Outpatient standards of care and the suicidal patient.

This article is a review of the legal and clinical literature on standards of care for nonhospitalized adult suicidal patients. The authors discuss effective assessment, management and treatment procedures that balance the need for high-quality care by a reasonable and prudent practitioner with the requirements of court-determined and statutory standards. Through a review of malpractice claims data and through an examination of the clinical literature we detail the essential guidelines for sound assessment, intervention, and management procedures. The authors specifically discuss common failure situations in outpatient care (e.g., problems in pharmacotherapy, the decision to hospitalize, inappropriate and dual relationships, the assessment of imminence and lethality, and so on). Details regarding practical considerations in developing an outpatient standard of care are provided, as are suggestions that such a standard of care must include an ongoing assessment of the therapeutic alliance.

A case of the attempted suicide of a 66-year-old female is presented. Comment based on the chapters in this volume is presented. Specific topics of demographics, psychiatric-psychological, object relations, the attempt, and concluding comments provide an idiographic illustration of suicidal behavior in an older adult.

Physicochemical properties of signal receptor domains as the basis for sequence comparison.

1. An algorithm of sequence comparison based on average bulkiness of amino acids in protein domains and not requiring sequence alignment is described. 2. A complete evolutionary tree of the signal receptor proteins is built. The STE2 proteins are shown to belong to this family. 3. Factorial analysis of average bulkiness makes it possible to discriminate functional and intraspecies differences between proteins.

Algorithms

Empirical criteria for the determination of suicide manner of death.

A 16-item instrument was constructed as a tool to assist medicolegal official in their investigations and certifications of suicidal deaths. The Empirical Criteria for the Determination of Suicide (ECDS)--derived from a combined set of the 22 criterion items of the Operational Criteria for the Determination of Suicide (OCDS) and 33 other items obtained from experts and the professional literature--was constructed and validated by using 126 suicide and accident cases obtained from 70 medical examiner participants. Analysis of the cases confirmed that suicide is a manner of death in which there is evidence of both self-infliction and intention to die. The 16 items retained in the ECDS discriminated suicides from accidents best in relation to self-infliction and intention. In analysis of its concurrent validity, the ECDS instrument predicted 100% of the suicides and 83% of the accidents, thus correctly identifying 92% of all cases.

Adolescent

Suicide attempts among adolescent drug users.

Substance use has been identified as a significant risk factor in nonfatal and fatal suicides during adolescence. A comprehensive questionnaire on patterns of alcohol and other drug use, early childhood and nuclear family psychological and behavioral history, and previous suicidal attempts was completed by 298 (88%) of 340 outpatient adolescent substance abusers in three geographic regions east of the Mississippi River. An abbreviated Beck Depression Inventory was included to ascertain current symptoms of depressed mood. Adolescents admitting to a previous suicide attempt (30%) were compared with two age- and sex-matched samples. Substance abusers were three times as likely as a normative population of non-drug-using age- and sex-matched peers to make a suicide attempt. Thirty-three percent of attempts reported occurred prior to high school. Both the wish to hurt oneself and actual suicide attempts were found to increase significantly after the initiation of substance use. Forty percent used drugs within 8 hours before the suicide attempt, and 23% of attempters reported that their families continued to have a firearm with ammunition in the home following the suicide attempt. Adolescent substance abusers who had attempted suicide were significantly more likely than a matched group of nonattempters in the same drug treatment facility to: (1) complain of usually feeling "blue" or sad (depressed affect) during early childhood, (2) identify important childhood behavioral problems, (3) identify long-standing self-perceived impaired self-concept, and (4) identify serious parental problems, such as chronic depression or alcoholism. Self-perceived chronic loneliness in childhood appears to be a singularly important initiator of adolescent drug use and subsequent suicide attempts among drug abusers.

Chlamydial ascites.

Currently, Chlamydia is a major cause of pelvic inflammatory disease (PID), which can be complicated by perihepatitis. Our patient presented with ascites secondary to Chlamydia infection and responded well to treatment with tetracycline. Since Chlamydia infection is becoming more prevalent in the United States, it should be considered in the differential diagnosis of ascites in all young sexually active females.

[Study of thermo-stabilizing effect of tocopherol on rhodopsin in the presence of fatty acids using the method of differential scanning calorimetry].

By the method of differential scanning calorimetry it was shown that the addition of arachidonic acid to photoreceptor membranes is accompanied by concentration-dependent shift of thermograms curve of rhodopsin value. Addition of tocopherol to photoreceptor membranes prevents the turbulent effect of the fatty acid on opsin and rhodopsin. The obtained data are discussed from the point of view of membrane protective properties of tocopherol.

Arachidonic Acid

Fictional depiction of suicide in television films and imitation effects.

Studies of imitative suicide by viewers of fictional depictions of suicide in television films have produced contradictory findings. Using a nationwide sample of cases of suicide, the author found no evidence for increased numbers of suicides after broadcast of three films. However, some support was found for an imitation effect specific to the depiction of a suicide method in one of these films (p less than 0.05). To the extent that fictional presentations of suicide may serve as stimuli for imitative behavior, the effect appears to depend on a complex interaction among characteristics of the stimulus, the observer of that stimulus, and conditions of time and geography.

Operational criteria for the determination of suicide.

Suicide is an important public health problem for which we have an inadequate public health database. In the United States, decisions about whether deaths are listed as suicides on death certificates are usually made by a coroner or medical examiner. These certification decisions are frequently marked by a lack of consistency and clarity, and laws and procedures for guiding these decisions vary from state to state and even from county to county. Without explicit criteria to aid in this decision making, coroners or medical examiners may be more susceptible to pressures from families or communities not to certify specific deaths as suicide. In addition, coroners or medical examiners may certify similar deaths differently at different times. The degree to which suicides may be underreported or misclassified is unknown. This makes it impossible to estimate accurately the number of deaths by suicide, to identify risk factors, or to plan and evaluate preventive interventions. To remedy these problems, a working group representing coroners, medical examiners, statisticians, and public health agencies developed operational criteria to assist in the determination of suicide. These criteria are based on a definition of suicide as "death arising from an act inflicted upon oneself with the intent to kill oneself." The purpose of these criteria is to improve the validity and reliability of suicide statistics by: (1) promoting consistent and uniform classifications; (2) making the criteria for decision making in death certification explicit; (3) increasing the amount of information used in decision making; (4) aiding certifiers in exercising their professional judgment; and (5) establishing common standards of practice for the determination of suicide.

Calorimetric study of thermal denaturation of vertebrate visual pigments.

The thermostability of visual pigments in bovine, rat and frog rod outer segments (ROS) has been studied by means of differential scanning calorimetry and thermal gel analysis methods. Use of the two different methods has allowed to assign calorimetric peaks to rhodopsin and opsin denaturation. The denaturation enthalpy changes for rhodopsin in bovine, rat and frog ROS are 630 kJ/mole (Td = 347 degrees C), 416 kJ/mole (Td = 340 degrees K) and 410 kJ/mole (Td = 340 degrees K), respectively. Corresponding values for opsins are 490 kJ/mole (Td = 332 degrees K), 269 kJ/mole (Td = 320 degrees K) and 158 kJ/mole (Td = 319 degrees K). The free energy of stabilization of the rhodopsin native structure is not very large and practically similar to that for native water soluble globular proteins.

Improving the validity and reliability of medical-legal certifications of suicide.

On a very basic level, those who study suicide are either explicitly or implicitly concerned with the certification of suicide deaths by coroners and medical examiners. Although many authors have questioned the reliability and validity of officially reported rates, these statistics continue to be quoted as fact--a potentially problematic practice for researchers who seek accurate and true causative factors for suicidal behavior. In relation to this topic, this article provides an overview of the existing vital statistics registry system and describes and proposes innovations that could ultimately improve the quality of officially reported suicide statistics.

[Structural stabilization of lipids and visual pigment rhodopsin in the photoreceptor membrane by vitamin E].

Interaction of alpha-tocopherol with free fatty acids in bovine retinal photoreceptor membranes was studied using ESR spin-probe technique and measurements of rhodopsin thermal denaturation rates. Exogenous alpha-tocopherol incorporated into photoreceptor membranes prevented thermal destabilization of rhodopsin caused by free fatty acids. The efficiency of the stabilizing action of alpha-tocopherol directly depended both on the chain length and the degree of fatty acid unsaturation.
