[A severely disturbed lipid profile].

We report here the case of a 30-year-old woman presenting a disturbed lipid profile since her childhood. The rarity of this pathology (Anderson' disease or chylomicron retention disease) and its recent recognition explain its late detection in this case. The description of the biochemical profile is interesting especially the low level of vitamin A and E which explains the severity of the disease. A vitamin enriched-diet may be useful.

[Fructosamine and HbA1c during monitoring of glucose levels in diabetes].

The authors compared the nycthemeral variations of fructosamine levels in nineteen patients with diabetes to fluctuations in their glucose levels. Thirty-five other patients with poorly controlled diabetes were given an optimal treatment then followed up for 2 months (mean glucose levels, fructosamine and HbA1c assays). Mean nycthemeral variations in fructosamine levels did not appear to be correlated with glucose levels. However, intraindividual nycthemeral variability was greater than the analytic variability. After correction for total protein levels this variability was smaller but remained significative. In this longitudinal study, fructosamine and HbA1c levels decreased when control of glucose levels improved. However, decreases were notably greater in HbA1c than in fructosamine compared at 15, 45 and 60 days of follow up. Inversely, fructosamine was correlated better with the mean glucose levels of the preceding 7 days than was HbA1c. When mean glucose levels were calculated for the 15, 30, 45 and 60 preceding days, the results favored HbA1c. The findings presented in this study show that HbA1c remains the best criteria for monitoring medium term glucose levels. However, further word is needed before this conclusion can be confirmed.

[The request for surgical biopsy in mammography. Apropos of 140 verified cases].

The authors present a series of 140 patients who underwent surgical biopsy guided by an isolated anomaly on mammography without any clinical sign. 27 cases presented an opacity with a long axis less than 10 mm, 59 cases presented isolated areas of micro-calcification and 54 cases presented images of focal distorsion of the glandular architecture. By means of pre- and intra-operative radiographic guidance, the histological analysis of serial sections of these abnormalities was possible in every case, resulting in the diagnosis of 54 cases of benign dysplasia, 31 transitional lesion, including 16 in situ carcinomas and 55 carcinomas, including 34 invasive cancers. The advantage and limitations of this diagnostic approach are analysed. The discovery of a large number of sub-clinical carcinomas, the favourable implications for preservation of the breast and the survival justify the indications for surgical biopsy on the discovery of an isolated mammographic abnormality.

[Value of computer tomography during investigation of sub-diaphragmatic adenopathies occurring during the course of lymphomas and pelvic cancers (author's transl)].

A series of 60 patients were examined by abdominopelvic computer tomography 48 hours after foot lymphography. The diagnosis was Hodgkins's disease in 10 patients, lymphomas other than Hodgkin's in 20 cases, and 30 patients with pelvic carcinomas. The value of lymphography in lymphomas other than Hodgkin's is re-emphasized, and a good correlation exists between the results obtained and those observed with computer tomography, which is particularly useful for studying the upper lumbo-aortic regions. The group of Hodgkin's disease patients particularly demonstrates the limits of pelvic lymph node investigations with computer tomography, but supplies confirmation of the quality of examination of the lumbo-aortic chains. The group of patients with pelvic carcinomas is more heterogenous, and computer tomography is if greater value, mainly in cases with blocked lymphatics. Computer tomography examinations do not replace lymphography in the study of sub-diaphragmatic lymph node chains but are a useful complement as they demonstrate excluded or poorly opacified adenopathies. When lymphography is contra-indicated, computer tomography can supply valuable information, but the differential diagnosis of inguino-iliac chain lesions is difficult. Computer tomography is a logical procedure during pretreatment investigations whenever there is a doubt concerning possible lesions in the lumbo-aortic chains. It is the most valid guide for establishing details of a course of radiotherapy.

[Measurement of inflammatory activity in chronic rheumatism by blood pretein levels].

Seventy six patients with chronic inflammatory rheumatism were treated with various non-steroid anti-inflammatory drugs for 5 to 7 days. The average variations of several serum proteins were studied. The variation of haptoglobin remained nil. Those of other serum proteins remained negative and significantly different from zero, distributed from the greatest to the smallest value of the degree of statistical significance as follow : ceruloplasmin (p < 0.0001), orosomucoide (p < 0.02), fibrinogen (p < 0.02) alpha-2-macroglobulin (p < 0.04) and alpha-1-antitrypsin (p < 0.05). The study of the partial results showed the variability of the degree of significance of the serum proteins in relation to the non-steroid anti-inflammatory drugs. The practical interest of these serum protein seemed low to measure in the short term, the efficacy of non-steroid anti-inflammatory drugs. Their interest in long term studies remains to be demonstrated compared with that of the measurement of the sedimentation rate by the technic of the sigma E.S.R.

[Intravenous urography after radiosurgical treatment of uterine cancer (stages T1 and T2). Normal transient features and urological complications (author's transl)].

The same radiosurgical treatment was given to 108 cases of stages T1 and T2 uterine cancers, and consisted of curietherapy followed by an extended colpohysterectomy with lymph node resection (Meigs operation) Intravenous urography was performed during operation in all patients, and at least one postoperative intravenous urography was also carried out. Stricture of the pelvic ureter was discovered postoperatively in 50.9% of cases, with or without subjacent stasis: this type of image is transient and disappears within eight months. A lymphocele can cause images showing signs of bladder pressure. Ureteral and vesical complications of a true and definitive nature are extremely rare.

[Tomodensitometric aspects of sarcoma of the soft tissue of the extremities in adults].

A group of 21 adults patients with soft tissues sarcomas of the extremities were studied by CT. CT was a diagnostic procedure in only one case. However it has always permitted a precise local work-up particularly in demonstrating the vasculo-nervous connections of the lesions. The place of CT with regard to the other radiological procedures is discussed. by its contribution to the locoregional and general pretherapeutic work-up and to a better determination of radiotherapeutical dosimetry, CT appears to have an up standing place in the staging and the follow-up of soft tissues sarcomas.
