Enucleation of an intramural lipoma of the oesophagus by videothoracoscopy.

An intramural oesophageal lipoma was enucleated by a videoassisted thoracoscopic technique in a patient with dysphagia and reflux symptoms. His symptoms and erosive reflux oesophagitis healed after the operation, but impaired oesophageal motility persisted as assessed by postoperative manometry four months later. The benefits of the videoassisted technique include decreased operative trauma, a short hospital stay and a rapid return to normal activities.

Tissue characterization by image processing subtraction: windowing of specific T1 values.

A method for windowing specific T1 values is presented. A 1.0 T imager with two routine pulse sequences was employed: A T1-weighted spin echo (SE) sequence and a short tau inversion recovery STIR sequence (fat-suppressed IR). A T1 window for fat was obtained by subtracting the STIR image from the SE image. Negative values were coded black. The method was tested on a normal human thigh, on a human liver with confirmed fatty infiltration, and on the livers of four live burbots. The fat-containing tissues of the two human volunteers were well depicted. The differences in fat concentration among the burbot livers were also clearly shown. The fat intensity seen in the images correlated well with the chemically measured fat concentration. This subtraction method for windowing T1 values proved feasible for fat. The method could be used for tissues with other short T1 values as well.

Lumbar spine interbody fusion with reinforced hydroxyapatite implants.

Porous coralline and synthetic hydroxyapatite blocks reinforced with either self-reinforced polyactide or polyglycolide were implanted into 15 lumbar intervertebral disc spaces in five minipigs in order to determine whether they could provide an osteoconductive bridge for interbody fusion. Histological examination and radiological analysis with plain films, computed tomography and magnetic resonance imaging were carried out. The osteoconductive properties were promising; creeping bone formation could be observed, although no complete fusion had been achieved at 24 weeks.

Benign muscular dystrophy with autosomal dominant inheritance.

A slowly progressive myopathy was discovered in a family in four successive generations. Eight patients (four female, four male) from three generations were examined and they showed muscle weakness affecting predominantly proximal, but also distal, muscles. Two patients had unequivocal findings in childhood, the others showed myopathy in their twenties or thirties. Working ability was lost in physically demanding jobs in the thirties, but activities of daily living were still preserved. Elbow contractures, tight heel cords and contractures of the interphalangeal joints were frequent. Serum CK activity was usually mildly elevated and electromyographic examinations revealed myopathic changes. Histopathological changes were compatible with moderately advanced muscular dystrophy in two patients, the six others had mild myopathic changes.

Imaging methods reveal unexpected patchy lesions in late onset distal myopathy.

Computed tomography (CT), ultrasonography (US) and low field magnetic resonance imaging (MRI) of muscles were performed in 13 patients of a large family with two clinically separate phenotypes of muscular dystrophy. Five patients had severe proximal muscle weakness and wasting like in limb-girdle muscular dystrophy. Imaging methods showed loss of muscle structure and replacement with adipose tissue especially in proximal muscles. Eight patients had distal myopathy of late onset with weakness and wasting of anterior tibial muscles. Imaging methods confirmed fatty degeneration of tibial muscles and, moreover, revealed unexpected large patchy lesions in several other clinically unaffected muscles. Our results indicate that some myopathies which are clinically localized, may actually have a more widespread patchy involvement as revealed by non-invasive imaging methods. In this family CT and MRI were more informative concerning lesions and distribution than US.

High-frequency ultrasonography of skeletal muscle in children with neuromuscular disease.

Ultrasonography of the thigh and calf was performed in 24 children who had primary neuromuscular disease and in 20 healthy children. The ultrasound image was clearly abnormal in all patients with progressive muscular dystrophy, and in the majority of children with benign myopathic disorders; the principal changes were increased muscular echogenicity and increased attenuation of ultrasound with a reduced bone surface echo. In 13 patients, the ultrasound findings were correlated with pathologic changes seen in muscle biopsy specimens: a clear correlation (r = .85) was found. Muscular dystrophies had a higher score of abnormal ultrasonographic and microscopic findings, while the more benign muscular diseases had a lower score.

Radiological progression of rheumatoid arthritis in renal transplant recipients.

Radiological progression of rheumatoid arthritis (RA) was evaluated retrospectively in eight patients with amyloidosis secondary to classical or definite RA and successful renal transplantation. Wrist joints, metacarpophalangeal, proximal interphalangeal, and metatarsophalangeal joints were graded on a scale from 0 to 5. The annual progression rate was calculated separately for the time before and after transplantation. A slight acceleration of the progression was observed after transplantation and is assumed to be due to activation of the rheumatoid disease when the azotaemia was corrected.

[Stress fractures of the femora in soldiers (author's transl)].

Amongst 936 stress fractures found in soldiers, there were 58 in the femora (6%); of these 31 were in the neck and 27 in the shaft. Two were bilateral, and two patients had other stress fractures. Three displacements were found in the necks. In the shaft, 20 fractures were proximal, four were in the middle third and three in the distal third. In the latter group, it is necessary to differentiate from a sarcoma. CT is a new aid in this respect. Sport in highly motivated individuals appears to contribute particularly to fractures of the shaft. The symptoms from these fractures are relatively mild.

Combination of multiple pencil-beam imaging to computed storage phosphor radiography: a new method.

Computed radiography (CR) with storage phosphors offers a wide dynamic range and improved sensitivity compared to film-screen technology. CR was combined in this study with a prototype multiple pencil-beam (MPB) imaging device which has been shown to be very effective in scatter reduction. The combination was analyzed and compared to the standard technique of grid screening in two ways: a free-response ROC (FROC) analysis was first performed followed by a blinded test arrangement for visual analysis of image quality in a series of computed radiography of the lumbar spine by both the MPB and grid modalities. The results of the FROC study showed a statistically significant (P less than or equal to 0.01) improvement in signal detection. The MPB-CR images of the lumbar spine had more contrast but also a slightly mottled or grainy appearance. Image quality was found good but contrast processing was criticized because it seemed to result in a too steep display of contrast in MPB imaging. This should be avoidable by changing the image processing parameters.

Ultrasonography, CT, and MRI of muscles in congenital nemaline myopathy.

Twelve patients with congenital nemaline myopathy were examined by ultrasonography and computed tomography (CT) and 4 of them by low-field magnetic resonance imaging (MRI) to investigate the distribution and nature of muscle involvement and to evaluate the yield of these techniques. A pattern of selective muscle involvement was consistently found. Ultrasonography revealed abnormally high muscle echogenicity. In contrast to the controls with simple disuse and/or neurogenic atrophy, CT revealed low muscle density in our patients. MRI confirmed that this pattern reflects patchy, fatty degeneration, a new finding in congenital nemaline myopathy. Ultrasonography appears useful for screening, provided that the dorsiflexors of the feet are examined. CT and MRI can be used to demonstrate abnormalities in mild cases, to select the biopsy site, and for follow-up. MRI, because it provides good soft tissue contrast, is useful for determining fatty infiltration.
