Polymorphonuclear leucocyte fluorescence and cryoglobulin phagocytosis in systemic lupus erythematosus.

Peripheral blood polymorphonuclear leucocytes (PMN) and cryoglobulins were isolated from patients with systemic lupus erythematosus (SLE). Fluorescent inclusions were found in PMN. Normal donor PMN were incubated with the sera and cryoglobulins from SLE patients. In most cases inclusions were observed after incubation. The high incidence of anti-IgG activity in phagocytosed cryoglobulins confirms the importance of the rheumatoid factor in phagocytosis of immune complexes. It is concluded that phagocytosis of cryoglobulins supports the suggestion that cryoglobulins are a subpopulation of immune complexes.

Membranous glomerulonephritis in congenital syphilis.

The glomerulonephritis of congenital syphilis is thought to be immune-complex mediated. We describe a case of C. S. in whom the only manifestation was a glomerulonephritis. Immunopathogenic studies allowed the identification of treponemal antigen in glomeruli. The elution of immunoglobulins from renal tissue showed that glomerular deposits contained antitreponemal antibodies. Cryoglobulins with antitreponemal activity were also isolated from the patient's serum.

[Cryoglobulinemia and mesangio-capillary glomerulonephritis (comparison of the 2 histological types)].

Sera from 21 patients with mesangiocapillary glomerulonephritis were examined for the presence of cryoglobulins, for the complement levels and in 8 cases for the C3 splitting activity. Patients were divided, on the basis of histology, into two groups; the first of 10 patients with subendothelial deposits, the second of 11 patients with intramembranous dense deposits. The incidence of cryoglobulins and the cryoprecipitate components were similar in the two groups. As previously described, in the first group low levels the C3 was low without alterations in the C4 levels. There are no previous reports on this high incidence of cryoglobulins in dense deposits disease and these data add further support to the hypothesis that this disease is, at least partially immunecomplexes mediated.

The effects on polymorphonuclear leucocyte function of prednisolone and azathioprine in vivo and prednisolone, azathioprine and 6-mercaptopurine in vitro.

The effects of prednisolone azathioprine and 6-mercaptopurine on polymorphonuclear neutrophil chemotaxis, phagocytosis, and killing of Staphylococcus aureus and Candida albicans have been studied. In twenty patients with a functioning kidney graft, taking both azathioprine 2.5 mg/kg/day and prednisolone (mean dose 0.59 mg/kg/day; range 0.30-1.0 mg/kg/day), the polymorphonuclear function did not significantly differ from that in either twenty-two normal or eighteen uraemic controls. Addition of prednisolone, 1.2 X 10(-5) M, azathioprine, 2.1 X 10(-5) M, and 6-mercaptopurine, 2.1 X 10(-5) M, using each drug alone, to normal human polymorphonuclear cells in vitro did not significantly alter their function. It is concluded that prednisolone and azathioprine together in vivo and that both these drugs and 6-mercaptopurine singly in vitro have no significant deleterious effect on polymorphonuclear function and do not contribute, in this way, to the increased susceptibility of patients receiving these drugs to infection.

3 R study: renal outcome in renal ischemia: revascularisation or medical treatment.

Ischemic nephropathy refers to the kidney damage following stenosis or an obstructive lesion in the main kidney arteries. This disorder has been overlooked in the past and a more rational and specific use of clinical criteria, and the development of not very invasive techniques with a good diagnostic accuracy such as spiral CT angiography, NMR angiography and echo-colour-Doppler have improved our ability to identify these patients. It is therefore likely that, in the next few years, we will find ourselves treating an increasing number of patients with renovascular ischemic disorders. Transluminal angioplasty and, more recently, the use of endovascular stents, have led to a marked improvement in the treatment of stenoses and, together with vascular surgery, allow to treat almost all patients with this disorder. There is, however, a lack of prospective and controlled studies, which demonstrate the long term benefit of revascularization treatment, as compared with optimum conservative treatment in reducing cardiovascular mortality, cardiovascular events and preserving renal function. The Ischemic Nephropathy Study Group of the Italian Society of Nephrology has organized a prospective, controlled study over a period of three years, aimed at comparing the effect of revascularization versus medical therapy in 300 patients with renal artery stenosis, ranging between 50 and 90 per cent, who will be randomly assigned to the two treatments. End point will be cardiovascular mortality and morbidity and need for renal replacement therapy.

[Historical Archives of Italian Nephrology. "Le Giornate Mediche Triestine del 1952" the first nephrological international meeting held in Italy].

The author came recently across the proceedings volume of a forgotten meeting on renal disease, held in Trieste in 1952. The invited speakers came from Italy, France and Switzerland, and were prominent figures of the European medicine. They dealt with various topics: renal pathophysiology, pathology, diagnosis, and treatment of renal disease. The analysis of the proceedings shows that most of the chosen topics were "hot" for that time and that the papers presented by Italian and foreign speakers were absolutely up-to-date. The discovery of the proceedings of this meeting, followed by other international renal meetings all over Europe, shows that, in spite of the lack of international recognition, Italian nephrologists were active at an early time and maintained a scientific relationship with their foreign counterpart.

The Crithidia luciliae test in the diagnosis of systemic lupus erythematosus.

Fluorescence of the kinetoplast of Crithidia luciliae was employed as a test to detect anti-DNA antibodies in 179 serum samples from 95 patients with various immunological diseases. The test was positive only in patients with systemic lupus erythematosus (SLE). The results confirm the high specificity of this test in the diagnosis of SLE.
