[AS & A (aqua-stream and aspirator) in neurosurgery: comparison with laser and CUSA].

A new instrument, the aqua-stream and aspirator (AS & A) is useful and safe for microscopical neurosurgery. The apparatus is simple and small. The handpiece is small and light as well as being easy to manipulate. Sterilization is easy and safe. Both streaming and aspirating tubes of AS & A are thin, so that operative field is not obstructed by the operative microscope. The operative field is kept dry and clean by means of washing the work of AS & A. It is economical, because less gas and saline solution than usual are needed during the usual craniotomy (a one liter soft bottle). It is superior to laser and CUSA for the removal of deeply seated soft tumors.

A case of oromandibular-digital hypogenesis syndrome.

The oromandibular-digital hypogenesis syndrome is characterized by failure of development of the intraoral region and the distal extremities. A 12 year old female with an anomaly of the lingual fraenum, hypodontia of the lower incisors and defective fingers and toes was referred to our clinic. The aetiology of the disorder is still obscure. The author has reviewed the literature and discusses the morphological variation of the lingual fraenum.

[A case of amebic liver abscess rupturing into the stomach].

A forty-eight year-old man complained of upper abdominal pain and diarrhea with mucinous bloody stool. He had not been abroad. Except high fever, anemia, leukocytosis and elevated rate of erythrocyte sedimentation, laboratory findings were not abnormal. Gastrofiberscopy showed the protrusion of gastric mucosa with a hollow on its surface in the angle. Abdominal CT scan and echogram revealed an abscess in the right hepatic lobe and an abscess in the left lobe. Ulceration and small protrusion of the rectal mucosa were found by romanoscopy. Stool examinations could not reveal amebas, but serological test for amebiasis by the Ouchterlony method showed positive. Under the diagnosis of the perforation into the stomach of amebic liver abscess, he was treated with Metronidazole and tetracycline. But, as a diffuse shadow appeared in the right thoracic cavity on the chest x-ray films with bloody sputa, perforation of amebic liver abscess to the right thoracic cavity was suspected. Laparotomy showed the communication of the abscess to gastric lumen. The post operative course was uneventful. We reported a case with the rare complication of amebic liver abscess.

Homotypic pair formation during conjugation in Tetrahymena thermophila.

In the ciliate Tetrahymena thermophila, conjugation has been believed to occur only between cells of different mating types. We found the formation of homotypic pairs during normal conjugation by using micronuclear morphological markers. Homotypic pairs formed preferentially during the first 10 min following the first pair formation and comprised about half of the pairs. These results suggest the involvement of mating-type non-specific adhesion of cells in the initial step of conjugation. Homotypic pairs apparently persist for at least 30 min and then separate into single cells. Homotypic pairs are also formed when conjugant pairs re-form after mechanical separation of heterotypic pairs. Five kinds of glycosidases, three kinds of proteases and phospholipase C showed no effect on either the formation of homotypic pairs of their separation. The relation between the mating-type substances and the molecules responsible for mating-type non-specific adhesion of cells is discussed.

Possible participation of Ca2+, Mg2+-dependent endonuclease in liver DNA fragmentation after N-methyl-N-nitrosourea treatment.

We examined the fragmentation of DNA treated with N-methyl-N-nitrosourea under conditions in which Ca2+, Mg2+-dependent endonuclease is active. The molecular mass of DNA found in mouse liver slices treated with methylnitrosurea in the presence of Ca2+ plus Mg2+ was 4 X 10(5) Da. Similar results were obtained with a reconstituted system containing partially purified Ca2+, Mg2+-dependent endonuclease and methylnitrosurea-treated DNA. The enzyme extensively cleaved methylnitrosurea-treated DNA, compared with non-treated DNA. The methylnitrosurea-treated nuclear proteins obtained from mouse liver nuclei had no effect on the DNA fragmentation by the enzyme. Using closed-circular DNA treated with methylnitrosurea, the enzyme produced single-strand cuts in the DNA, as was seen in non-treated, closed-circular DNA, however, the rate of hydrolysis was increased. Ca2+, Mg2+-dependent endonuclease thus warrants further investigation, with regard to the precise mechanism of extensive degradation of DNA in cells treated with carcinogenic alkylating agents.

Detection of early gastric cancer by panendoscopy.

With the implementation of a small-caliber panendoscope which was made according to our suggestion, we gradually replaced the conventional barium meal as the first line of the upper gastrointestinal tract examination by panendoscopy after 1976. The efficacy of panendoscopy for the early detection of gastric cancer in our 8-yr experience was investigated. In 910 matched pairs of patients subjected to "screening" fluoroscopy or panendoscopy two and six cases of early gastric cancer were detected respectively. Panendoscopy was superior to fluoroscopy in its sensitivity for detection of gastric cancer (86.4% vs. 80.0%), in its specificity (99.5% vs. 97.0%) and in its low false-positive rate (17.4% vs. 69.2%). After 1976, the detection of early gastric cancer increased year by year. The frequency of early gastric cancer detected by panendoscopy was about three times that of early gastric cancer detected by fluoroscopy (61.5% vs. 21.7% of all cancer). Early cancer located in the proximal third of the stomach and superficial esophageal cancer were detected only by panendoscopy. All gastric cancers that were overlooked by panendoscopy were in the gastric body. How to prevent such overlooking is discussed. In 24,000 panendoscopies we have encountered six complications (0.03%). The cost of panendoscopy and fluoroscopy is 6,500 yen and 11,000 yen respectively.

Effects of protease inhibitors on early events in the conjugation of paramecium caudatum.

The effects of 11 different protease inhibitors on mating reactivity and pair formation in the conjugation process of the ciliated protozoan Paramecium caudatum were investigated. Of the 11 inhibitors tested, six microbial and three synthetic inhibitors showed no effect on either pair formation or mating reactivity even at their highest concentrations. Soybean trypsin inhibitor and ovomucoid also showed no effect on pair formation even though they temporarily repressed mating reactivity. These results indicate that the process of pair formation during the conjugation process does not involve an extracellular proteolytic reaction.
