Effects of high nitrate intake in rats.

The purpose of this study was to determine the effects of administration of high dose nitrate in drinking water on weight gain, hematological parameters and osmotic fragility in rats. We compared these parameters in 40 rats divided into four groups (one control and three treatment groups). Control animals drank filtered tap water containing a maximum of 10 mg/l nitrate while the treatment groups drank 100 mg/l, 200 mg/l and 400 mg/l nitrate-containing water ad libitum for 60 days. Animals in the treatment groups gained less weight than the control group and the differences between the control and treatment groups were statistically significant (p < 0.05). At the concentration of 100 mg/l nitrate, platelet counts and hemoglobin levels were significantly increased compared with the control group (p < 0.05). At the concentration of 200 mg/l nitrate, erythrocyte counts, hemoglobin and hematocrit levels were significantly increased compared with the control group (p < 0.05). At the concentration of 400 mg/l nitrate, platelet counts were decreased significantly when compared with the first two treatment groups (p < 0.05). There were statistically significant differences in osmotic fragility ratios between treatment groups and the control group (p < 0.05). We concluded that high nitrate intake in drinking water decreases weight gain, affects hematological parameters by inducing bone marrow activity at low doses and inhibiting it at high doses, and increases erythrocyte osmotic fragility.

The effect of octreotide on postoperative adhesion formation.

OBJECTIVE: To investigate the effect of octreotide, a long-acting analogue of somatostatin, on postoperative adhesion formation, because somatostatin inhibits secretion of some growth factors that have modulatory effects on collagen synthesis. DESIGN: An experimental study. SETTING: Surgical Research and Biochemistry laboratories at Hacettepe University, Ankara, Turkey. SUBJECTS: Male Swiss albino mice. INTERVENTIONS: Both sides of a 5-cm ileal segment from Swiss albino mice were scraped 10 times, and transient ischemia was induced by clamping the segmental artery. Animals were injected subcutaneously with 1 mL/d of saline for 3 days (group 1), a single 5-mL intraperitoneal dose of saline (group 2), subcutaneously with 10 micrograms/kg daily of octreotide for 3 days (group 3) or a single 10 micrograms/kg intraperitoneal dose of octreotide (group 4). In half of the animals repeat laparotomy was performed on postoperative day 5. After adhesions were graded, the scraped ileal segments were excised for determination of hydroxyproline quantity. The same procedure was repeated on postoperative day 14 for the remaining animals. OUTCOME MEASURES: Adhesion grading, hydroxyproline levels. RESULTS: On postoperative day 5, the intraperitoneal octreotide group (group 4) had a significantly lower median adhesion score than groups 1 and 2. On postoperative day 14, both octreotide groups (3 and 4) had a significantly lower median adhesion grading than both saline groups (1 and 2). Hydroxyproline levels of the groups were not significantly different on either day 5 or day 14. CONCLUSION: Octreotide has a beneficial effect in decreasing adhesion formation in the early postoperative period.

Experimental study of the effect of different meshes on bacterial translocation.

Implantation of intraabdominal biomaterials promotes bacterial translocation (BT). In this study we planned to determine the effect of different types of mesh on the induction of BT. Swiss albino mice were divided into four groups: control, polypropylene (PP), polyglactin 910 (PG), and dura mater (DM). At hour 0, an abdominal wall defect was created in all animals. In the control group the defect was closed primarily. PP, PG, and DM meshes were used to repair the defects such that the intestines were in contact with the mesh in the remaining three groups. BT was evaluated 4, 24, and 48 hours later. At 4 hours total BT increased in the PP group compared with that in the control (p = 0.0321) and DM (p = 0.0098) groups. At 24 hours the PG group had increased BT compared with the controls (p = 0.0392) and the DM group (p = 0.0274), whereas the PP group had increased BT compared with the DM group only (p = 0.0477). At 48 hours both PG and PP groups had increased BT compared with controls (p = 0.0431 and p = 0.0001, respectively); the PP group had also increased BT compared to the DM group (p = 0.001) and the PG group (p = 0.017). In the control, DM, and PP groups there was no intragroup statistical difference. In the PG group there was a significant increase in BT at 24 hours compared to that at 4 hours (p = 0.0274). Prosthetic meshes led to increased BT compared to that with the DM mesh. This effect might be attributed to the different organic and physical properties of the meshes.

Effect of intraperitoneal and extraperitoneal insertion of mesh on bacterial translocation: does it make a difference?

OBJECTIVE: To asses the effect of insertion of mesh, with or without contact with the peritoneum, on the induction of bacterial translocation. DESIGN: Open experimental study. SETTING: Surgical research laboratory, Turkey. SUBJECTS: 158 Swiss albino mice. INTERVENTIONS: A defect in the abdominal wall was created. In the control group, the defect was closed primarily. In the extraperitoneal group, polypropylene mesh was sutured over the abdominal wall after primary closure of the peritoneum and in the intraperitoneal group, polypropylene mesh was sutured to close the created defect so that it was in contact with the intestines. MAIN OUTCOME MEASURES: Bacterial translocation at 4, 24 and 48 hours. RESULTS: Insertion of mesh in contact with the peritoneum led to increased bacterial translocation to mesenteric lymph nodes at 4 (p = 0.02) and 48 (p = 0.03) hours compared with insertion without contact. CONCLUSION: Contact between a foreign body and the peritoneum is required to induce bacterial translocation.

Effect of hypertonic saline and alcohol on viability of daughter cysts in hepatic hydatid disease.

OBJECTIVE: To test the efficacy of hypertonic saline (20%) and absolute alcohol on the integrity of daughter cysts and the viability of the protoscoleces contained in these cysts. DESIGN: Experimental study. SETTING: Teaching hospital, Turkey. MATERIAL: 80 daughter cysts obtained from two patients with Gharbi type III hydatid cysts of the liver. INTERVENTIONS: The cysts were divided into two groups, in the first of which cysts were placed into hypertonic saline and in the second into absolute alcohol; they were kept there for 5, 15, 30 or 60 minutes. MAIN OUTCOME MEASURES: Integrity of the cyst wall and viability of the contents were evaluated using a vital staining technique with 0.1% eosin. RESULTS: Neither hypertonic saline nor absolute alcohol solution had any effect on the integrity of the daughter cysts or the viability of the protoscoleces. CONCLUSION: Percutaneous drainage of type III hydatid cysts can lead to high recurrence rates.

Experimental study of the effects of splenectomy and partial splenectomy on bacterial translocation.

BACKGROUND: The aim of this study is to evaluate the effect of splenectomy and partial splenectomy in a burn-induced bacterial translocation model and to study Kupffer cell (KC) morphology and number. METHODS: Mice were divided into sham-burn and burn groups. Each group was also subdivided to sham-splenectomy, partial-splenectomy, and splenectomy subgroups. At day 0, operations were performed. At the postoperative 10th day, a sham burn or burn injury was made in all animals. Twenty-four hours later, cultures for bacterial translocation were obtained and livers were evaluated for the quantity and morphology of KCs. RESULTS: Burned-splenectomized animals had significantly decreased bacterial translocation when compared with sham-splenectomized animals (p = 0.031). Interestingly, in both the sham-burned and burned groups, splenectomy subgroups had significantly higher numbers of KCs compared with partial-splenectomy and sham-splenectomy subgroups (p<0.00000). Burn injury caused a significant decrease of KC numbers in all subgroups compared with their correspondent sham-burned subgroups (p<0.05). CONCLUSION: Results revealed that splenectomy decreases bacterial translocation and also increases the number of KCs.

Do cochlear nuclei contribute to auditory lateralization? A stereological evaluation of neuron numbers.

The aim of this study was to estimate the total number of cells in the subdivisions of the cochlear nucleus (CN) of the rat with unbiased stereological methods. The total number of neurons was determined in both the ventral cochlear nucleus (VCN) and the dorsal cochlear nucleus (DCN) to compare the right and left sides. The total cell numbers were 15,280 in the left VCN, 15,400 in the right VCN, 10,260 in the left DCN, and 10,860 in the right DCN. Comparison of the right and left major subdivisions of the CN of the rat showed that there was no significant difference between the right and left sides of the rat CN. This result indicates that the CNs do not contribute to auditory lateralization in the rat in regard to cell numbers.

Scolicidal agents in hydatid cyst surgery.

Injecting scolicidal solutions into the hydatid cyst and packing the operative field with sponges soaked in scolicidal agents have been used to avoid dissemination of the parasite during surgery. In the first part of this invitro study, we tried to determine the scolicidal property of various agents in different concentrations and exposure times. In the second part, we tested whether sponges soaked in different type and concentrations of scolicidal agents have any role beyond being a mechanical barrier. 20% saline, 3% hydrogen peroxide, 1.5% cetrimide-0.15% chlorhexidine (10% Savlon), 95% ethyl alcohol, 10% polyvinylpirrolidone-iodine (Betadine) and their further dilutions were used in this study. Protoscoleces were obtained from the cyst containing livers of the sheep and viability was determined with dye-uptake (0.1% Eosin) and flame cell activity. Savlon was found to be the least concentration dependent scolicidal agent among those studied. Scoleces sprayed on sponges soaked in 20% saline, 95% ethyl alcohol, Betadine and 3% hydrogen peroxide were killed after 15 minutes. 3% and 10% saline and normal saline were ineffective. Sponges work not only as a mechanical barrier but also as a chemical one if the agent is chosen correctly. In purely cystic hydatid liver disease, the risk of dissemination of the cyst contents can be avoided by injection of a potent scolicidal agent such as Savlon.

Thyroidectomy with and without drains.

Drainage after thyroid surgery has been widely used to prevent life-threatening complications. However, it is well known that drains do not always prevent haematoma formation. In this retrospective study, we reviewed our experience with 1057 thyroidectomies with and without drains in order to assess this issue. Between 1983 and 1993, 520 patients were closed with drains and 537 patients without drains after thyroid surgery. The indications for drainage of the selective period included wet operative field and large areas of dead space at the conclusion of the operation. Reoperation for bleeding was done in twelve patients in the drainage group and two patients in the non-drainage group (p < 0.05). Wound infection was seen in seven patients in the drainage group and none in the no-drainage group (p < 0.05). This study suggests that routine drainage after thyroid surgery is not necessary and a selective policy can be applied safely.

The zipper-mesh method for treating delayed generalized peritonitis.

Recently, many articles have been published related to the "open abdomen" concept which is performed particularly on patients with delayed generalized peritonitis. Since most arguments still support the effectiveness of this method and the results have been mostly favourable, it was decided to use the technique on 14 patients with peritonitis. Commercial zippers either with or without mesh were used on all. Four patients, however, died in the very early postoperative period and were excluded from the study. Others were examined as to the operative findings, period of delay, concomittant diseases, primary operations, indications of the technique, the number and type of the reinterventional procedures, status of remote organ failure, expected and realized mortality. Forty laparatomies were performed on 10 patients. In addition to obtaining a clean intraperitoneal cavity, some additional procedures such as anastomotic repair or gastroenterostomy could be done. The mean period of ventilator support was 27 h, the ambulation period was 3 days. The start of enteral nutrition early was able to be achieved more easily. In the patients whose expected mortality rate was as high as 70%-90%, a rate of 40% was established in those ten patients.

Perforated appendicitis in an incarcerated scrotal hernia--a case report.

A 72-year-old man in whom perforated appendicitis developed within an incarcerated scrotal hernia sac is presented herein, because of both its rarity in the literature and the difficulty encountered in establishing a diagnosis. If such an infection is limited to within the hernial sac, the prognosis is benign, however, the presence of peritoneal contamination affects the prognosis negatively. Moreover, there is no supplemental risk in performing a primary hernia repair following appendectomy in the same session.

Hydrochloric acid for treating metabolic alkalosis.

Six patients with severe metabolic alkalosis were treated with intravenous hydrochloric acid (HCl) infusion. HCl was given through a central venous catheter, at a concentration of 0.1 mEq per ml. At least two of the following criteria were considered for initiation of the therapy: An arterial pH of greater than 7.45, a base excess (BE) of greater than +7 mmol/L, a PaCO2 of greater than 50 mmHg. The HCl amount was calculated using the BE formula, however, two thirds was infused for avoiding excessive acid loading. Patients were monitored by the blood gases, serum electrolytes, hemoglobin, hematocrit, bilirubin determinations and blood smear findings. While a significant decrease was noticed in pH and BE values, moderate changes were detected in PaCO2 due to different ventilatory status of the cases. All laboratory test results remained within normal limits and no complication was encountered. The advantage of the therapy is that less volume is needed for the correction of alkalosis, particularly in the cases requiring fluid restriction. HCl therapy, moreover, is a safe and time-saving method because of having rapid response to the treatment in the critically ill surgical patients.

Chemical burn caused by topical vinegar application in a newborn infant.

Although there is increasing interest in "alternative medicine," including nontraditional and homeopathic remedies, all around the world, they are not always safe and beneficial and may have adverse effects. We report a chemical burn caused by vinegar applied topically to lower body temperature in a febrile newborn and discuss briefly chemical skin burns caused by organic acids.

Prevention of intraabdominal adhesions produced by polypropylene mesh.

Polypropylene mesh (PPM) is widely used in ventral hernia repair, however is also associated with visceral adhesions when the mesh is exposed to intraabdominal organs. In this study, a composite mesh [ePTFE (expanded polytetrafluoroehylene) + PPM] and sodium hyaluronate/carboxymethyl cellulose (NaHA/CMC) membrane laid under PPM are evaluated in terms of adhesion formation in a rat model of ventral hernia repair. In this experimental study, a 2 x 1 cm of peritoneum and underlying muscle defect was created at the right side of the anterior abdominal wall of 37 male Wistar albino rats. These defects were repaired with 2.5 x 2 cm PPM in group 1 (n = 13), with composite mesh in group 2 (n = 12) and with PPM+NaHA/CMC in group 3 (n = 12). Rats were sacrificed after 14 days and the prosthetic materials were examined for the calculation of surface area percentage covered by adhesions, for organ involvement in the adhesions and for histological evaluation. There was a reduction in the adhesion-covered area in group 2 and group 3. Organ involvement was predominantly seen in group 1. Neoperitoneum was perfect in group 2. Fibrosis and inflammation were higher in group 1. All groups showed adhesiogenic potential to some extent. This potential was maximum with PPM. Both ePTFE and NaHA/CMC displayed similar effects in preventing adhesion formation.

Chronic idiopathic neutropenia associated with gingival enlargement.

A girl with chronic idiopathic neutropenia who developed gingival enlargement at seven years of age is presented. Intraoral examination revealed generalized gingival inflammation with a tendency to bleeding and inflammatory gingival enlargement localized to the anterior region. A considerable amount of bacterial plaque was noted on the teeth. There were also 4-5 mm pocket depths around the first molars. Radiographic examination also indicated the presence of incipient bone loss around the first molars in both jaws. The patient, who was diagnosed as localized prepubertal periodontitis with generalized gingival inflammation and anterior gingival enlargement, accentuates the importance of evaluation of periodontal status in patients with chronic idiopathic neutropenia, to avoid the destruction of supporting structures of the dentition.
