Study of 5-hydroxytryptamine (serotonin) in pericardial fluid in different causes of death (II.). Experimental study of 5-HT levels in two types of shocks (hemorrhagic and septic) in dogs.

In the present work we studied the levels of 5-HT in the pericardial fluid of 160 cadavers according to cause of death; such as myocardial infarction, violent asphyxia, pulmonary embolism, infections, bronchopulmonary diseases, traumatic and hemorrhagic diseases in the CNS, and multiple traumatism. We did not find significant differences in the various causes of death. A complementary study of 20 dead dogs having suffered induced shock, either hemorrhagic or septic, was made. In both series we studied the serum levels of 5-HT and the following parameters: systolic and diastolic arterial pressures, central venous pressure, hematocrit value, pH value, total proteins, albumin, PO2, and PCO2. We found the significant correlation (r = 0.828, P less than 0.01) only between 5-HT serum levels and the systolic arterial pressure in the hemorrhagic shock.

The postmortem determination of CK isozymes in the pericardial fluid in various causes of death.

The authors have studied the CK isozyme pattern in the pericardial fluid of 100 cadavers autopsied in the Anatomic Forensic Institute of Granada. The samples were classified into several groups according to the cause of death: --Multiple trauma with thoracic contusion --Pneumonia and pulmonary embolism --Mechanical asphyxia --Cranio-cerebral trauma --Acute haemorrhage --Myocardial infarction --Others. The results showed that the CK isozyme pattern of pericardial fluid provides useful postmortem information of cardiac "status", adding to the diagnostic potential of CK isozymes.

[Behavior of 5-hydroxytryptamine and 5-hydroxyindoleacetic acid in relation to agonized suffering (I)].

The authors have studied the levels of 5-HT and 5-HIA,--in serum, CSP fluid and pericardial fluid of cadavers at the Forensic Anatomic Institute of Granada and its relations with the length of agonized suffering. The results showed us that the study of 5-HT and its main metabolite, the 5-HIAA, may be useful for the exact evaluation of agonized suffering, and the pericardial fluid is most suited for the analysis of 5-HT and 5-HIAA. We found that there was a significant correlation between the 5-HT and 5-HIAA and the agonized suffering. When the agonized sufferings of a moderate length (10 min-6h) were compared with the slow and long processes of death, the correlation was highly significant (P less than 0.001).

Biological monitoring of occupational exposure to isoflurane by measurement of isoflurane exhaled breath.

The relationship between isoflurane environmental concentrations in operating rooms and the corresponding isoflurane concentration in the exhaled air of the operating personnel at the end of the exposure has been investigated. Isoflurane was retained in an adsorbent cartridge and after thermal desorption the concentration was estimated by gas chromatography. Significant correlation between environmental and exhaled air isoflurane concentrations allowed the establishment of a biological exposure index and biological exposure limits corresponding to proposed atmospheric threshold values.

Correlation between concentrations of n-hexane and toluene in exhaled and environmental air in an occupationally exposed population.

We determined the correlations between the concentrations of n-hexane and toluene in exhaled and environmental air in the shoe manufacturing industry. Data were collected in 1988 and in 1992 from a total of 265 subjects. Environmental air samples were collected with personal diffusive samplers by adsorption on activated charcoal during exposure and from end-expired air (alveolar air) on cartridges of activated charcoal after exposure. Both compounds were desorbed with carbon disulphide and analysed by gas chromatography. Linear regression analyses showed a good correlation between environmental and end-expired air concentrations (r = 0.82 for n-hexane and r = 0.81 for toluene). These correlations allowed us to calculate the concentrations in expired air corresponding to current environmental limit values. The calculated concentrations in end-expired air that correspond to current environmental threshold limit values of 176 mg m-3 for n-hexane and 377 mg m-3 for toluene are 28 mg m-3 (95% confidence limit, 27-29 mg m-3) and 40 mg m-3 (95% confidence limit, 39-41 mg m-3), respectively. Similar correlations were found when the data from the two study periods were analysed separately.

The determination of Ck, LDH and its isoenzymes in pericardial fluid and its application to the post-mortem diagnosis of myocardial infarction.

After having studied 144 pericardial fluids, from the same number of judicial corpses, we propose the measurement of CK for the postmortem diagnosis of myocardial infarction, since it is a reliable, exact and easy technique available to an ordinary laboratory. Providing that contaminated samples be disregarded, the fluids are centrifuged early, and the cadavers be under 48 hours old, the study of CK enzymatic levels in the pericardial fluid allows the postmortem differentiation of myocardial infarction from other causes of death.

Aminopeptidase and cathepsin A activity in vitreous humor in relation to causes of death.

Brain autolysis happens rapidly, especially when environmental temperatures are high, and poses serious limitations for evaluating damage using morphologic methods. In the present study we have measured total proteins, cathepsin A and aminopeptidase activity in the vitreous humor in relation to cause of death and survival time. We have studied vitreous humor samples from 106 cadavers autopsied in the I.A.F. of Granada. The samples were classified according to causes of death as follows: myocardial infarction; hanging; other mechanical asphyxias; multiple trauma; craniocerebral trauma; other violent death; pulmonary functions; and other natural deaths. Total protein was measured by Lowry's method, and cathepsin A and aminopeptidase activity by the Bowen and Davison (1973) and Greenberg (1962) methods, respectively. Results are expressed in IU/l and in mIU/mg of protein. We found higher values of aminopeptidase and cathepsin A activity in groups with severe brain damage (craniocerebral trauma, multiple trauma, etc.) and lower values in groups of natural deaths. We believe, therefore, that aminopeptidase activity in vitreous humor may be a useful parameter for evaluating brain damage.

Comparison of different techniques for the postmortem diagnosis of myocardial infarction.

A study was performed on 74 medicolegal autopsy cases for the purpose of comparing the reliability of four different techniques (haematoxylin-eosin stain, acridine orange method, formazan test and K/Na ratio) used for the postmortem diagnosis of myocardial infarction. Although the results showed that the formazan test and K/Na ratio give a good reliability to rule out a myocardial infarction, the most specific technique for the diagnosis of infarct is the formazan test.

Syngeneic islet transplantation into seminal vesicles of diabetic rats.

Pancreatic islet transplantation has been proposed as an attractive option for the treatment of type I diabetes. Transplantation into different sites has been investigated, among them those that are immuno-logically privileged (e.g., thymus, uterus, brain, anterior eye chamber, and testicle). Because of their characteristics, seminal vesicles could be considered as immunologically privileged organs, but there is no worldwide experience that can confirm it. The purpose of the present study is to assess the viability and functionality of islet transplantation into seminal vesicles of diabetic rats. One hundred ninety inbred adult male syngeneic Lewis rats were used as donors (n = 72), receptors (n = 36), and controls(n = 11). Diabetes was chemically induced through a single intraperitoneal injection of streptozotocin. Groups of 1200 purified islets were introduced in the right seminal vesicle of diabetic rats. Diabetic control rats were sham transplanted. Body weight and glycemia were monitored every 2 d. Of transplanted rats, 16.7% achieved a good function due to islet engraftment, while 30.6% achieved a partially good response, and 52.7% were considered as nonresponding. This is the first report about islet transplantation into seminal vesicles of diabetic animals. Our results indicate that islet transplantation into rat seminal vesicles is technically possible, and that islets can function normally after engraftment into the wall of the seminal vesicle.

Detection methods of the drug-addiction and alcoholism treatment programme of the Spanish National Railway Company (RENFE).

BACKGROUND: Social problems in enterprises need to be approached by all groups involved in order to achieve a satisfactory result. With regard to Occupational Health and, specifically, drug-addiction, the groups concerned are Occupational Health Services, Trade Unionists and Managers. OBJECTIVES: To analyse the involvement of the social groups (Occupational Health Service, Management and Trade Unions) in relation to the detection of workers suffering from drug-addiction and/or alcoholism. METHODS: During the first eight years of implementation (1990-1997) of the Drug-Addiction and Alcoholism Treatment Programme of the Spanish National Railway Company (RENFE), a total of 477 workers consuming alcohol and drugs were detected and treated under the Programme (1.1% prevalence), 465 men (97.5%) and 12 women (2.5%), with an average age of 39.8 years. RESULTS: The subjects were detected by occupational physicians (55.4%), occupational psychologists (2.9%), managers (11.3%), trade unionists (19.5%), via self-referral (9.4%) and by other means (1.5%). CONCLUSIONS: The results demonstrate the efficiency and the profitability to the company of prevention and treatment interventions and the need for the coordination of managers, trade unions and occupational health services.

[Evaluation of palatability of two special oral diets for institutionalized elderly diabetics, Glucerna SR vs Resource Diabet].

OBJECTIVES: To evaluate the organoleptic characteristics of two specific nutritional supplements for diabetes (Glucerna SR and Resource Suport) and compare them. SETTING: An evaluation was made of 456 patients with Type 2 diabetes (most of them receiving OADs or insulin) from 34 centers. They were non-smoking elderly patients (mean age 73 to (71-78) admitted to nursing homes or homes for the aged distributed all over the Spanish territory. INTERVENTIONS: Palatability was studied using a modified wine tasting scale assessing 6 parameters (appearance, smell, body, sweetness, aftertaste and taste) that were scored individually, with a total score ranging from 0 (most unfavorable) to 20 (most favorable). Each brand was evaluated in two flavors (vanilla and strawberry) according to a crossover, double-blind design. RESULTS: A total of 906 evaluable observations were made, and highly statistically significant differences favorable to Glucerna RS were found in all parameters considered both absolutely and relatively, exception for sweetness, for which statistical significance was not reached because it was relatively evaluated. No statistical differences were found between the two flavors (vanilla and strawberry). The only significant confusing factor found was age; the older the age, the more the differences were noted between the two brands. CONCLUSIONS: Glucerna SR has a better flavor than Resource Suport for institutionalized elderly diabetic patients.

Quinapril versus enalapril in the treatment of mild-to-moderate essential hypertension.

Two angiotensin converting enzyme inhibitors, quinapril and enalapril, were evaluated for comfort, safety, and efficacy in the treatment of mild-to-moderate hypertension. After a four-week placebo period, 53 hypertensive outpatients (diastolic blood pressure, 95 to 115 mmHg), aged 31 to 66 years, were randomly assigned to received 10 to 20 mg of quinapril or enalapril daily for 12 weeks. Two hours after the first dose of each drug, sitting and standing systolic and diastolic blood pressures were reduced significantly from baseline values; these reductions were maintained for the 12 weeks of treatment, with no significant between-group differences. No changes in heart rate were noted. Treatment was discontinued in three quinapril-treated patients and in one enalapril-treated patient because of treatment side effects. It is concluded that both quinapril and enalapril are safe and effective in the treatment of mild-to-moderate hypertension.

Cytokine serum profiles in allergic and non-allergic asthma. Increased production of IL-10 by non-allergic asthmatic patients.

Studies were undertaken to determine whether differences in serum cytokine balances could be involved in the pathogenesis of allergic and in non-allergic asthma. At this propose, interferon-gamma, tumor necrosis factor-alpha, interleukin-2, interleukin-4, interleukin-6, and interleukin-10 were measured by enzimoimmunoassay. The analysis was performed on 24 allergic and 24 non-allergic asthmatic patients and 16 healthy subjects. IFN-gamma and TNF-alpha, included into the type 1 cytokines, appeared significantly increased in the allergic with respect to the non-allergic asthmatic patients (p = 0.01) and (p < 0.001) respectively, while IL-10, which belongs to the type 2 cytokines, was significantly increased in the non-allergic asthmatic (p < 0.001). The IL-6 analysis did not show any significant difference in either of the study group. The most interesting finding was the high serum IL-10 values detected in intrinsic asthmatic patients, which in turn, suggests that this cytokine could participate in the regulation of different immunological features that occurs in non-allergic asthma, and maybe it could indicate a higher stimulated state of cells in this type of asthma. The data presented in this report show a different cytokine profile in serum from allergic and non-allergic asthmatic patients and denote a stronger prevalence of type 2 cytokines in intrinsic asthma.
