Imaging temporomandibular joint abnormalities in patients with rheumatic disease. Comparison with surgical observations.

The preoperative examination findings in the soft tissue and bone of 22 temporomandibular joints of 15 patients with rheumatic disease were compared with the diagnosis after TMJ surgery. Agreement was found in 15 joints with rheumatic involvement and in 4 with internal derangement. In 5 (with unsuccessful arthrotomography) of the 15 rheumatic joints, magnetic resonance imaging showed destruction of disks with soft-tissue replacement, corresponding to fibrous tissue/ankylosis observed at surgery. Bony fusions in 2 of these joints were depicted with computed tomography. In the remaining 10 joints, arthrotomography showed irregularly outlined small compartments corresponding to synovial proliferations observed during surgery. Similar arthrotomographic interpretation, however, was made in 2 of 3 temporomandibular joints with imaging-surgery disagreement; surgery showed fibrous adhesions. In the third joint with unsuccessful arthrotomography, magnetic resonance imaging showed internal derangement but no synovial proliferations that were surgically observed. As experienced with other joints, synovial proliferations (or fibrous adhesions) could not be depicted with magnetic resonance imaging. Thus, differentiation between internal derangement with and without rheumatic involvement could be impossible with both arthrotomography and unenhanced magnetic resonance imaging.

Efficacy and skeletal side effects of two years' acitretin treatment.

In this prospective study, 51 patients suffering from psoriasis and ichthyosis were treated with acitretin for 2 years. The average dose was 0.5 mg/kg/day. We have evaluated the efficacy and side effects, focusing on skeletal side effects. X-ray examinations were done before treatment and after 1 and 2 years. Forty-five patients completed the study. Acitretin had a good clinical effect, with 75% improvement in 35 of the patients. Apart from the well-known side effects affecting the mucous membranes, one patient developed biopsy- proven toxic hepatitis. In 2 patients we observed unusual skeletal calcifications located in the forearms and in the hip. These were considered to be caused by the drug. In this study, which included patients up to 71 years, only radiographs of the pelvis and the forearms were of value as routine follow-up films.

Acitretin

CT appearances of normal and obstructed submandibular duct.

CT scans of 40 patients without pathology in the floor of the mouth or the submandibular glands were reviewed. Intraglandular ducts were visualized in 27 and extraglandular ducts in 3 patients. The CT appearances of dilated submandibular ducts are described in 4 patients with proven causes of obstruction. Widening of the narrow gutter between the mylohyoid and hyoglossus muscles in one scan level is a prominent feature. An intra- or extraglandular duct diameter of 3 mm or more indicates possible obstruction, and the CT images should be scrutinized to reveal the cause.

CT and angiography in adrenocortical carcinoma.

CT and angiography were performed in 15 patients with adrenocortical carcinoma. The tumors had a mean diameter of 11 cm (range 4-20 cm). At CT, the 8 largest tumors were ill-defined, and in these, the organ of tumor origin could not be established. Angiographically the correct organ of tumor origin was established in all but one patient. It is concluded that CT is excellent in showing the extent of an adrenal tumor, but is often unable to predict the organ of origin in large tumors. Angiography is still of great value in the preoperative work-up in patients with large adrenocortical carcinomas for correct identification of tumor origin and for vascular mapping.

High resolution CT and bronchography in the assessment of bronchiectasis.

To elucidate the reliability of CT in the assessment of bronchiectasis, a retrospective study of high resolution CT and bronchography was carried out. A segment by segment comparison of 259 segmental bronchi from 70 lobes of 27 lungs in 19 patients was performed using bronchography as standard. CT was positive in 87 of 89 segmental bronchi with bronchiectasis giving a false-negative rate of 2%. CT was negative in 169 of 170 segmental bronchi without bronchiectasis at bronchography, giving a false-positive rate of 1%. There was agreement between the two modalities in identifying the different types of bronchiectasis.

Isolated cavernous bodies.

Corpus cavernosography in a patient with erectile dysfunction revealed absence of communication between the 2 cavernous bodies. No similar case was found in a review of 100 consecutive cavernosograms or in the literature. The abnormality may be a source of error in pressure recordings or in intravenous injection of pharmaceutical agents.

Osseous temporomandibular joint abnormalities in rheumatic disease. Computed tomography versus hypocycloidal tomography.

Computed tomography (CT) of the temporomandibular joint (TMJ) was compared with hypocycloidal tomography in 30 joints of 15 adults with rheumatic disease. CT included 1.5 mm thick axial scans (at 1.0 mm intervals) with reformatted oblique sagittal and oblique coronal sections. Multisection (at 2.0 mm intervals) tomography included oblique sagittal and occasionally, oblique coronal sections. CT demonstrated bone abnormalities in 21 and tomography in 20 joints, indicating high agreement between the imaging modalities regarding number of abnormal TMJs. Bone structures were, however, better visualized by multiplanar CT due to superior contrast and spatial resolution particularly in the most lateral and medial parts of the joint, indicating superiority of CT for depicting subtle bony TMJ abnormalities in patients with rheumatic disease.

Rhabdomyoma of the tongue. Report of a case with light microscopic, ultrastructural and immunohistochemical observations.

Adult rhabdomyoma is a benign neoplasm of skeletal muscle origin, which mainly occurs in the head and neck region. Because of its rarity the diagnosis may be missed, especially if cross-striations cannot be demonstrated by light microscopy. By applying supplementary diagnostic evaluation with immunohistochemistry and electron microscopy, the diagnosis is established. Positive staining with desmin unequivocally demonstrates that the tumour cells are derived from muscle tissue. The presence of myofibrils, Z-bands and numerous mitochondria by ultrastructural examination are characteristic features of this tumour.

Radiological appearance of the ampulla of Vater.

The localized dilatation below the confluence of the bile and pancreatic ducts is known as the ampulla of Vater. The purpose of the present study was to delineate the radiological features of the choledochopancreaticoduodenal junction, with special reference to ampullar formation in this region. This was done by specimen radiography and review of the films from 255 examinations with endoscopic retrograde cholangiopancreatography. Full evaluation was possible in 200 cases. Ampullar dilatation was observed in only 9 (5.1%) of the 175 patients with a common terminal opening. No ampullar dilatation was observed in the 25 patients with separate openings of the duct. The appearance in the presence of ampullar dilatation is described. The distal end of the common bile duct, below its confluence with Wirsung's duct, has been known as the ampulla of Vater for nearly 130 years. This is a most important diminutive region of the human body, but there is little in the literature concerning its radiological presentation. In this report a radiological study is presented on the choledochopancreaticoduodenal junction.

Calcifications of the palatine tonsillary region: CT demonstration.

The scans of 100 consecutive patients referred for computed tomographic examinations that included the oropharynx were reviewed with regard to calcifications of the palatine tonsillary region. Calcifications were found in 20% of the female patients and 13% of the male patients. Ten patients had one calculus, and six had two or more. The sizes of the calculi ranged from 1 to 7 mm. Tonsillary calculi must be included among the diagnostic possibilities when survey radiographs of the neck show soft-tissue calcifications.

Adult Wilms tumor. A case report.

We report a case of Wilms tumor in an adult. Pictorial and descriptive pathology information is included. The tumor was stage IV with pulmonary metastasis at diagnosis. The patient was treated with surgery and combined chemotherapy according to protocols used in childhood Wilms tumor. The patient is clinically free of disease at 21 months postoperatively.
