Budesonide: treatment of bronchial asthma during childhood.

Nowadays inhaled corticosteroids are the first step in treating inflammation of the airways. It is evident that these drugs are not free of side effects; however, the fact that at present the most commonly used method is by inhalation and that these drugs undergo metabolic changes in the lung, makes them safe in that a part of the side effects, which are observed when they are administered systemically, disappear. Budesonide was one of the last corticosteroids for topical application to appear on the market and its powerful anti-inflammatory action together with in versatility (it can come in aerosol form using MDI or in solution for nebulizers as well as in dry powder) make it possible to use during all stages of a child's development. In 75 children treated with Budesonide over a long period (18-24 months) we were able to evaluate the drug's clinical efficacy in the reduction in number and intensity of attacks registered together with their accompanying symptoms. Significantly (p < 0.001) it were able to restrict the bronchodilatory medication needed to control their asthma. Similarly, we were able to verify that the FEV1 was normalized and that the Peack Flow was stabilized. We were not able to find any interruption in the hypothalamo-hypophysio-suprarenal axis and the weight-height curves continued to evolve chronologically. With regards to the side effects, three cases of colonization candidiasis were recorded through there were no clinical repercussions and treatment did not need to be suspended.

[Supplementary criteria of evaluation of health status in selecting the personnel for work in coal mines].

200 newly-engaged young miners were medically examined in an outpatient department. The routine examination techniques were supplemented with studies of the changes after 2 minute-long inhalations of 0.6% acetylcholine chloride and mine dust (78 mg/m3), hemodynamics and electrocardiography indices, as well as the indices of the immunologic and hormonal state. Established was diversified manifestations of the respiratory tract's response to the aerogenic irritations, combined with deviations in the immunologic state indices. The studies provide basis for further improvements in the preventive examination techniques for workers in coal-mining industries.

Ultrasonographic diagnosis of portal vein cavernous transformation in children.

The aim of this work was to evaluate the efficiency, sensitivity, and specificity of ultrasonographic findings in the diagnosis of portal vein cavernous transformation and secondary splanchnic hypertension in children. Twenty patients with esophageal varices on endoscopic sclerotherapy, with ages ranging from 1 to 15 years (mean of 7.5), were compared with 20 age-matched normal asymptomatic controls. In 17 cases, diagnosis was confirmed by splenoportography. Ultrasonographic portal vein features, respiratory variations in splenic and superior mesenteric veins, lesser omentum to aorta diameter ratio and spleen size, as well as the presence of vessels in the lesser omentum and of spontaneous anastomoses, were analyzed in both groups. The efficiency of ultrasonography to establish a diagnosis of portal vein cavernous transformation reached 100%. Splenic and superior mesenteric vein respiratory variation was negligible in patients and inspiration versus expiration diameter differences ranged from 1 to 4 mm (mean of 2.4) in controls (p less than .001). Mean lesser omentum to aorta ratio was 2.07 in patients versus .9 in controls (p less than .001). Splenomegaly was found in all but two cases; lesser omentum vessels were visualized in nine cases; whereas five presented spontaneous splenorenal anastomoses and gallbladder varices were observed in five patients. Thus, ultrasonography allowed portal vein cavernous transformation to be diagnosed throughout; portal hypertension was also detected. It may be concluded that ultrasonography, either alone or associated with endoscopy, provides a reliable method for portal vein cavernous transformation diagnosis. Angiography should be reserved for preoperative or controlled hemodynamic studies.

Electromyography of the rectum and colon in Hirschsprung's disease.

The electromyographic activity of the large bowel was studied in patients suffering from aganglionic megacolon and in comparable normal infants and children, using of an intraluminal electrode. Graphs obtained in control children showed a basic electrical rhythm of 4-6 waves/min with bursts of high frequency spikes, 3-4/sec, superimposed on the top of the slow waves. The recordings in Hirschprung's disease showed an irregular basic electrical rhythm with waves of variable duration and no high frequency potentials, as long as the electrode was positioned in the aganglionic zone. As soon as the electrode was introduced further up into the normally innervated colon the graph became identical to that seen in the normal bowel. Intraluminal electromyography of the rectum and colon seems to be a suitable procedure to diagnose aganglionosis of the rectum and the extent of the lesion.

[The physiological basis of the compensatory-adaptive significance of expiratory bronchospasm].

A purposeful analysis of home and foreign literature has been made, its results being inconsistent with generally accepted notion of a bronchial spasm as a pathogenetic sign and so impossible to be logically interpreted. The analysis supports the supposition on the possibility of the manifestation of the expiratory bronchial spasm as a compensatory-adaptative mechanism directed to increase the intrathoracic pressure, alveolar-capillary diffusion of oxygen and oxygenation of the aerated blood in the case of hypoxemia. Results of examination of 30 miners with the expiratory bronchial permeability in the presence of the hypoxic and load-induced hypoxemia give grounds to confirm existence of the breathing regulation mechanism and to elaborate a differential approach for estimation of the bronchial spasm in patients with pulmonological diseases.
