Seven new mutations in the nicotinamide adenine dinucleotide reduced-cytochrome b(5) reductase gene leading to methemoglobinemia type I.

Cytochrome b(5) reductase (b5R) deficiency manifests itself in 2 distinct ways. In methemoglobinemia type I, the patients only suffer from cyanosis, whereas in type II, the patients suffer in addition from severe mental retardation and neurologic impairment. Biochemical data indicate that this may be due to a difference in mutations, causing enzyme instability in type I and complete enzyme deficiency or enzyme inactivation in type II. We have investigated 7 families with methemoglobulinemia type I and found 7 novel mutations in the b5R gene. Six of these mutations predicted amino acid substitutions at sites not involved in reduced nicotinamide adenine dinucleotide (NADH) or flavin adenine dinucleotide (FAD) binding, as deduced from a 3-dimensional model of human b5R. This model was constructed from comparison with the known 3-dimensional structure of pig b5R. The seventh mutation was a splice site mutation leading to skipping of exon 5 in messenger RNA, present in heterozygous form in a patient together with a missense mutation on the other allele. Eight other amino acid substitutions, previously described to cause methemoglobinemia type I, were also situated in nonessential regions of the enzyme. In contrast, 2 other substitutions, known to cause the type II form of the disease, were found to directly affect the consensus FAD-binding site or indirectly influence NADH binding. Thus, these data support the idea that enzyme inactivation is a cause of the type II disease, whereas enzyme instability may lead to the type I form.

Relationships between tissue concentrations of polycyclic aromatic hydrocarbons and antioxidative responses of marine mussels, Perna viridis.

Local mussels, Perna viridis, were transplanted from a relatively clean site to various polluted sites in Hong Kong. After a 30-day field exposure, different antioxidant parameters including glutathione S transferase (GST), superoxide dismutase (SOD), catalase (CAT), glutathione peroxidase (GPx), glutathione reductase (GR), NADPH DT-diaphorase (DT-d), glutathione (GSH) and lipid peroxidation were quantified, and tissue concentrations of benzo[a]pyrene (B[a]P) as well as a total of five polycyclic aromatic hydrocarbons (PAHs) with potential carcinogenicity were determined for individual mussels. Results indicated that: (1) tissue concentrations of B[a]P and total PAHs from the same site were highly variable; (2) gill SOD, DT-d and lipid peroxidation showed no response to tissue pollutants; (3) the majority of the antioxidant parameters were induced by increasing tissue pollutant concentrations; and (4) amongst the various parameters, oxyradical scavenger GSH best correlated with tissue concentrations of pollutants.

A polymorphism in the coding region of interleukin-13 gene is associated with atopy but not asthma in Chinese children.

BACKGROUND: Interleukin (IL)-13 is an important cytokine secreted from type 2 helper T lymphocytes. It is essential for modulating IgE synthesis by human B cells. Previous studies showed that polymorphisms in the IL-13 gene were associated with serum total IgE or allergic asthma. The relationship of this marker with sensitization to individual aeroallergens has not been evaluated. OBJECTIVE: We tested whether a polymorphism in the coding region of the IL-13 gene is associated with asthma and atopy in asthmatic children in Hong Kong. METHODS: We used restriction fragment length polymorphism to detect R130Q genotype in Chinese children with asthma and control subjects. Serum total IgE was measured by microparticle immunoassay and specific IgE to common aeroallergens was measured using fluorescent enzyme immunoassay. Pulmonary function studies were performed using spirometry. RESULTS: One hundred and fifty-seven patients and 54 control children were recruited. Their mean serum total IgE concentrations were 994 kIU/L and 473 kIU/L, respectively (P < 0.0001). Atopy as defined by > or = 1 positive RAST was found in 141 patients and 32 control children. The GlnGln form of the R130Q polymorphism in the IL-13 gene was associated with serum total IgE (P = 0.005) as well as specific IgE to Der p 1 (P = 0.021), mixed cockroaches (P = 0.03) and dog (P = 0.003) but not with physician-diagnosed asthma (P = 0.621). In addition, the R130Q polymorphism did not correlate with subjective or objective indicators of asthma severity in our patients. CONCLUSION: Our results suggest that the R130Q polymorphism of the IL-13 gene is associated with elevated serum total and allergen-specific IgE but not asthma in Chinese children.

Obstructive sleep apnoea in children with adenotonsillar hypertrophy: prospective study.

OBJECTIVE: To determine clinical and baseline polysomnographic data on obstructive sleep apnoea secondary to adenotonsillar hypertrophy in Hong Kong Chinese children. DESIGN: Prospective study. SETTING: University teaching hospital, Hong Kong. PARTICIPANTS: Fifty children (35 boys and 15 girls) suspected to have obstructive sleep apnoea were recruited between January 1999 and December 1999. MAIN OUTCOME MEASURES: Symptoms questionnaire, electrocardiogram, chest radiograph, and full-night polysomnography. RESULTS: All patients had symptoms suggestive of obstructive sleep apnoea. None were found to have clinical evidence of cor pulmonale. Forty-five (90%) of 50 children had obstructive sleep apnoea with a respiratory disturbance index of greater than five. Central and mixed apnoeas were rare. Tonsil size did not correlate with the severity of obstructive sleep apnoea. CONCLUSION: Symptoms and signs suggestive of obstructive sleep apnoea can lead to a high detection rate and confirmation of obstructive sleep apnoea by polysomnography.

[The role of Anopheles anthropophagus in malaria transmission in in Xinyang City of Henan Province].

OBJECTIVE: To study the role of Anopheles anthropophagus in malaria transmission and transmission threshold so as to provide basis for vector surveillance and malaria control strategy. METHODS: Parasitological and entomological methods were used in the investigation at 5 villages of Xinyang City, Henan Province. RESULTS: From July to August, 1999, 74 febrile cases (10.9% of the total population) were examined. Among them 50 were infected, the incidence in the population of surveyed spots was 7.4%. Active detection was made in another randomly selected two villages and found that the parasite rate in the inhabitants was 2.0%, and the positive rate of IFA was 8.4%. Only vivax malaria was detected. An. anthropophagus and An. sinensis were collected, with An. anthropophagus as the predominant one in human dwellings. The estimated man-biting rate and the human blood index were 4.9388 and 0.7858 respectively. The vectorial capacity of An. anthropophagus was 5.5296. The critical man-biting rate of An. anthropophagus was 0.2407 as calculated by the formula (ma = -rlnP/abPh) according to Macdonald's model. The local man-biting rate was 20 times higher than that of the critical man-biting rate. CONCLUSION: The results demonstrated that An. anthropophagus is the principal vector in malaria transmission in the area. The findings imply that the critical man-biting rate is of practicable importance in vector surveillance.

Allergen sensitisation in asthmatic children: consecutive case series.

OBJECTIVE: To investigate the pattern of sensitisation to aero-allergens and food allergens among asthmatic children in Hong Kong, and to investigate any correlation between atopy and the severity of asthma. DESIGN: Consecutive case series. SETTING: Paediatric out-patient clinic of a university teaching hospital, Hong Kong. PARTICIPANTS: Two hundred and four consecutive children with physician-diagnosed asthma who attended from January 1999 through June 2000. MAIN OUTCOME MEASURES: Demographic data, questionnaire assessment of asthma control, spirometric evaluation, response to skin prick testing, eosinophil count, and total serum immunoglobulin E concentration. RESULTS: The median patient age was 8.2 years (range, 5.7-11.3 years), and the median 1-second forced expiratory volume was 95% of the predicted value. The median absolute eosinophil count in the peripheral blood was 0.48 x 10(9) /L and the ratio of total serum immunoglobulin E to the age-adjusted upper limit of the normal range was 2.7. Atopy, as defined by at least one positive response to skin prick testing, was found in 170 (83.3%) of the 204 patients. House dust mites were the most commonly sensitised aero-allergen (n=167; 81.9%). Sensitisation to food allergens was found in 47 (23.0%) of the patients. The self-reported frequency of asthmatic attacks was associated with a positive response to skin prick testing with animal allergens (P for trend = 0.001), whereas spirometric indices correlated with the degree of atopy and the presence of in vivo cockroach-specific immunoglobulin E. CONCLUSION: Sensitisation to indoor aero-allergens, as determined by skin prick testing, is prevalent among Chinese children with mild-to-moderate asthma, whereas sensitisation to food allergens is not. The severity of asthma is correlated with skin prick test responses to pet and cockroach allergens.

Recovery of Salmonella by using selenite brilliant green sulfa enrichment broth.

The efficacy and sensitivity of selenite brilliant green sulfa enrichment (SBG) broth for the isolation of Salmonella from fecal specimens were evaluated by using both clinical and artificially infected (artificial) fecal specimens. An examination of 1,588 clinical fecal specimens found Salmonella in 296 specimens, including 89 cases detected by the direct-plating xylose-lysine-desoxycholate method and an additional 207 cases detected after enrichment with SBG broth. Therefore, the recovery of Salmonella with SBG broth is increased 3.3-fold over that by the direct-plating method alone. Furthermore, the isolation rate of Salmonella is higher when using SBG broth than when using gram-negative (GN) broth or GN broth supplemented with sodium selenite. To determine the sensitivity for the recovery of Salmonella, artificial specimens containing various amounts of Salmonella were prepared and analyzed. The results indicated that the sensitivity is also higher with SBG broth than with GN broth. Moreover, the optimal incubation period for SBG broth can be extended to 24 h. In conclusion, the SBG enrichment method provides a higher recovery rate of Salmonella from fecal specimens.

Interrater and intrarater reliability in the measurement of kyphosis in postmenopausal women with osteoporosis.

STUDY DESIGN: A reliability study was performed using repeated random measurements involving three observers, 26 subjects and three instruments. OBJECTIVES: To determine the most reliable, cost-effective, noninvasive, and clinically feasible method of measuring spinal kyphosis. SUMMARY OF BACKGROUND DATA: The most clinically useful, noninvasive and reliable method of measuring postural deformity in spinal osteoporosis (kyphosis) remains unqualified. Despite traditional use of costly, invasive roentgenographs for the evaluation of spinal kyphosis, the reliability of this method remains questionable. METHODS: Twenty-six postmenopausal women with known bone mineral density and a diagnosis of osteoporosis were recruited from the Osteoporosis Program at Women's College Hospital, Toronto, Canada. Non-invasive measurements of thoracic kyphosis were obtained by three trained examiners using the DeBrunner's kyphometer and the flexicurve ruler. The intrarater and interrater reliability of and between each method was compared, using roentgenographic films obtained in the sagittal plane. Spinal posture was classified according to the method of Itoi (1990). Statistical computations were performed using SAS statistical software. RESULTS: Consistent measurements were obtained with the DeBrunner's kyphometer and the flexicurve ruler by each observer, according to the results of critical two-way analysis of variance (Intraclass Correlation Coefficient 2, 1). Measurements in two subgroups, healthy backs (n = 11) and rounded backs (n = 13), showed consistent use of each noninvasive instrument with some examiner preference for specific tools. There was marginally better intrarater and interrater reliability using the DeBrunner's kyphometer compared with that obtained with the flexicurve ruler. Two-way analysis of variance (Intraclass Correlation Coefficient 2, 1) of collapsed data showed no significant difference in the reliability of the kyphometer, flexicurve ruler, or roentgenographs in the measurement of thoracic kyphosis. CONCLUSIONS: The flexicurve ruler and DeBrunner's kyphometer had the closest agreement in the measurement of spinal kyphosis. The kyphometer demonstrated the least variation in intrarater and interrater reliability when compared with the flexicurve ruler and roentgenographs. The flexicurve ruler permits qualitative assessment of posture, however, and is the most cost-effective instrument. The results of this study challenge the traditional belief that roentgenographic analysis is the best method for evaluating spinal kyphosis. The DeBrunner's kyphometer and flexible ruler may represent viable, cost-effective and noninvasive alternatives to roentgenographic evaluation of spinal kyphosis.

A cross-sectional study of growth, puberty and endocrine function in patients with thalassaemia major in Hong Kong.

METHODOLOGY: A cross-sectional study of growth, puberty and endocrine function was performed on 35 girls and 33 boys with thalassaemia major. RESULTS: Despite regular transfusion and chelation therapy, 75% of the girls and 62% of the boys over the age of 12 years were below the third percentile for height. Hypogonadotropic hypogonadism was found in a similar percentage of patients. Moderate to marked zinc deficiency secondary to chelation therapy was considered unlikely because normal serum zinc levels were found in all but three of our patients, but we could not exclude the possibility of a marginal status of zinc nutrition causing growth failure. Growth hormone deficiency and diabetes mellitus were sometimes encountered but hypothyroidism, hypoparathyroidism and adrenal insufficiency were rare among our patients. Most of the patients with growth failure had normal growth hormone (GH) response to insulin induced hypoglycaemia. The serum insulin-like growth factor-1 (IGF-1) levels were low in our patients and no significant difference in the serum IGF-1 levels was found between prepubertal children with or without growth failure (0.4 +/- 0.1 mU/mL vs 0.37 +/- 0.11 mU/mL, P = 0.39). Similarly, no difference in the serum IGF-1 levels was found between pubertal children with or without growth failure (0.48 +/- 0.2 U/mL vs 0.56 +/- 0.14 U/mL, P = 0.26). CONCLUSIONS: Delayed sexual maturation and a possible defect in growth unrelated to the GH-IGF-1 axis may be responsible for the growth failure in adolescent children with thalassaemia major.

[A study on the changes of myocardial intracellular calcium in patients with heart failure].

Myocardial intracellular calcium was studied in patients with chronic rheumatic valvular disease. Calcium deposit localized by pyroantimonate method was significantly decreased or disappeared on plasmic membrane and was markedly increased in cytoplasma, intramitochondria and I- and A- bands of myofilament in patients with heart failure due to chronic rheumatic heart valvular disease. The degree of intracellular calcium deposit was consistent with the injury of myocardial ultrastructure, suggesting that calcium overload may be one of the major causes of myocardial injury and the angiotensin-converting enzyme inhibitors, beta-adrenergic blockers and calcium channel blockers may be useful for protecting myocardium and improving cardiac function.

Hepatitis A and B infections in transfusion-dependent thalassaemia from endemic areas.

The aim of this study was to determine the prevalence of previous hepatitis A virus (HAV) and B virus (HBV) infection which is in 64 transfusion-dependent (TD) patients with thalassaemia including 26 patients who were transfused before blood donors were screened for HBV. Serial blood samples taken from these 64 patients and 10 non-TD beta-thalassaemia intermedia patients during a 3 year period, were tested for antibody to HAV (anti-HAV), hepatitis B surface antigen (HBsAg), antibody to HBsAg (anti-HBs), antibody to core antigen (anti-HBc) and when indicated, antibody to Delta virus (anti-Delta) and HBV DNA. Liver function tests were performed also. Similar tests were conducted on 50 donor blood units. None of the 64 TD patients had evidence of past HAV infection, but 50% of blood donors had evidence of past infection (P less than 0.001). Only 2 brothers and their mother were positive for HBsAg, and 38 patients (59.4%) had persisting HBV antibodies compared with 26% of blood donors (P less than 0.001). Our TD thalassaemic patients acquired passive immunity from donor plasma, which protected them against HAV and possibly modified the outcome of HBV infection.
