[Infectious diseases in 2003].

A brief survey of the present incidence of infectious diseases (by WHO) and new possibilities of their team study. Global significance of morbidity and mortality of some infections is stressed, namely those of malaria, tuberculosis, HIV infection, infantile diarrhoea, respiratory diseases. Alarming trends in age distribution of some diseases (87% of HIV infection affects young population between 15 to 19 years) or the significance of HIV infection in combination with tuberculosis in previous 10 years in Africa are pointed out. Serious appears the information on the pharmaco-resistance of mycobacteria as well as the growing resistance of Plasmodium falciparum to chlorochin, namely in the regions of the Southeast Asia.

[New drugs for treatment of parasitic infections].

In this review we have summarized published data on two new compounds, which can represent important antiparasitic drugs in the near future, nitazoxanide for treatment of intestinal parasitic infections including cryptosporidiosis and miltefosine for oral treatment of visceral leishmaniasis.

Lymphocyte subset numbers in cerebrospinal fluid: comparison of tick-borne encephalitis and neuroborreliosis.

OBJECTIVE: The aim of this study was to analyze lymphocyte subset numbers in cerebrospinal fluid (CSF) from patients with tick-borne encephalitis (TBE) and acute neuroborreliosis. METHODS: CSF lymphocyte subsets were enumerated in 42 TBE and nine neuroborreliosis patients using flow cytometry. RESULTS: The CSF numbers of CD4+, CD8+, HLA-DR+ and total-T lymphocytes, B lymphocytes, and NK cells were all greater in neuroborreliosis patients than in TBE patients. Neuroborreliosis patients showed positive correlation of CSF protein levels with the numbers of CD4+, HLA-DR+ and total-T lymphocytes. Also, the numbers of CSF B lymphocytes correlated positively with intrathecal Borrelia burgdorferi-specific IgG antibodies. Conversely, TBE patients demonstrated intrathecal protein levels that correlated positively with all investigated CSF lymphocyte subsets. CONCLUSION: These results suggest an intensive recruitment of lymphocyte subsets into the central nervous system (CNS) during acute neuroborreliosis, whereas TBE is characterized by a lower accumulation of lymphocyte subsets in the CSF.

Changes in lymphocyte subpopulations and CD3+/DR+ expression in sepsis.

OBJECTIVE: To detect lymphocyte subpopulations and CD3+/DR + expression in sepsis. METHODS: In a prospective clinical study we evaluated subpopulations of lymphocytes and percentage of CD3+/HLA-DR+ lymphocytes using two-color flow cytometry in 40 patients with sepsis and compared them with 34 healthy adults. RESULTS: Septic patients, when compared with healthy controls, have significantly lower percentage and absolute numbers of total T lymphocytes and CD4 T lymphocytes (P < 0.01). Absolute numbers of CD8 T lymphocytes, NK cells, CD3+/DR + lymphocytes and CD4/CD8 ratio were also decreased (P < 0.01). The percentage of B lymphocytes was increased (P < 0.01). CONCLUSION: Our results are in agreement with previous findings in patients with sepsis after major surgery or trauma. The decreases in the percentage and absolute numbers of circulating lymphocyte subsets in non-surgical sepsis could represent a general reaction to stress. Increased percentage of B lymphocytes is most probably related to the bacterial etiology of the disease.

[The first death from tertian malaria in the Czech Republic].

The authors describe a fatal case of tertian malaria (Plasmodium vivax) imported to the Czech Republic from India. Severe clinical symptoms leading to shock and multi-organ failure are more frequent in tropical malaria (P. falciparum). Despite the use oc continuous veno-venous haemodiafiltration HLA-DR decline and death occurred. The effect of the administered antimalarias drugs was good.

[The ocular form of toxocariasis].

Ocular toxocariasis (ocular form of larval toxocariasis) arises mainly unilaterally and represents no rare disease. On 3rd Department of Infectology, 1st Faculty of Medicine, Charles University, Prague, 102 patients with proved larval toxocariasis were treated from 1981 to 1990. Ocular toxocariasis concerned only one third of this number. Most frequent form was the retinal toxocaral granuloma (in 55.2%), positioned by two thirds at the posterior pole of retina. In one case, endophthalmitis led to amaurosis of the eye. Clinical forms typical for ocular toxocariasis are presented. No statistically significant difference was observed in treatment effects using thiobendazole or diethylcarbamazine. All patients with ocular toxocariasis were treated with systematic steroids.

[The first hyperimmune immunoglobulin against cytomegaloviruses made in the Czech Republic].

The Czech immunoglobulin CMV was administered to 16 subjects with active CMV infection--mostly women with repeated miscarriages, CMV primoinfection during pregnancy, to children with congenital CMV infection and to patients with other clinical forms. The immunoglobulin was administered in a single dose or repeatedly by the intramuscular route. The decline of activity of the CMV infection was demonstrated by a decline of specific IgM CMV antibodies, disappearance of previously assessed viriuria and isolation of CMV in smears from the uterine cervix.

[The role of cytomegaloviruses in childhood deafness].

The relationship between cytomegalovirus (CMV) and the auditory apparatus is still unclear. The authors examined 120 children from nursery as well as elementary schools for children with hearing defects and, using the complement-fixation method, they determined the CMV antibodies. The result showed a higher quantity of CMV than that found in a group of healthy children. A detailed examination of the auditory functions (of what remained of them) in this group did not eliminate the possibility of CMV effect on the cochlea, acoustic nerve or even on the higher levels of the hearing passage.
