Adult metachromatic leukodystrophy with an unusual relapsing-remitting course.

A 46 year old woman had a relapsing-remitting course of hemiparesis, disorientation, paraparesis and seizures, followed by progressive dementia, spasticity and ataxia. Computed tomography at onset showed a parietotemporal hypodense area with diffuse mottled enhancement obliterating the lateral ventricle. Subsequent scans demonstrated symmetric periventricular non-enhancing hypodensities, progressive ventricular enlargement and atrophy. Adult metachromatic leukodystrophy was diagnosed on the basis of low leukocyte arylsulphatase A level and metachromatic material accumulation at neural nerve biopsy.

Aphasia in multiple sclerosis: clinical and radiologic correlations.

Aphasia is not commonly reported in multiple sclerosis (MS). We report on two clinically definite MS patients, with a remitting-relapsing course, who presented with acute onset of aphasia. MRI demonstrated giant plaques in the left frontal region in one and in the left centrum semiovale in the other. These lesions were not evident in MRI performed several months previously, and may account for the aphasia. In both patients, language functions markedly improved within several weeks. In one patient, follow-up MRI 3 months later revealed a moderate reduction in the size of the plaque.

Sodium valproate in the treatment of cluster headache: an open clinical trial.

The effect of prophylactic treatment with sodium valproate in cluster headache was investigated in an open pilot study. Fifteen cluster headache patients were studied of whom two suffered from chronic cluster headache and thirteen from the episodic type. The dose used was between 600 and 2000 mg per day in two divided doses. Eleven out of the fifteen patients (73.3%) reacted favorably to the treatment. Nine reported a complete disappearance of pain and two, a marked improvement. Sodium valproate appears to be an effective drug in the treatment of cluster headache. A double-blind controlled study is needed for further evaluation.

Retinitis pigmentosa and early onset myopathy: a case report.

Retinitis pigmentosa (RP) and primary muscle disease are both relatively rare inherited disorders. We present a patient in whom both conditions coexisted. The possibility of a genetic association between RP and primary muscle disorder is discussed. It is our belief that this extremely rare association is merely coincidental.

Clonidine treatment in paroxysmal localized hyperhidrosis.

We report two cases of paroxysmal localized hyperhidrosis (PLH), a rare disorder usually of undetermined origin. The patients were treated with clonidine hydrochloride on the assumption that this paroxysmal phenomenon is of CNS origin, probably hypothalamic. Both patients responded favorably to treatment. In treating PLH, clonidine may serve as an alternative approach to local or surgical treatment.

Cluster headache-like pain caused by an upper cervical meningioma.

A 68-year-old man with chronic lung disease suffered from cluster headache (CH)-like pain associated with an upper cervical meningioma extending to compress the lower brain stem. The pain disappeared after tumour excision. We suggest that compression of the brain stem and hypoxemia associated with chronic lung disease might have caused the attacks, especially in the early morning when REM sleep hypoxia was prevalent.

Prevalence of Raynaud's phenomenon in patients with migraine.

We assessed the prevalence of Raynaud's phenomenon (RP) in 111 migrainous patients and 111 controls matched for age and sex. While 29 (26%) of the 111 migrainous patients suffered from RP, its prevalence among the controls was seven (6%) of 111. Raynaud's phenomenon was diagnosed among 13 (33%) of 39 patients with classic migraine and 16 (22%) of 72 patients with common migraine. Drug treatment did not account for the different prevalence in the two groups. We conclude that RP occurs more frequently in patients suffering from migraine than in the non-migrainous population.

Diagnosis of misdiagnosis: on some of the origins and functions of psychophysical misdiagnosis.

Four cases of Peripheral Neurological Disorders (PND) are reviewed as models of misdiagnosis. Weaknesses in the logical process of diagnosis formation, combined with the dismissive function of the diagnosis, may lead to psychiatric misdiagnoses such as hysteria. The same logical and emotional factors can prevent correction of misdiagnosis and thus the misery of the patient may continue. Comments are made about the dangers of misusing the diagnosis of hysteria.
