Zinc concentration in umbilical cord tissue and cord plasma in appropriate-for-gestational-age babies.

Zinc concentration in cord plasma was determined in 148 healthy appropriate-for-gestational-age babies delivered at various gestational ages. Zinc concentration in cord tissue was also determined in 62 of these babies. There was a significant rise in the cord tissue zinc concentration just after the 35th week of gestational age; thereafter the levels remained relatively unchanged. The cord plasma zinc concentration decreased slightly with increasing gestational age, but these changes were not significant. There was a statistically significant positive correlation between cord tissue zinc concentration and birth weight. No correlation was found between cord plasma zinc concentration and birth weight or cord tissue zinc concentration.

Poor antenatal attendance and obstetric performance.

A survey of attendance for antenatal care at a public hospital in Brisbane was carried out. "Poor attenders" were more likely to be young, unmarried, grande multiparous, or aboriginal, and to live in a poorer area of the City. This group comprised 6.5% of the total population, but accounted for 14.6% of all low Apgar scores, 23% of all low birth weight babies, 23.1% of all stillbirths, and 18.2% of all neonatal deaths.

Control chart for urinary oestriol excretion during pregnancy.

Calculations to define the distribution of normal urinary oestriol excretion values during pregnancy are presented. The values had a normal distribution, uniform variance and increased linearly with time after logarithmic conversion. The variance could, therefore, be partitioned and a control chart constructed.

A comparative trial of haematinic supplements in pregnancy.

Four haematinic supplements--Slow-Fe, FGF, Ferro-Gradumet and Ferro-Grad tfolic--were prospectively studied in 103 healthy pregnant patients. Significant differences in the mean cell counts, haematocrit values, haemoglobin levels, iron-binding capacities and serum folate levels were noted. The changes paralleled the elemental iron present in the compount. In this series, side effects principally vomiting and constipation, were more frequent in the lower dose iron group. Form both the haematological status aspect and the lack of side effects, Ferro-Grad Folic and Ferro-Gradumet were superior to both FGF and Slow-Fe (old formulation).

Salivary estradiol and progesterone during the normal ovulatory menstrual cycle in Chinese women.

Salivary and serum levels of estradiol and progesterone were measured by radioimmunoassay in 10 Chinese women during their normal menstrual cycles. Changes in salivary estradiol and progesterone levels followed a similar pattern to that in the serum. Significant correlation was found between salivary and serum levels of estradiol and progesterone (p less than 0.001). Measurements of these salivary steroids may be used to assess follicular dynamics. Moreover, salivary sampling is simple, convenient and stress free.

Premenstrual syndrome in employed Chinese women in Hong Kong.

Premenstrual syndrome (PMS) has been studied in many countries, but few studies have been reported internationally on the experience of Chinese women. Because culture and employment are important factors in the perception of health status, in this study we sought to determine the existence and features of PMS in Chinese clerical women in Hong Kong. We used a cross-sectional, retrospective approach to collect data with a translation of an established questionnaire (the Menstrual Distress Questionnaire). Fatigue was found to be the most prevalent symptom, and the Pain, Water Retention, Behavioral Change, and Negative Affect scales had more than 64% frequency. The main difference between these findings and those of other studies is that negative affect featured most prominently in Western samples, whereas pain featured most highly in this sample of Chinese women.

Use of microdissected airways to define metabolism and cytotoxicity in murine bronchiolar epithelium.

The use of the mouse for carcinogenesis bioassays has raised questions regarding the cell of origin of lung tumors. Since a feature of chronic lung injury from aromatic hydrocarbons is an apparent alteration in target cell susceptibility, the present study was designed to test the feasibility of using microdissected pulmonary airways to evaluate the metabolism and cytotoxic response of one of the potential targets of pulmonary carcinogens, the bronchiolar Clara cell. Airways were microdissected from mouse lungs that had been filled by injection of agarose (1%) into the trachea. Ultrastructural integrity of the explants has been maintained for up to 8 h in culture. The cytotoxic response of bronchiolar epithelium in explants incubated with naphthalene (0.5 mM) was identical to the vacuolation and exfoliation observed in bronchioles of mice 24 h after intraperitoneal administration of naphthalene (100 or 300 mg/kg). Pre-incubation of the explants with piperonyl butoxide, a cytochrome P-450 monooxygenase inhibitor, prevented naphthalene-induced cytotoxicity. Naphthalene monooxygenase activity was easily measurable in all levels of airway, including trachea, lobar bronchi, major and minor daughter pathways, and distal bronchioles. No metabolism was detected in lung parenchyma or large vessels. Dihydrodiol and a glutathione adduct derived from 1R, 2S-naphthalene oxide were the sole metabolites detected by HPLC in incubations of airway explants. Formation of a single diastereomeric glutathione conjugate indicated that the metabolic epoxidation of naphthalene was highly stereoselective. Glutathione S-transferase activity was measured in all compartments, with the highest activities in trachea and lowest in distal bronchiole and pulmonary vein. Explants maintained pools of reduced glutathione for up to 4 h in culture. We conclude that microdissected airways have excellent potential for: (1) defining the capability of bronchiolar epithelium to catalyze xenobiotic biotransformation, (2) comparing activity in target and nontarget lung compartments as a means of identifying specific metabolic pathways associated with the cytotoxic response, and (3) use with a variety of species, including nonhuman primates and humans, as a means of providing appropriate data for extrapolation of effects in the intact animal to the human, where bioassay is not possible.

Experience of palliative home care according to caregivers' and patients' ages in Hong Kong Chinese people.

PURPOSE/OBJECTIVES: To identify the relationship between family caregivers' reported difficulty in managing caregiver tasks and ages of caregivers and patients. DESIGN: Cross-sectional, descriptive survey. SETTING: A hospice homecare program in Hong Kong. SAMPLE: Twenty-nine Chinese family caregivers who had experienced at least weekly caregiving responsibility for more than two months and were able to read and understand Chinese. METHODS: Respondents completed a caregiver task inventory. Four homecare nurses assisted in the distribution and collection of questionnaires. MAIN RESEARCH VARIABLES: Caregiver tasks and age and patient age. FINDINGS: Caregivers' age was negatively correlated with reported difficulty in overall tasks and in interpersonal ties. The patients' age was negatively correlated with reported difficulty in direct care to patients, intrapersonal tasks, and overall tasks. CONCLUSIONS: The younger the caregiver, the more difficulty he or she experienced in the caregiving role, particularly in the maintenance of social and family ties. Caregivers of younger patients experienced more difficulty in most aspects of caregiving tasks. More research with a larger sample size is required to fully investigate the effect of age on the family caregiving experience and the validity of the caregiver task inventory. IMPLICATIONS FOR NURSING PRACTICE: Nursing support and preparation to younger caregivers and caregivers of younger patients are suggested in the practice of palliative home care.
