[The use of an optical polarization method for the evaluation of oculomotor muscle function in vertical deviation].

The state of oculomotor muscles in vertical deviation has been studied by means of polarioptic method. A control group included 62 healthy persons, of them 24 (48 eyes) with orthophoria, 19 (38 eyes) with esophoria and 19 (38 eyes) with exophoria. A group of patients with vertical deviation included 39 (78 eyes) with concomitant convergent squint with a vertical component and 9 children (18 eyes) with concomitant vertical squint without a horizontal component. In patients with vertical squint, the polarioptic method revealed high frequency of functional muscular imbalance and anatomic asymmetry of muscles of both horizontal and vertical action. It is shown that in case of convergent squint with a vertical action, not only the frequency is higher, but to a much greater degree increases the value of anatomic asymmetry in the position of horizontal muscles.

[The diagnostic importance of studying the periphery of the fundus oculi in the central vitreoretinal edematous fibroplastic syndrome].

Examination of 75 persons with a clinical picture of central vitreoretinal edematous fibroplastic syndrome (150 eyes) has shown dystrophic changes in the equatorial and peripheral zones of the eye fundus in all persons. It was also recorded that patients with edematous fibroplastic process in the central zone accompanied by vitreoretinal tractions often develop equatorial dystrophies, this being a risk factor of retinal detachment. A conclusion is made that it is important to examine the eye fundus periphery and equator in patients with central vitreoretinal edematous fibroplastic syndrome.

[Photopsia in the form of "side lightning" as a symptom of posterior detachment of the vitreous and vitreo-retinal pathology].

In 78 of 103 patients with various vitreo-retinal pathology a peculiar photopsy has been revealed, which was called by the authors photopsy in a form of "side lightning". It appeared as bright arc-shaped light flashes in the periphery of the temporal part of visual field. It is stated that this symptom characterizes posterior detachment of the vitreous, which, in its turn, accompanies many vitreo-retinal diseases. Among 78 patients, 14 had retinal detachment, 52--central vitreo-retinal fibroplastic syndrome, 10--idiopathic macular apertures. In all cases photopsy in a form of "side lightning" was one of leading subjective symptoms. Correct assessment of this symptom favoured a timely diagnosis of vitreo-retinal pathology.

[The importance of a new compression test in the diagnosis of preglaucoma and initial glaucoma].

A new compression test with an especially constructed mechanic oculopressor allows to make both functional and tonometric measurements during the test. The method was used in 48 eyes of healthy persons and 196 eyes with advanced, initial glaucoma, preglaucoma and ocular hypertension. The results obtained have shown that the test is effective for detection of preglaucoma and ocular hypertension as well as for differential diagnosis of initial glaucoma and ophthalmic hypertension by the analysis of changes of the blind spot area, the time of restoration of its sizes to the initial state and dynamics of annular compression coefficients at the time of the test.

[Significance of the intolerance index in glaucoma].

In 1979 Vodovozov proposed a new individualized index-the intolerance index-for the prognosis of glaucoma. It is calculated as the difference between tonometric and tolerable intraocular pressure, determined after the intake of hypotensive drugs. The lowered tonometric value which coincides with the maximum improvement in visual function (reduced size of the blind spot, enlarged visual field) is considered the individually compatible (tolerable) level of intraocular pressure. The tolerable intraocular pressure and the intolerance index were determined in 379 eyes of 213 patients with open-angle glaucoma; the follow-up period ranged from 18 months to 5 years. Statistical evaluation of the data obtained showed a significant difference between the values for the intolerance index of glaucomatous eyes with stabilized and nonstabilized visual function. In stabilized glaucoma the intolerance index is at most 4 mm Hg. If this is exceeded, one ought to intensify medication or resort to surgery. Lobstein's index (the difference between the mean blood pressure in the central retinal artery and the intraocular pressure) was found to be unreliable in assessing the prognosis.

[Individually consistent (tolerable) intraocular pressure in glaucoma].

The author has devised a technique for determining tolerable intraocular pressure; after the intake of hypotensive drugs the intraocular pressure and the optic functions (size of the blind spot, visual fields, visual acuity) are periodically checked. The lowered tonometric value which coincides with the maximum improvement in visual function is considered the individually compatible level of intraocular pressure. Determination of the individually tolerable pressure is important for diagnosis, prognosis, and treatment. Actual intraocular pressure must not exceed the tolerable pressure by more than 5 mm Hg, otherwise the prognosis will be poor. Differential diagnosis between glaucoma, ocular hypertension, and low-tension glaucoma can be established by determining the tolerable pressure.

[Clinical picture of an embolism of the central retinal artery caused by cholesterol crystals].

On the basis of six relevant personal observations the ophthalmoscopic signs of an embolism in the retinal vessels caused by cholesterol crystals are described. In the fundus characteristic red and yellow iridescent light reflexes from the crystals and muff formation around the afflicted vessels can be observed. Fluorescence angiography illustrates the grade of impediment to the blood-circulation. Embolism caused by cholesterol crystals in the retina is a bad prognostic sign with respect to the patient's life.
