The internal mystery plays: the role and physiology of the visual system in contemplative practices.

In summary, the "White Light" and the imagery reported in contemplative practices appears to be a complex system of reflexes that mediate ischemia to the cerebral cortex, stimulating "release" of the occipital poles and the rostral midbrain to discharge these images in any profound state of cellular agony. These vascular events are mediated by way of the autonomic nervous system, either accidentally or purposefully, thus acquiring a near-death experience. Understanding the mechanism of the reflexes has taken many years and thousands of researchers to elucidate. In spite of these discoveries and those yet to come, no one will ever fully know why these reflexes and the sensations associated with them exist, or why they cause such profound psychic, physical, and spiritual changes in the individuals who have them. At this point, I can only be humbled by the process that has brought me to the paradigm I have described. I only know that most of the connections and correlations expressed in this article were the product of some of those methods mentioned herein; therefore, I cannot, and will not, take credit for that portion provided by the Guiding Spirit. Thus, the internal mysteries of the brain will always hide the Inner Mysteries, although the heart will always have a way of finding the right answers. We have become the Uroboric serpent (the serpent that eats its tail), in that we have returned to the ancient Egyptians' way of considering the eye as the seat of the soul and the doorway into the Inner Mysteries of the religious experience. Now, we find t hat in many ways they were correct that the eye, through its physiology, is another key to understanding these Inner Mysteries.

Lens implant exchanges for incorrect power: results of an informal survey.

Despite the introduction of ultrasonic axial length measurements and a variety of intraocular lens implant prediction formulas, postoperative predicted errors greater than 2 diopters occur in approximately 5% to 10% of lens implantations, and occasional errors of 5 diopters to 7 diopters or greater are still encountered. An informal survey revealed successful lens implant exchanges for initial postoperative refractive errors of +4.37 to -14.00 diopters in eight eyes.

Four-incision radial keratotomy for low to moderate myopia.

The first 112 consecutive eyes (75 patients) to undergo four-incision radial keratotomy (RK) performed by four surgeons were evaluated retrospectively. The preoperative myopia (spherical equivalent) ranged from -1.12 to -7.00 D (95% between -1.12 and -4.25 D). The preoperative uncorrected visual acuity was 20/100 or worse in 95% of the eyes. A six-month follow-up was achieved in 95% of the eyes and 45% were followed over one year. Following four incision RK, 79% of the eyes achieved 20/40 or better vision and 82% were corrected to within 1 D of emmetropia. Following repeat RK in 15 eyes (13%), 82% had uncorrected visual acuity of 20/40 or better, and 90% were corrected to within 1 D of emmetropia. Only 3.5% of the eyes were overcorrected by more than 1 D and only one eye had induced astigmatism greater than 1 D. For eyes with preoperative myopia up to -4.25 D over 90% achieved uncorrected visual acuity of 20/40 or better and were within 1 D of emmetropia following primary four incision RK and a second four incisions in 13 eyes (12%). Only 3.5% of the eyes in this group were overcorrected by more than 1 D. There were no serious complications.

Mydriasis from Datura wrightii.

A 4-year-old girl had painless, uniocular mydriasis after picking flowers. Pharmacologic testing with 1% pilocarpine confirmed the probability that an atropine-like agent had been instilled in the affected eye. The flowers were identified as Datura wrightii and contained scopolamine, hyoscyamine, and atropine on thin-layer chromatography.

Treatment of corneal abrasions with soft contact lenses and topical diclofenac.

BACKGROUND: Treatment of corneal abrasions often involves antibiotic ointment and pressure patching. The corneal abrasions following excimer laser photorefractive keratectomy have been managed with disposable soft contact lenses and diclofenac (Voltaren) eye drops. METHODS: We report 13 patients with corneal abrasions from trauma or recurrent corneal erosions treated with application of a disposable soft contact lens and instillation of diclofenac and antibiotic eye drops. RESULTS: All 13 patients reported significant pain relief and all abrasions healed within 3 days (most within 24 hours). Two of the recurrent erosion patients suffered subsequent spontaneous abrasions and one of the traumatic abrasion patients developed a possible infectious keratitis which cleared without visual loss. CONCLUSIONS: In this small series, the combination of a disposable soft contact lens and the instillation of diclofenac drops provided significant pain relief while the abrasion healed and allowed the patients to function with binocular vision. This treatment regimen offers an alternative to pressure patching in the treatment of corneal abrasions.
