[A marked elevation in serum CPK following sevoflurane anesthesia].

A one year-old girl with an arachnoid cyst received an arachnoid cystperitoneal shunt operation two tims, firstly under sevoflurane anesthesia, and secondarily under isoflurane anesthesia. After sevoflurane anesthesia, her serum CPK level increased markedly up to 7550 mU.ml-1, but such an increase was not observed following isoflurane anesthesia. The perioperative management was almost identical on these two occasions. A combination of sevoflurane anesthesia and succinylcholine might cause a significant increase in the serum CPK level.

[Perioperative management of patients on long-term administration of psychotropic drugs].

We experienced perioperative management of thirty two surgical patients complicated with either schizophrenia or depression on long-term administration of psychotropic drugs during a period of three years from 1989 through 1991. They underwent various operative procedures under either general anesthesia or epidural anesthesia. Mental confusion was most frequently encountered in immediate postoperative period. The longer the patients had been treated with psychotropic drugs, the more postoperative complications were observed. We lost four patients within two weeks postoperatively. They all underwent laparotomy and had been treated with psychotropic drugs for a prolonged period. No significant association between the types of anesthesia and postoperative complications was observed. Surgical patients on long-term administration of psychotropic drugs should be managed carefully particularly during postoperative period.

[Autonomic nervous function and plasma catecholamine levels of perioperative patients treated with antipsychotic drugs].

The RR intervals on ECG and the coefficient of variation of R-R interval (CV) and perioperative plasma catecholamine levels were measured in patients receiving long-term administration of antipsychotic drugs. CV was significantly reduced (P less than 0.05) in patients on antipsychotic drugs. Preoperative plasma catecholamine levels increased significantly and elevation of plasma catecholamine levels on the first postoperative day was less than that of the control group. These findings suggest that the patients on antipsychotic drugs have disturbed autonomic nervous function. Therefore, a careful perioperative care is mandatory.

[Cerebral infarction during operation on the neck of the femur under epidural anesthesia: a report of two cases].

Cerebral infarction occurred during operation in two cases. An 83-year-old senile demented female underwent osteosynthesis with Ender pins under epidural anesthesia. Soon after Ender pins were inserted, her systolic blood pressure fell from 180 mmHg to 140 mmHg. The patient complained of chest pain and developed disturbed consciousness and right hemiparesis. On the second postoperative day, CT scanning showed the infarction of a left middle cerebral artery region. A 93-year-old female was another patient. She underwent a cup arthroplasty of the right hip bone fracture. Soon after bone cement was applied, she developed a mental confusion and her blood pressure increased from 120/60 mmHg to 160/80 mmHg. CT scanning on the first postoperative day showed the infarction of a left middle cerebral artery region. Sudden changes in systolic blood pressure and changes of mental status are possible signs of cerebral infarction not to be neglected. If elderly patients with any episodes of cerebral function troubles would undergo operation of the hip fracture, it is necessary to pay a particular attention to possible cerebral infarction.

[An emergency cesarean section using general anesthesia for a patient with schizophrenia].

A 27 year-old female in 39th week gestation with schizophrenia underwent an emergency Cesarean section using general anesthesia. A diagnosis of schizophrenia was made two years previously, since then oral anti-psychotic drugs such as chlorpromazine had been given to her. In June 1989 she suddenly became excited and generalized muscle rigidity was observed without any triggering episodes. Her excitement was so marked that we had to administer intramuscular levomepromazine 75 mg and diazepam 10 mg to her, but they failed to sedate her adequately. Emergency Cesarean section was scheduled to overcome this situation. Spinal or epidural anesthesia was not indicated because of her vigorous excitement, and anesthesia was induced with thiopental 350 mg and succinylcholine 40 mg. Induction-delivery time was 12 minutes. Pentazocine 30 mg in combination with nitrous oxide was given for the maintenance of anesthesia. Plasma levomepromazine levels were 46.9 ng.ml-1 in the mother and 11.3 ng.ml-1 in the umbilical vein, respectively. The baby's Apgar score was 9 and 1 min and 9 at 5 min after the delivery. The baby developed slight generalized tremor until next day, probably due to effect of levomepromazine given before the Cesarean section. The patient was discharged without any cardiorespiratory trouble and her baby has been doing well so far.

[Anesthetic experience of a patient with essential thrombocythemia].

We reported a 54-year-old male with essential thrombocythemia, who underwent coronary artery bypass grafting. Anesthesia was maintained with enflurane in nitrous oxide and oxygen supplemented with fentanyl. Heparin 7mg. kg-1 was administered intravenously to obtain adequate anticoagulant effect during 89 minutes of extracorporeal circulation. Aspirin and dipyridamole were also administered as anti-platelet therapy. No complications were observed during and after anesthesia. It is advocated that administration of anticoagulants such as heparin, aspirin and dipyridamole is effective to prevent thrombus formation.

[Respiratory gas exchange during total body hyperthermia in man].

Respiratory gas exchange was evaluated using indirect calorimetry during total body hyperthermia (TBH) in 7 postoperative patients with disseminated gastric cancer to the peritoneal cavity. TBH was induced using veno-venous bypass and extracorporeal circuit incorporating a heat exchanger to keep pulmonary arterial temperature 42 degrees C. The high temperature was maintained for 3 hours under general anesthesia with droperidol (0.15 mg.kg-1), morphine (1 mg.kg-1) and enflurane (less than 0.5%). Oxygen consumption (VO2) was also calculated using the Fick equation and thermodilution cardiac output. Hyperthermia for three hours increased both VO2 and carbon dioxide output (VCO2) values to 1.5 times as compared to the control values before heating, respectively. VO2 values measured by indirect calorimetry correlated well with the values calculated from the Fick equation when respiratory and cardiovascular system were relatively stable during the procedure. Gradual but statistically insignificant increase in respiratory quotient was observed after recooling started. These results suggest that indirect calorimetry was valuable to measure respiratory gas exchange continuously during hyperthermic state as well as normothermic state. However, RQ value observed in the present study may be modified by several factors including lactate accumulation in the blood, and may not reflect substrate utilization during the hyperthermic state.

Current perception thresholds and postoperative pain in schizophrenic patients.

BACKGROUND AND OBJECTIVES: Schizophrenic patients may have less sensitivity to pain; however, pain insensitivity in schizophrenia has not been adequately evaluated. We investigated current perception threshold (CPT) and postoperative pain intensity in patients with long-standing and treated schizophrenia and control patients. METHODS: We measured CPTs for 2,000 Hz, 250 Hz, and 5 Hz and postoperative pain intensity using a visual analogue scale (VAS) in 50 chronic schizophrenic patients who were on chronic phenothiazine derivatives (> 10 years) and for 25 control patients. RESULTS: CPTs for 2,000 Hz, 250 Hz, and 5 Hz in schizophrenic patients were 334.2 +/- 112.2, 303.9 +/- 117.1, and 165.0 +/- 72.3, respectively. CPTs for 2,000 Hz, 250 Hz, and 5 Hz in schizophrenic patients were significantly higher than those of control patients. VAS pain scores of schizophrenic patients were 4.0 +/- 1.7 at 2 hours post-operatively and 3.8 +/- 1.5 at 5 hours postoperatively, which were significantly (P < .05) lower than those (5.0 +/- 1.6 and 5.1 +/- 1.9) of the control group. CONCLUSIONS: Chronic schizophrenic patients have increased current perception threshold and lower VAS pain scores in postoperative pain compared with control patients.

Effect of epidural analgesia on postoperative paralytic ileus in chronic schizophrenia.

BACKGROUND AND OBJECTIVES: Postoperative paralytic ileus is frequently encountered in chronic schizophrenic patients who undergo abdominal surgery. We investigated whether epidural analgesia with local anesthetics minimizes postoperative ileus in schizophrenic patients who are treated long term with antipsychotic drugs. METHODS: We measured the VAS pain after surgery and the time that elapsed before the first passage of flatus and/or feces after the end of surgery in schizophrenic patients provided analgesia with systemic buprenorphine (group A) and schizophrenic patients receiving epidural analgesia with local anesthetics (group B). RESULTS: The frequency of patients who did not pass flatus and/or feces for more than 120 hours postoperatively was significantly higher in group A. Postoperative pain scores of group A at 8 and 24 hours after the end of anesthesia were 36.0 +/- 12.8 and 31.7 +/- 10.7 (0 to 100 mm scale), which were significantly higher than 25.4 +/- 13.2 and 20.5 +/- 9.4 scores in group B. CONCLUSIONS: Epidural analgesia with local anesthetics in chronic schizophrenic patients undergoing abdominal surgery minimizes postoperative ileus compared to patients receiving systemic buprenorphine.

Current perception thresholds of patients with long-term administration of maprotiline.

The purpose of this study was to evaluate sensory nerve function using current perception thresholds (CPTs) in patients who were administrated maprotiline. Twelve patients with post-herpetic neuralgia and 20 control subjects were studied. The patients with post-herpetic neuralgia were given a daily dose of 60 mg of maprotiline and were maintained at the same dose for 6 months. Twenty control subjects were randomly selected from healthy volunteers. ACPT test was used for quantitative assessment of A beta, A sigma, and C fiber transmission, which are associated with pain, by three (2000, 250, and 5 Hz) different frequencies of electric stimulation. CPTs of 5, 250, and 2000 Hz in patients with post-herpetic neuralgia 2 months after administration of maprotiline were 141.7 +/- 17.3 for 5 Hz, 120.8 +/- 12.9 for 250 Hz, and 256.4 +/- 18.0 for 2000 Hz, which were significantly (P < 0.01) higher than those (67.0 +/- 9.1 for 5 Hz, 73.4 +/- 7.0 for 250 Hz, and 191.3 +/- 20.2 for 2000 Hz) before treatment and than those (35.3 +/- 15.8, 62.0 +/- 18.9, and 198.9 +/- 15.8) of control subjects. An increase in CPT for 5 Hz at 2 months after administration of maprotiline correlated (r = 0.71, p = 0.01) with a decrease in pain score. There were no correlations between an increase in CPT and changes in Hamilton Depression Scale (HAMD) values until 3 months after maprotiline treatment. However, we found that an increase in CPT for 5 Hz at 6 months after maprotiline treatment correlated (r = 0.68, p = 0.015) with a decrease in HAMD values. In conclusion, administration of 60 mg maprotiline significantly increased current perception thresholds at 2 months after the administration.
