[Transcranial Doppler sonography and hemodynamics].

13 patients without cerebral or cerebrovascular diseases were investigated by transcranial Doppler sonography during Swan-Ganz catheterisation for cardiologic purposes. Massive increases of mean arterial pressure, heart rate and cardiac index were observed during exercise; flow velocities in the middle cerebral artery increased only to the same extent as the endexpiratory carbon dioxide rose. However, the pulsatility index increased significantly (p less than 0.01) although a decrease should have been expected due to carbon dioxide accumulation. These results are indicative of a functioning autoregulation independent of carbon dioxide; they are relevant for interpretation of TCD results in patients with disturbed autoregulation.

[Risk of contrast medium reactions in cardiac diagnosis. Pretreatment with glucocorticoids and antihistaminics in known cases of intolerance to contrast media].

Among 4178 patients who between 1980 and 1986 had undergone left-heart catheterization with left-heart injections and coronary angiography there were 76 (1.8%) with previous reactions to contrast media. These latter patients were given, three days before the planned investigation, 6 alpha-methylprednisolone, 24 mg daily by mouth, and phenhydramine hydrogenmaleinate, 150 mg daily, and two hours before the investigation 80 mg 6 alpha-methylprednisolone hemisuccinate intravenously. The effect of this prophylactic regimen was tested prospectively. Diatrizoate 76% was the contrast medium used. Of 4102 patients without known contrast-medium intolerance 137 (3.34%) had a reaction, 27 of them (0.66%) severe enough to require treatment. Among the 76 patients with known previous reactions, nine (11.8%) had reactions, one very severe requiring treatment, the others mild. The described pre-injection regimen thus allows indicated left-heart contrast-medium injection to be undertaken at a justifiable risk.

[Ventricular arrhythmias in cor pulmonale. The effect of oxygen treatment].

Long-term ECG monitoring was carried out on 36 patients with chronic obstructive airway disease and cor pulmonale. Ventricular extrasystoles were detectable in all patients with greater than 30/h occurring in 13. Multifocal ventricular extrasystoles were seen in 29 patients, couplets in 16, runs in 6 and early (R-on-T) extrasystoles in 8 patients. When the ventricular extrasystoles exceeded 30 per hour the average oxygen partial pressure (paO2), determined in capillary blood, was 59.5 mm Hg, whereas it was higher, 66.4 mm Hg (P less than 0.0125), when there were less than or equal to 30/h. Oxygen (2 l/min) was administered to 13 patients between 20.00 h and 08.00 h and then room-air the following night via a nasal tube. After the administration of oxygen mean paO2 was 72.3 mm Hg and after breathing room-air 62.6 mm Hg (P less than 0.01). During the administration of oxygen, extrasystoles appeared less frequently than after breathing room-air (927 vs 1211; P less than 0.05). These results show that ventricular extrasystoles appear more often with decreasing paO2 and that oxygen therapy can reduce their frequency.

[Protease inhibitors, serum endotoxin and serum immunoglobulins following portacaval end-to-side anastomosis in animal experiments].

In order to clarify the pathophysiological mechanism of certain biochemical and immunological changes (endotoxin in serum, protease inhibitors, immunoglobulins) found in a former study on human cirrhosis of the liver the porto-caval end-to-side anastomosis of rats with unaffected livers was chosen as test model. With the aid of this bypass the "spill-over" phenomenon of the liver can be completely imitated. In this study, 7 operated and 5 or 14 control animals resp. are referred to. Serum endotoxin, acid-stable and acid-unstable protease inhibitors and immunoglobulins IgG, IgA and IgM were determined 20 months after operation. For the determination of potential hemodynamically or toxically induced effects on these organs, morphologic liver and lung examinations were performed. On the average, the operated rats showed a weight loss of 8 percent, i.e. from 378.4 +/- 9.2 g to 348.4 +/- 17 g. Compared to control rats, their relative liver weights were significantly lower (34%) (mean = 2.23 +/- 0.2 compared to 3.4 +/- 0.44 g, p less than 0.0005). Serum immunoglobulins IgG, IgA and IgM in operated animals were significantly higher (p less than 0.005 or p less than 0.025 resp. and 0.0025). Endotoxin in serum could be identified in 4 out of 7 operated animals (57, 1%), but in none of the control animals. While there was no difference in serum levels of acid-unstable protease inhibitors between the two groups levels of acid-stable protease inhibitors were in operated animals by 24% higher than in control animals (mean = 35.3 +/- 3.3 compared to 28.5 +/- 2.3 mU/ml, p less than 0.0005).(ABSTRACT TRUNCATED AT 250 WORDS)

[Left coronary artery arising from the pulmonary artery. Demonstration by 2-dimensional echocardiography].

The authors report the case of a baby in whom an ectopic left coronary artery arising from the pulmonary artery was suspected on clinical and, in particular, electrocardiographic criteria. M mode echocardiography showed left ventricular dilatation with reduced shortening fraction of the LV internal diameter. 2D echocardiography with a mechanical sector scanner demonstrated the left coronary artery arising from the pulmonary artery. The short axis view of the basal vessels in the left parasternal incidence visualised 2 cm of the left coronary artery arising from the posterior part of the pulmonary artery.

[Comparison of left ventricular function in children hemodialyzed with bicarbonate and acetate. An echocardiographic study].

Left ventricular function before and after hemodialysis with acetate (A) and hemodialysis with bicarbonate (B) was investigated in 8 children on regular dialysis treatment. The decrease in left ventricular volume overload is similar in A and B. There is a significant increase in shortening fraction (B: 9,7 +/- 16,1%; p less than 0,05. A: 7,8 +/- 10,9%; p less than 0,05), in velocity of circumferential fiber shortening (Vcf) (B: 30,6 +/- 16,5%; p less than 0,0005. A: 23,5 +/- 16,6%; p less than 0,0025) and in corrected Vcf (B: 19,3 +/- 16,4%; p less than 0,0025. A: 13,6 +/- 11,4%; p less than 0,005). The ratio of pre-ejection period with the left ventricular ejection time (PEP/LVET) is increased significatively only after hemodialysis with bicarbonate. This difference may be due to a better left ventricular function in hemodialysis with bicarbonate than with acetate.

[Experience with 208 "screw-in" type pacemaker leads with particular reference to atrial positioning (author's transl)].

In 190 patients, 208 "screw-in" type pacemaker leads were implanted; 147 in the right ventricle and 61 in the right atrium. There was no significant difference in the time required for atrial (54.7 +/- 19 min) or ventricular fixation (56.0 +/- 25 min). Similarly, fluoroscopy times for both atrial (6.3 +/- 4 min) and ventricular positioning (8.1 +/- 6 min) were equivalent. In the subsequent follow-up period ranging from one to 23 months, only one dislocation of a "screw-in" type atrial lead was seen, and that in a patient with "twiddler syndrome". Otherwise, in patients in whom the lead was securely implanted, no evidence of dislocation has been observed. In one patient "under-sensing" developed subsequent to repositioning for exit-block. In this patient population, the use of "screw-in" type electrodes, in particular through facilitation of atrial positioning, has substantially lessened the rate of pacemaker complications.
