[X-ray endovascular embolization of the hepatic artery in the palliative treatment of hepatocellular cancer].

X-ray endovascular embolization of hepatic artery (HAE) was carried out in 10 cases of liver cell carcinoma (hepatic cirrhosis included). Immediate beneficial effect was recorded in 8 cases. Remission for 12 months and more was registered in three cirrhosis-free patients; mean survival in five patients was 15 months. In cases of cancer aggravated with cirrhosis, HAE effectiveness proved much lower and survival dropped to average 5.5 months.

[Clinico-endoscopic evaluation of the effectiveness of the treatment of uncomplicated gastroduodenal ulcers after local administration of solcoseryl and aevit].

The complex of conservative measures for noncomplicated peptic ulcers included curative endoscopy in 105 patients, in 61 of them Aevitum was used as an application with additional injections of Solcoseryl around the ulcers. In 44 patients Aevitum was used along with Solcoseryl which was injected into the submucous layer around the pathological focus of the stomach and duodenum. The results obtained gave a convincing evidence of advantages of interstitial administration of Aevitum allowing to considerably shorten time of treatment (up to 10-12 days) of noncomplicated ulcers of the stomach and duodenum.

[Priority therapeutic methods in hepatology, gastroenterology and urologic oncology].

The author briefly describes endovascular surgical methods; some of them were developed and used in clinical practice for the treatment of portal hypertension, liver cirrhosis and tumors, renal and bladder cancer. Besides, priority methods of endoscopic surgery employed in the treatment of peptic ulcer, are described.

[Embolization of the hepatic artery in the treatment of malignant tumors of the liver complicated by jaundice].

Embolization of the hepatic artery (EHA) was performed in 7 patients with malignant tumors of the liver. Before EHA all the patients had an increased level of bilirubin and transaminase of blood serum. After performing EHA their levels decreased within 5-12 days in 6 patients, the average survival of the patients was 14 months. One patient died 5 days after EHA from hepatic insufficiency resulting from necrosis of the massive tumor. The conditions for safe performing of EHA under conditions of hyperbilirubinemia are discussed.

[Scintigraphic evaluation of the results of treatment of alcoholic liver cirrhosis].

The results of static and dynamic scintigraphy in patients with alcoholic liver cirrhosis showed changes in liver sizes, a decrease in the RL/LL ratio, an increase in spleen sizes, the reduction or distortion of the L/S ratio, extrahepatic uptake of RP, and predominance of the arterial component of the blood flow over the portal one. However the main scintigraphic signs of alcoholic liver cirrhosis were undetectable. After therapy (lymphovenous anastomosis, embolization of the hepatic artery) the recovery of functions was more rapid in patients with cirrhosis of alcoholic etiology who discontinued alcohol consumption than in those with cirrhosis of some other etiology.

[Embolization of the splenic artery in liver cirrhosis].

An experience with 70 embolizations of the splenic artery in 62 patients points to the importance of differential approach in choosing this method of treatment. Of primary importance in solving this problem is thought to be the degree of the disease which must determine the variant of reduction of the splenic bloodflow.

[Secondary splenomegaly in cholestasis and disorders of portal blood flow in an experiment detectable by intravenous liposome splenography].

The paper is concerned with the results showing high diagnostic potentialities of i.v. liposome lienography for the detection of the splenomegalic syndrome in acute cholestasis, cholestatic hepatitis and disturbances of the extrahepatic portal blood flow. The authors managed to establish the dependence of the value of secondary splenomegaly on the period of cholestasis and a degree of occlusion of the portal vein. In acute cholestasis splenomegaly was manifested in 5 days by a two-fold increase in the spleen with reverse development within 2.5 mos. A rapid 2-fold increase in the spleen for 24 h was noted in occlusion of the portal vein by 40%, and a three-fold increase was noted in 5 mos. with subsequent death of the animals. A decrease in the extrahepatic portal blood flow by 25% also resulted in splenomegaly but without death of the animals. The authors have assumed that the use of i.v. liposome lienography ensures timely diagnosis of the splenomegalic syndrome and provides an opportunity to assess function of the reticuloendothelial system of the spleen.

[Catheterization of the visceral segment of the internal iliac artery for long-term infusion of hemostatic and chemotherapeutic drugs in inoperable cancer of the bladder].

The method described was used for the intraarterial selective hemostatic therapy and resulted in a persistent arrest of bleeding in 8 of 10 patients while surgical ligation of the internal iliac arteries for hemostasis proved to be of little effect in 7 of 15 patients with bleeding carcinoma of the urinary bladder. After performing selective continuous monochemotherapy (with dioxadet, cis-platinum or adriamycin) of inoperable local carcinoma of the urinary bladder, the full or partial effect was achieved in 6 of 12 patients, and progress of the disease was noted in one patient only.

[Long-term selective infusion of cis-diamminedichloroplatinum into the internal iliac arteries in the treatment of bladder cancer].

Long-term selective infusion of 70-120 mg/m2 cis-diamminedichloroplatinum into internal iliac arteries was used in 20 cases of advanced bladder cancer (T2-3, T3a-5, T3b-4 and T4-4 patients). X-ray endovascular catheterization of arteries was performed in 16 and surgical chronic one--in 4 cases. Complete remission was recorded in 1, partial remission--10 and stabilization--in 9 patients. Pronounced symptomatic effect was observed. No serious complications were registered. The procedure requires further study.

[Ligation and prolonged catheterization of the internal iliac arteries in hemorrhaging bladder cancer].

Internal iliac arteries were ligated in 15 cases of extensive cancer of the urinary bladder to arrest persistent bleeding. A stable effect was obtained in 8 cases only. The procedure of maintenance of a permanent catheter in the visceral part of internal iliac artery suggested by the authors was successively used to stimulate hemostatic effect in the remaining 5 cases. The procedure was carried out in conjunction with intraarterial injection of hemostatic agents.

[Roentgenological-endovascular occlusion and ligation of internal iliac arteries in bleeding tumors of the bladder].

A comparative estimation of the effectiveness of hemostasis by means of ligation of the internal iliac arteries and roentgen-endovascular occlusion of these vessels was carried out in 30 patients with a bleeding carcinoma of the urinary bladder and prostate growing into its wall. For the occlusion of the internal iliac artery metallic spirals were used in 15 patients, in 4 of them they were used in combination with a gelatine sponge. The roentgen-endovascular occlusion is thought by the authors to be the method of choice.
