Placental toxicity of cadmium in the rat: an ultrastructural study.

Pregnant rats on day 18 of gestation were injected s.c. with 40 mumol/kg CdCl2 which caused fetal death and placental necrosis. The placental changes were studied by electron microscopy and indicate that there is a direct placental toxic effect of cadmium which appears targeted at the trophoblast and, in particular, trophoblast cell layer II. Findings in cell layer II which suggest a toxic effect were lysosomal vesiculation, 'buckshot' nuclear chromatin clumping, nucleolar changes and apparent mitochondrial calcification. Furthermore, the selectivity of the effect on cell layer II and the rapidity of the necrosis are also consistent with a toxic effect. Trophoblast cell layer II first undergoes necrosis, but is rapidly followed by the rest of the trophoblast. Many of the changes at this necrotic stage suggest a secondary ischaemic effect or a combined ischaemic and toxic effect. Therefore it appears that cadmium induces placental necrosis via a direct effect on the trophoblast, especially on layer II.

Evidence of HTLV-I infection in Singapore prostitutes.

Sera obtained in 1987 from 63 male and 632 female Singapore prostitutes were screened for antibody to human T-cell leukaemia virus (HTLV)-I with a particle agglutination test. Of the 3 males and 4 females who were positive one had antibody to HTLV-I core and envelope antigen on Western Blot. Two subjects had presumptive antibody to HTLV-I core antigen and a third subject had such antibody on a repeat specimen in 1989. These sera were negative for HIV-1 antibody. There is evidence of infection with HTLV-I or a variant virus in this population. The infection is likely to have been sexually transmitted.

Anaphylactic-like reactions to zomepirac.

The authors report two cases of anaphylactic reaction to zomepirac. A brief discussion of anaphylactic reactions to other nonsteroidal antiinflammatory agents, for example, tolmetin and sulindac, is included. The authors note that first exposure to the NSAIDs usually produces few, if any, adverse effects in patients who later suffer severe reactions.

Persistence of the primary vitreous in association with the morning glory disc anomaly.

Two patients with the morning glory disc anomaly are reported. In one, fibrovascular stalk passed forward into the vitreous cavity, while in the second a persistent hyaloid artery extended anteriorly from a tuft of fibrous tissue to the posterior lens capsule. These cases suggest that in some eyes the morning glory disc anomaly occurs as a variant of persistent hyperplastic primary vitreous (PHPV).

Simultaneous surgery for bilateral congenital cataracts.

In about one third of patients congenital cataracts are bilateral. If the cataracts are dense, and the patient an infant, bilateral simultaneous cataract removal reduces anesthetic risk, hospital stay, and cost and allows earlier optical correction. This study represents a retrospective survey of 32 eyes (16 cases) with simultaneous surgery for bilateral congenital cataracts. Bilateral simultaneous surgery can be performed to avoid a second general anesthesia in infants who have bilateral dense congenital cataracts.
