Localized Wegener's granulomatosis: predominance of CD26 and IFN-gamma expression.

The immune response in Wegener's granulomatosis (WG) has been characterized as a predominant, potentially pathogenic Th1-like reaction by blood T cells and T-cell clones from diseased tissues. To elucidate further the immunopathogenic mechanisms, this study analysed the phenotypes of inflammatory infiltrates in frozen nasal biopsies with involvement of the upper respiratory tract only (localized or 'initial phase' WG) and with multi-organ involvement, including systemic vasculitis (generalized WG). The expression and production of Th1 and Th2 cytokines were examined in tissue specimens and peripheral blood mononuclear cells (PBMCs) of localized and generalized WG. The number of CD3+ T cells in inflammatory infiltrates ranged from 50 to 70%, together with approximately 30% CD14+ monocytes/macrophages. An average of 40% of T cells expressed CD26 in nasal biopsies of localized WG, compared with about 16% in specimens of generalized WG. In parallel, a higher number of interferon-gamma (IFN-gamma)-positive cells were detected in nasal tissue of localized than in generalized WG. PBMCs from localized WG similarly exhibited higher spontaneous IFN-gamma production in contrast to generalized WG (207 vs. 3 pg/ml, p<0.05). Interleukin-4 (IL-4) mRNA was found in higher amounts in generalized than in localized WG. IL-4 production was negligible in both disease and controls. In addition, both IL-10 mRNA and IL-10 protein levels of activated PBMCs from localized WG were elevated when compared with generalized disease (574 vs. 154 pg/ml, p<0.05) or healthy controls (574 vs. 246 pg/ml, p<0.05). It is conluded that in nasal tissues, mainly CD4+/CD26+ T cells as well as IFN-gamma-positive cells may support a polarized Th1-like immune response. Furthermore, the data suggest that this in situ immune response is already initiated and established in localized WG, accompanied by increased peripheral IFN-gamma and IL-10 production.

Ribosomal RNA of Nosema algerae and phylogenetic relationship to other microsporidia.

Microsporidia are intracellular parasites that are common in invertebrates. Taxonomic classification is mostly restricted to morphologic and physiologic data. Limited data are available about taxonomic classification using DNA-sequence data for analysis. We examined the small-subunit (SSU) rDNA, the intergenic spacer (ITS) region, and a part of the large-subunit (LSU) rDNA of Nosema algerae, a parasite of mosquitoes, taken from a laboratory colony of Anopheles stephensi. Target gene amplifications were done by polymerase chain reaction (PCR) and, after cloning, DNA fragments were sequenced. The SSU-rDNA sequence obtained was aligned with several other microsporidian SSU-rDNA sequences available from the GenBank or EMBL data bases and was analyzed by different methods. On the basis of the results of our phylogenetic analysis, we suggest that our N. algerae isolate is not closely related to other microsporidia belonging to the genus Nosema.

Taxonomic position of the human intestinal protozoan parasite Isospora belli as based on ribosomal RNA sequences.

The taxonomic positions of Isospora belli and other members of the genus Isospora are controversial. We determined the small-subunit ribosomal RNA of I. belli and used this sequence in combination with other coccidian RNA sequences for analysis of the taxonomic position of I. belli. The phylogenetic trees we obtained provide molecular evidence for three clades within a monophyletic group that represents the suborder Eimeriina. The clade containing I. belli consists of tissue-cyst-forming coccidia (Toxoplasma and Neospora) and members of the genus Isospora (I. ohioensis, I. suis, I. belli). The second clade, representing a sister clade of that containing the Isospora species, contains members of the genus Sarcocystis. The third one consists of members of the family Eimeriidae, including Eimeria and Cyclospora species. This shows that although I. belli as well as other members of the genus Isospora belong to the suborder Eimeriina, the family to which they belong is not Eimeriidae but rather Sarcocystidae. We suggest that the genus Isospora should be removed from the family Eimeriidae and placed into the family Sarcocystidae within the suborder Eimeriina.

[Sponge anesthesia with intraocular lidocaine application in cataract surgery].

BACKGROUND: For cataract surgery in this country retro- and peribulbar anesthesia is the standard method. It totally stops eye movement as well as sensitivity to pain. Especially peribulbar anesthesia is known as a procedure with minimal complications, but the potential risks can be severe. In the past sponge anesthesia could not be used on patients with difficult cataract surgery due to the insufficient anesthetic effect. A modified anesthetic method is demonstrated for complicated cataract surgery. PATIENTS AND METHODS: In 281 non-selected, successive phacoemulsifications, all performed by the same surgeon, we applied sponge anesthesia and 1.5 mg midazolam as premedication. Only in cases of moderate and severe sensitivity was 1% 0.15 ml lidocaine applied intraocularly. The intensity of sensitivity was graded as 1 for low, 2 moderate, and 3 severe. RESULTS: Ninety eyes had complicated cataract surgery. Of the patients, 84.3% felt no pain during surgery; 11.4% were part of group 2 (moderate). Ten of 12 patients who had stated moderate sensitivity at first were free of pain after intraocular application of lidocaine, while the other 2 patients only felt minor sensitivity. CONCLUSION: The anesthetic procedure we present offers a safe alternative method to surgeons who are able to do surgical procedures on a mobile eye.

Indolic constituents and indole-3-acetic acid biosynthesis in the wild-type and a tryptophan auxotroph mutant of Arabidopsis thaliana.

The tryptophan auxotroph mutant trp3-1 of Arabidopsis thaliana (L.) Heynh., despite having reduced levels of L-tryptophan, accumulates the tryptophan-derived glucosinolate, glucobrassicin and, thus, does not appear to be tryptophan-limited. However, due to the block in tryptophan synthase, the mutant hyperaccumulates the precursor indole-3-glycerophosphate (up to 10 mg per g FW). Instability of indole-3-glycerophosphate leads to release of indole-3-acetic acid (IAA) from this metabolite during standard workup of samples for determination of conjugated IAA. The apparent increase in "conjugated IAA" in trp3-1 mutant plants can be traced back entirely to indole-3-glycerophosphate degradation. Thus, the levels of neither free IAA nor conjugated IAA increase detectably in the trp3-1 mutant compared to wild-type plants. Precursor-feeding experiments to shoots of sterile-grown wild-type plants using [2H]5-L-tryptophan have shown incorporation of label from this precursor into indole-3-acetonitrile and indole-3-acetic acid with very little isotope dilution. It is concluded that Arabidopsis thaliana shoots synthesize IAA from L-tryptophan and that the non-tryptophan pathway is probably an artifact.

Binding of human lipoproteins (low, very low, high density lipoproteins) to recombinant envelope proteins of hepatitis C virus.

Heterogeneities in the density of hepatitis C virus (HCV)-RNA-carrying material from human sera (1.03-1.20 g/ml) are partially due to the binding of lipoproteins [low density (LDL), very low density (VLDL), high density (HDL) lipoproteins] and immunoglobulins. In this study we demonstrate the binding of recombinant HCV envelope protein (El/E2) to human LDL, VLDL and HDL on a molecular basis. The binding of lipoproteins was restricted to the middle part of the El gene product (amino acids 222-336) and the C-terminal part of the E2 protein (amino acids 523-809). Lipoproteins did not bind to recombinant HCV core protein.

[Health care utilization in patients with Fibromyalgia Syndrome (FMS)].

BACKGROUND: The purpose of the study was to examine the pain related health care utilization in patients with fibromyalgia syndrome. METHODS: In a multicenter study six rheumatological hospitals associated in the rheumatological center of Heidelberg participated. All consecutive patients which fulfilled the ACR-criteria of fibromyalgia were assessed. Data about pain and related treatments were available from n=259 patients. RESULTS: Patients suffered from chronic generalized pain on average for 5 years before being diagnosed as fibromyalgia. In most of the cases longlasting localized pain symptoms preceeded generalized pain. The spectrum of pain related treatments seems to be wide and arbitrary: 87% took questionable or excessive medication. 86% had some form of physiotherapy and 41% already experienced psychological treatment. 28% of the patients consulted dental surgeons including teeth extraction and 20% underwent multiple orthopaedic surgery. A mean of five different health care professionals were consulted regarding the pain problem. Treatment satisfaction rated by the patients was lowest with pharmacological and highest with psychological treatment and physiotherapy. CONCLUSIONS: The health care-seeking behavior of fibromyalgia-patients seems to be characterized by a multitude of different treatment attempts. Treatment was often invasive and focused on localized symptoms rather than on the generalized pain syndrome. An early confirmation of the diagnosis and a more evidence based treatment is strongly indicated.

Closure of recurrent frontal skull base defects with vascularized flaps -- a technical case report.

Techniques for vascularized reconstruction of the anterior cranial fossa floor defects causing recurrent cerebrospinal fluid fistula are discussed in this report. The closure employs the use of local random- or axial-pattern vascularized flaps in simple cases. In complicated cases (for instance, status after repeated exploration) the tissue of the cranial base is severely compromised and shows low potential for healing. Non-vascularized grafts only add avital scars to the already present ones leading to recurrent fistulas. Free vascularized flaps show more mechanical strength and less scar contraction, resistance to infections and survive better in a compromised surrounding, thus leading to long term sealing in such cases. The technical issues of vascularized closure of defects of the frontal skull base are discussed in this report.

Detection of Isospora belli by polymerase chain reaction using primers based on small-subunit ribosomal RNA sequences.

The aim of the present study was to use small-subunit (SSU)-rRNA sequences of Isospora belli to design specific primer pairs and a hybridization probe for the detection of Isospora belli in human samples by PCR and Southern blot hybridization. PCR amplification with the primer pairs produced correct DNA fragments with target DNA from samples of Isospora belli-infected patients and from cloned SSU-rRNA of Isospora belli. The nature of the PCR products was confirmed by Southern blot hybridization. No amplification was seen with template DNA extracted from other parasites. Although Isospora belli infections can be easily diagnosed using light microscopy, molecular-based techniques may prove useful as an additional diagnostic tool.

Synthetic HLA-A2 derived peptides are recognized and presented in renal graft recipients.

Indirect presentation of allogeneic MHC antigen is an important pathway by which allografts are rejected and tolerance maintained by regulatory CD4(+) T cells. In this study HLA-A2 derived synthetic peptides were used to determine whether T cells of non-HLA-A2 renal graft recipients, which had been HLA-A2 mismatched to their organ donors, recognize some of the HLA-A2-derived peptides. Among the HLA-A2 mismatched patients, 60% recognized residues 56--69, 65--79, and 75--89. Peripheral blood lymphocytes derived from healthy individuals showed low reactivity towards allopeptides, indicating that sensitization towards HLA-A2 induced response towards HLA-A2 derived peptides. The response to the peptides was blocked by antibodies to HLA-DR, -DQ, and CD4. Depletion of antigen presenting cells abrogated response towards the allopeptides, confirming that the observed proliferation was mediated by the indirect pathway. Interestingly, although none of the HLA-A2 mismatched patients had any signs for either acute or chronic rejection, considerable response to allo-derived HLA-A2 was observed.

Projection neurons in the superficial layers of the superior colliculus in the rat: a topographic and quantitative morphometric analysis.

The present study deals with qualitative and quantitative investigations in the superior colliculus of the rat. Tracer studies were correlated with Nissl staining to calculate the quantitative ratio between projection neurons and interneurons in the upper three layers of the superior colliculus. In order to reveal the projections from the superior colliculus, the first group of rats received injections of the tracer FluoroGold into the nucleus lateralis posterior thalami, the lateral geniculate body, the nucleus parabigeminalis, and the predorsal bundle. Commissural connections between the superior colliculus were traced in a second group of animals, which received Biocytin and FluoroGold injections in the upper layers of the right superior colliculus and small deposits of the carbocyanine tracer DiI in the deeper layers of the left superior colliculus. Additionally, double-labelling with FluoroGold tracing and the histochemical detection of NADPH-diaphorase activity was carried out to distinguish between projection neurons and interneurons. These experiments showed that 66% of the neurons within the superficial layers of the superior colliculus were represented by ascending projection neurons, whereas only 2-3% could be identified as descending neurons. Ascending neurons were scattered throughout the three laminae and descending neurons were localized in a cluster-like pattern. Approximately 2-3% of the neurons in the superficial layers were found to be commissural and interlayer neurons which were represented by an identical cell type, since both transcommissural and interlayer processes were originated from their somata. The somata of these commissural-interlayer neurons were all located in the mediorostral part of the superior colliculus and contained NADPH-diaphorase activity. The axon terminals of the interlayer-commissural neurons formed net-like structures which surrounded neuronal somata within the ipsilateral deep layers and within the contralateral upper layers of the superior colliculus, respectively.

Effect of chronic angiotensin II inhibition on the cardiovascular system of the normal rat.

Previous studies have demonstrated in normal rats that chronic treatment, from weaning to 30 days, with either enalapril or losartan, induced significant changes in cardiovascular structure and function. The present study was performed to assess the effect of either enalapril or losartan on the structure and function of the heart and arteries given to normal rats from weaning until 6 months of age. Animals (n = 48) were divided into three groups: control, enalapril treated, and losartan treated; treated rats received 10 mg/kg/day of drug. Blood pressure, body weight, and water intake were recorded for that time period. DNA, cGMP, collagen, degree of fibrosis, and nitric oxide synthase-NADPH-diaphorase-dependent activity in the heart and arteries were determined. Only significant differences (P < .05) are reported. Blood pressure increased only in control rats (13 +/- 1 mm Hg), enalapril treatment enhanced water intake and reduced the rate of body growth (control, 672.9 +/- 15.4 g; losartan, 692.4 +/- 21.8 g; enalapril, 541.8 +/- 13.8 g). In the heart, DNA (control, 120 +/- 5; losartan, 99 +/- 4; enalapril, 93 +/- 6 microg/100 mg), collagen (control, 2.5 +/- 0.2; enalapril, 1.85 +/- 0.08 microg/100 mg), and fibrosis (control, 3.5 +/- 0.4%; losartan, 2.2 +/- 0.3%; enalapril, 2.1 +/- 0.4%) were reduced by treatment. In the aorta, cGMP (control, 0.15 +/- 0.01; losartan, 0.24 +/- 0.02 pmol/mg), and NADPH-diaphorase (control, 0.114 +/- 0.003; losartan, 0.148 +/- 0.006; enalapril, 0.169 +/- 0.003 as optical density) were enhanced. The enzyme was also higher in the aortic endothelium of treated animals (control, 0.193 +/- 0.010; losartan, 0.228 +/- 0.009; enalapril, 0.278 +/- 0.005). The lower rate of body weight increase, the enhanced water intake, and the reduced cardiac and left ventricular weight attributable to enalapril treatment do not seem to be related to inhibition of the renin-angiotensin system. On the other hand, renin-angiotensin system inhibition induces a protective effect on the heart and aorta through structural and functional changes. Most of this action seems to be exerted through angiotensin II type 1 receptors.

Light and confocal laser-scanning microscopical evidences for complementary patterns of glial fibrillary acidic protein and Wisteria floribunda agglutinin labeled structures in human and rat brain.

We investigated the pattern of glial fibrillary acidic protein (GFAP) and Wisteria floribunda agglutinin (WFA) labeled structures in the superior colliculus and in the somatosensory cortex of humans and rats of different age groups using immunohistochemical methods, light and confocal laser-scanning microscopy. We never found a double labeling of WFA and GFAP positive structures neither in the superior colliculus nor in the cortex of both man and rat. The complementary pattern of WFA and GFAP labeling was present both at the macroscopic and microscopic level. We found a clear prevalence of either WFA or GFAP expression in the arborization of the astrocytes as well as in the pattern of lamination.

Human herpesvirus type 8 in salivary gland tumors.

BACKGROUND: The new human herpesvirus type 8 (HHV-8) has been detected in all types of Kaposi's sarcomas, as well as in body-cavity lymphomas and Castleman's disease, furthermore molecular biologic studies have identified a number of potential viral oncogenes. There is evidence for sexual transmission of HHV-8 in HIV-seropositive patients, but the route of infection among the HIV-seronegative population is uncertain. Findings of HHV-8 DNA in saliva in some cases are suggestive of nonsexual transmission associated with latent infection of the salivary gland (as it is known for EBV, CMV, HHV-6 and HHV-7). OBJECTIVE: As little is known about the etiological factors of salivary gland tumors and to give more insights into HHV-8 cell tropism normal salivary gland tissue (n=12) and different salivary glands neoplasm (n=58) were tested for HHV-8 sequences and antigens in HIV-seronegative patients. STUDY DESIGN: Biopsies of both normal salivary gland and tumors were investigated for HHV-8 sequences. A nested-PCR method was used for amplification of HHV-8 DNA fragments and the nature of the amplification products was confirmed by Southern blot hybridization. In addition, we used an in situ hybridization technique and immunohistochemical staining for detection of HHV-8 infected cells. The sera of the respective patients were tested for anti-HHV-8 antibodies using commercial IFA and an ELISA-assay. RESULTS: HHV-8 DNA sequences could be detected in one bilateral MALT-lymphoma of the parotid gland of a HHV-8 seropositive female patient suffering from Sjögren's syndrome (SS). The remaining parotid samples did neither show HHV-8 sequences nor HHV-8 antigens. Using above assays only one additional patient was seropositive for HHV-8. CONCLUSION: Our data suggest that HHV-8 does not usually infect the salivary gland in HIV-seronegative patients and does not seem to play a pathogenic role in vascular and epithelial salivary gland neoplasm. Pathogenic role of HHV-8 in Sjögren's syndrome associated MALT-lymphoma remains unclear and should be subject of further studies.
