Radical hysterectomy: a critical review of twenty-two years' experience.

One hundred seventy-eight patients who had radical hysterectomy performed for management of invasive carcinoma of the cervix over 22 year period at the Medical University of South Carolina were critically analyzed. The degree of differentiation of the malignancy appeared to be of more significance than tumor size in Stage 1 lesions. With proper patient selection and excellent operative technique, survival rates well over 90% with minimal complications can be obtained. The radical hysterectomy should never be used in association with irradiation.

Characteristics of tissue growth into Proplast and porous polyethylene implants in bone.

(1) Bone does not form within internal pores of undistorted Proplast implants because of the small interconnecting pore size of the material; (2) the nonosseous, fibrous tissue which exists in the pores of Proplast implants in bone is not attached to the surrounding bone (i.e., Sharpey's fibers are not present). The load-bearing support which can be afforded by Proplast implants is limited by the incomplete bone ingrowth along the margins of the material and the tensile strength of Proplast.

Intravascular papillary endothelial hyperplasia: light and electron microscopic observations of a case.

The ultrastructure of a case of intravascular papillary endothelial hyperplasia is presented. The papillae and septa within the lesion showed a similar organization. The surface was lined by a nonstratified layer of plump endothelial cells. Immediately beneath lay cells and processes of pericytes rich in thin filaments with fusiform densities, resembling smooth muscle cells. Primitive mesenchymal cells were observed to lie among the pericytes. The stroma beyond the subendothelial layer was collagenous and contained fibroblasts. The ultrastructure of the cells and their organization closely resembles human granulation tissue.

Carcinoma of the endometrium: radiation followed immediately by operation.

A prospective study was established in August, 1967, to treat all adenocarcinomas of the endometrium by protocols of preoperative radiation followed immediately by operation. Two hundred and ninety-five women have been treated, 220 of whom had Stage I disease. In these cases, factors known to be associated with survival were studied, and their influence upon survival was noted. Preoperative radium followed immediately by operation was the primary method of therapy. Life tables demonstrated a five-year survival rate of 91 per cent with a low complication rate in patients with Stage I disease. Cell type, degree of differentiation, and depth of myomentrial invasion were the primary factors influencing survival.

Cell-mediated immune responses of breast cancer patients to autologous tumor-associated antigens.

Lymphocyte proliferation assays with autologous tumor material in mixed leukocyte-tumor interactions (MLTI) were employed to monitor tumor-associated cell-mediated immune responses of peripheral blood lymphocytes from patients with carcinoma of the breast. In addition, leukocyte migration inhibition (LMI) assays were employed to compare reactivity to autologous breast-tumor extracts versus allogeneic breast-tumor extracts. Positive lymphoproliferative responses to tumor-associated antigens (TAA) were observed in the MLTI assay with the use of either intact autologous tumor cells or crude extracts (in mug and ng quantities) in 12 of 34 (35%) breast cancer patients studied. Positive reactivity to tumor, but not to normal tissue of reactive patients, was observed in repeated assays. Finally, patients demonstrating positive MLTI responses to autologous tumor extracts likewise responded in LMI assays to these same autologus extracts as well as to allogeneic breast-tumor extracts, but not to non-breast-tumor extracts. Thus breast tumors appeared to possess common TAA among both male and female patients.

Hashimoto's thyroiditis and its relationship to other thyroid diseases.

The experience of the Medical University of South Carolina with Hashimoto's thyroiditis revealed an 8.3 per cent incidence of carcinoma. The most common type of carcinoma was an associated lymphoma. There was an additional 8.3 per cent incidence of a benign adenoma. Needle biopsy as a mechanism for diagnosing Hashimoto's disease, we believe, is inaccurate. A larger aliquot of tissue should be removed for the pathologist if there is a suspicion of an associated tumor. An open biopsy permits the pathologist to make an accurate diagnosis with a minimal amount of morbidity.

Gonadoblastoma: clinicopathologic correlation in six patients.

Six patients with a total of nine gonadoblastomas are presented; three--and possibly a fourth--had dysgerminomatous overgrowth which was massive in two patients. Calcification detected by abdominal films was present in three sufficient for preoperative diagnosis. All patients were found to have a Y stem line on peripheral leukocyte chromosome cultures except one patient, who had a 46 XX/45 XO karyotype. She was found to have Y chromatin bodies in the germ cells of her tumor which was in a normal ovary found at exploration for an ectopic pregnancy. Three were found in virilized phenotypic females investigated for amenorrhea, and two for therapy of pelvic masses due to dysgerminomatous overgrowth. Y chromatin studies are reported on gonadal tissue.

Dysgerminoma

Bone growth into porous high-density polyethylene.

The purpose of this study was to delineate the process by which bone comes to fill the pores of porous high-density polyethylene (PHDPE) implants. PHDPE (450 mu pore size) pellets 4 mm in diameter and 1 cm long were implanted into the femurs of dogs. A bone biopsy procedure was utilized to obtain PHDPE pellets implanted for periods of 3 days through 8 weeks. A one-year biopsy specimen taken from the PHDPE coating on the stem of a canine total-hip prosthesis was also studied. The results demonstrated that significant amounts of bone formed within the PHDPE pellets as early as 14 days after implantation. Bone was identified throughout the specimens after 4 weeks. After 6 weeks, the tissue in hematopoietic marrow. Scanning electron microscopy was utilized in conjunction with light microscopy and microradiography to study the ultrastructural features of the bone ingrowth process.

Histology of the endometrium in long-term use of a sequential oral contraceptive.

Non-atypical adenomatous hyperplasia was found in 13.5% of 111 biopsies obtained from long-term, predominantly black, users of Oracon. An association between length of time of use (95 cycles for those with adenomatous hyperplasia versus 73 for those without) and the development of adenomatous hyperplasia was noted, but there was no association with hypertension or obesity. Cystic glandular dilatation was seen in 63% but was not thought to represent hyperplasia. No more advanced lesions were seen.

Synovial regeneration and articular cartilage changes after synovectomy in normal and steroid-treated rabbits.

Ltttle is known of the effects of synovectomy on articular cartilage. In order to investigate this matter, anterior synovectomy of the knee was performed in thirty-five normal adult rabbits and in thirty-five which were given 25 milligrams of hydrocortisone intramuscularly each week afterwards. The animals were killed at intervals from four to 110 days after synovectomy. Histological examination of the regenerating synovium in both groups showed complete structural and functional regeneration by eighty days in the first group and a delay in regeneration in the steroid group. Sulphur autoradiographs of the articular cartilage of femoral and tibial condyles revealed surface fibrillation and chondrocyte death in 23 per cent of normal knees after eighty days but only 1.8 per cent of knees of animals receiving hydrocortisone. Thus synovectomy in a healthy joint may have an unfavourable effect on the physiology of cartilage by alteration of synovial composition and hyaluronate content in normal joints. Systemically-administered hydrocortisone may reduce this harmful effect in normal cartilage.

Liposarcoma of the maxilla.

A clinically appearing, well-encapsulated lipoma was locally excised from the right posterior upper buccal gingival sulcus of a 24-year-old man and diagnosed histologically as pleomorphic liposarcoma. A right maxillectomy with preservation of orbital function was followed by 5,000 rads of planned postoperative radiation therapy. Four months later the tumor recurred outside of the original site, which has been controlled by further radiation and chemotherapy to these areas.
