement was evaluated by the Wide range achievement test-Revised (WRAT-R). Assessment of visuo motor integration was done by the Bender Gestalt Test. The proportion of children having soft neurological signs was determined. Vineland Social Maturity Scale was performed on all children. Fifty-nine LBW children and fifty-seven matched control children participated in the study. 27 of LBW children were examined at a mean age of 7.0 +/- 1.1 years (group I) and 32 were examined at a mean age of 10.6 +/- 1.2 years (group II). The LBW children as a group performed in the normal range on the tests of cognition and academic achievement, but were significantly disadvantaged (p < 0.005) as compared to controls. A higher percentage of LBW children had low scores on the Bender Gestalt. Test as compared to controls but the difference was not significant. A significantly higher proportion of LBW children of both the groups showed the presence of soft neurological signs as compared to controls. The social quotient as assessed by the vineland Social Maturity Scale was significantly lower in the LBW children as compared to controls. Thus, though the LBW children were performing in the normal range on various measures, comparison with the control group showed that they were clearly disadvantaged on nearly every measure tested, emphasizing the need for early detection and referral for special education.
