[Medullary carcinoma of the thyroid gland. Prognostic factors].

OBJECTIVE: To analyze the clinical features and possible prognostic factors involved in the clinical course of a series of 26 patients with the diagnosis of thyroid medullar carcinoma (TMC). MATERIALS AND METHODS: An analysis was made of the variables age, sex, clinical course to diagnosis, paraneoplastic manifestations (flush, diarrhoea), clinical type (sporadic, non-associated, familial MEN IIa associated TMC, familial MEN IIb associated TMC), histologic uni or multicentricity, and stage of disease as possible predictive factors for the evolution. RESULTS: The most remarkable prognostic factor was tumoral stage. The likelihood of a patient diagnosed with stage I or II being free of metastatic disease was 1, 0.66 and 0.33 at 15, 16 and 17 years, respectively. In contrast, those patients in stage III at diagnosis had a likelihood of being free from metastasis 5 years after diagnosis and therapy of 0. With regard to sex, a better clinical course was observed among males than among females, statistically significant at 3 and 5, but not at 10 years. No statistical significance was reached with the other possible prognostic factors investigated. CONCLUSION: Given the obvious influence of tumoral stage of disease on prognosis, an early access to medical attention is desirable for diagnosis and treatment of TMC, as well as an early detection of family cases by a screening test.

Effects of estrogens on the redox chemistry of iron: a possible mechanism of the antioxidant action of estrogens.

The preventive effects of estrogens on FeSO4-induced lipid peroxidation are closely correlated with their inhibition of Fe(II) oxidation during peroxidation. Estrogens affected the redox status of iron in aqueous solution with varying degrees of effectiveness. 2-Hydroxyestradiol substantially decreased the oxidation of Fe(II) and was the most potent Fe(III) reductant. Diethylstilbestrol and 4-hydroxyestradiol also exhibited reduction properties, whereas the phenolic estrogens 17 beta-estradiol, estrone, and 17 alpha-ethynylestradiol displayed slighter or no effects. Present results demonstrate that catecholestrogens and diethylstilbestrol directly alter the iron redox chemistry, this fact probably being involved in the antioxidant effects of these molecules.

[A left intraventricular thrombus in an adolescent. Its relationship to the anticardiolipin antibody syndrome].

The authors are going to present the clinical case of a 14-years-old female patient who has got an anticardiolipin antibodies syndrome, a theme to which they add some ideas concerning physiopathology and diagnosis criteria. They'd like to emphasise the echocardiographic observation, which allowed them to find out and follow the evolution of a left sided intraventricular thrombus, the real cause for the systemic thrombosis.

[An evaluation study of a scale of diabetological education in primary care].

OBJECTIVE: To devise and evaluate a scale of measurement which would permit evaluation at primary care level, of the knowledge that diabetic patients have of diabetes. Secondly, to ascertain the level of knowledge of the sample of diabetic patients attending the Xixona Health Centre. DESIGN: Non-longitudinal, observational, descriptive study. SETTING: Primary health care (health centre). MEASUREMENTS AND MAIN RESULTS: The degree of internal consistency of the diabetes scale of knowledge (DISK) attained a Cronbach Alpha value of 0.87. The coefficients of correlation of each of the factors--defined a priori (general knowledge, diet, exercise, hygiene and self-regulation)--oscillated between 0.77 and 0.83. The Garret index of corrected difficulty was at 0.64. The homogeneity of each item with the questionnaire showed values above 0.31. The reliability measured by means of the KR20 index was 0.86. The odd-even coefficient of reliability was at 0.89. The mean level of correct answers was 70%. A relation was found between the level of knowledge and the variables age and years the patient had been subject to the condition. No relation was found with the treatment, sex, glycaemia or HbA1C level, nor educational level. CONCLUSIONS: The indices obtained by the scale can be considered to be judiciously acceptable, both in terms of their internal consistency and their ability to characterise. The DISK (ECODI) is, then, a valuable tool in the field of primary health care.

The effect of food deprivation on the activities of some enzymes of the metabolism of carbohydrates in the submandibular salivary glands of rats.

Male albino rats of the Wistar strain were deprived of food, but not water, and killed at various periods. Enzymes of the metabolism of carbohydrates were assayed in the submandibular salivary glands after 24, 48, 72, 96 and 120 hours of starvation. The body weight loss was accompanied by a decrease in gland weight and gland protein content. Blood glucose fells to about 70% the control level. Hexokinase showed no variation with increasing periods of food deprivation. The decrease in the activity of phosphofructokinase was greater than that one observed for glucose-6-phosphate dehydrogenase, indicating a shift in glucose utilization by the cells, from the glycolytic to the pentose phosphate pathway.
