[Compartment syndrome of the leg (author's transl)].

The authors have observed ten compartment syndromes of the leg. Nine occurred after tibial fractures, one after a contusive trauma. Early signs those of muscular and neural ischemia, with an acute pain when the toes are passively mobilised. The cast must be promptly removed. In 6 cases the two compartments were involved. In all cases the pulses were palpable. Arteriographs could occasionaly verify the integrity of the main arterial trunks. The treatment is based on early fasciotomies. It was realised in 5 cases, with good results when the fasciotomy was realised more than twelve hours after the trauma, the muscles were more or less necrotic. In one case all the muscles of the leg had to be excised.

[Infected total knee prostheses treated by Papineau's technique and arthrodesis (author's transl)].

The technique of arthrodesis of the knee after removal of an infected total prostheses is described. It was used in four cases. In the first stage, the prostheses and cement were removed, the wound was left open and knee immobilized by Hoffman external fixation. Details of the fixation are given. The second stage of filling of the wound by cancellous chips was performed two weeks later. On average, the fixation was removed after seven months. Some mechanical experiments were made to establish the best type of fixation device.

[Immobilisation hypercalcaemia. 1 case (author's transl)].

A 21-year-old man suffered severe (up to 15.5 mg%) and prolonged (7 months) hypercalcaemia as a result of immobilisation following severe trauma. During the period of hypercalcaemia, plasma parathormone and calcitonin levels were undetectable. A study using radioactive calcium revealed a very marked increase in bone resorption, a moderate increase in accretion and virtually no intestinal absorption. Iliac biopsy revealed dense calcification front and there was no osteoid substance. This hypercalcaemia appeared to be due to increased bone resorption, while the synthesis of osteoid substance seemed to be insufficient to permit an equivalent increase in calcium accretion.

[Severe ligamentous injury due to trauma of the mobile spinal segment of the cervical column (author's transl)].

The mobile spinal segment includes anatomically, all the soft parts which lie between two neighbouring vertebrae. Its involvement in trauma is not always obvious on initial X Ray. However, the course may lead to complete vertebral dislocation. The initial diagnosis is based on opening of the joint spaces, study of the mobility between two vertebrae by dynamic radiography. The distinction between a benign and a severe ligamentous sprain may thus be made. The attitude in cases of mobile spinal segment involvement, i.e. severe ligamentous injury, should include arthrodesis of the mobile segment affected and this is discussed here.

[Chronic disseminated histiocytosis in the adult. Apropos of a case with a 12-year remission].

The authors report a rare case of diffuse histiocytosis X in an adult aged 47 followed up for 12 years. There was extensive skeletal and oral dissemination but without visceral, cutaneous or neurological involvement. Blood investigations showed only a raised erythrocyte sedimentation rate. The literature of histiocytosis is reviewed; few papers could be found concerning this type of disease in adults.

Chronic Disease↗

[Failures and limitations of the Papineau technic. A report of 54 cases].

The authors have treated 41 cases by Papineau's technique. There were 19 cases of osteitis, and 35 septic non-unions (9 in the femur, 16 in the tibia and 10 in other sites). Twenty-four cases healed, the average follow-up being one year. The causes of failure were analysed. Some are related to the technique itself - sepsis and fractures at the sites of graft removal, inadequate quantities of cancellous bon, long delay in bone union and skin problems particularly in the leg. The indications for this technique are compared with those of other techniques. It is concluded that it is a useful technique; only two patients required amputation.
