Neurotoxic effects of the fungicide triphenyl-tin acetate were examined in pups of mothers treated perorally on day 7-15 of gestation. The gait and development of motor coordination did not differ from those of control animals, in spite of the high mortality rate of control pups during the nursing period. Spontaneous locomotor activity of treated pups at the age of 23 and 36 days was increased, however by the age of 90 days activity returned to control levels. Conditioned avoidance was acquired more rapidly, but was also extinguished sooner in animals born from, the nursed by poisoned mothers than in control.
The effect of alcohol ingestion on the prognosis of chronic liver diseases was studied'n. Among 880 patients with chronic liver diseases the percentage of alcoholics was high, 33%, compared to 3.2% in the control group. The period of survival time is decreased by one half in patients with regular alcohol ingestion. According to 'in vitro' investigations they report that in chronic active hepatitis the low Et-rosette forming cell number is further decreased after incubation with alcohol, and ethanol inhibits the immune-stimulating effects of levamisole on Ett-rosette forming cells.
The relationship between doctor and patient receives increasing interest in the public press and in scientific discussions. An increasing number of intervention studies aiming at an enhancement of doctor-patient communication is currently in action. This paper examines the potential of a patient satisfaction questionnaire (PSQ) to assess the need for such an intervention study or to identify those residents who would profit most from an intervention. We investigated a sequential sample of 257 outpatients at the Outpatient Clinic of a Medical University Clinic (137 men, 120 women, mean age 42 +/- 17 years) who were seen by eleven residents for the first time. We used the german translation of an american 14-item questionnaire. Typical psychosocial characteristics of the sample were also assessed. 73.2 percent of the patients are "satisfied" or "very satisfied" with various physician activities during the examination, the mean sum score of the PSQ is 60.1 +/- 7.1 points (range: 39-70 points; maximum score: 70 points). Residents reach a mean total score between 58 and 63 points. Neither in single items nor in the sum score was there any significant difference between residents. We found no differences in PSQ values attributable to sociodemographic variables. The results show that the PSQ is not able to identify those residents to whom intervention studies should be offered in order to enhance psychosocial skills. Its use as an evaluation tool seems also limited because patient satisfaction is very high already prior to an intervention.
The authors describe a case of intraperitoneal anorectal injury. Such injuries now have an improved survival rate as a result undoubtedly of superior resuscitation, early operative management and early use of antibiotics. Civilian life rectal wounds are usually of the penetrating type. Major complications, both of intra and extraperitoneal injuries, are related to infection. The surgical management of these lesions is primarily a proximal diversion of the fecal stream. This is accomplished by a divided--end colostomy. Local wound care must be given for the inevitable infected foreign tract. When sepsis occurs, the surgeon must be prepared to handle it adequately by appropriate incision and drainage.
The authors report their experience of the use of mechanical staplers in total gastrectomy. The advantages of their use can above all be seen in anastomosis, in particular in high-risk areas like esophago-jejunal anastomosis, in which there is a lower incidence of dehiscence and mortality than when manual suturing techniques are used.
The paper reports a case of intestinal occlusion due to adenocarcinoma of the large intestine as a sequel to ureterosigmoidostomy for exstrophy of the bladder which was subsequently converted to ureteroileocutaneostomy. Inadequate follow-up did not allow an early diagnosis to be made. The authors underline the need for annual colonoscopy in all patients suffering from or with previous ureterosigmoidostomy given the high incidence of malignant transformation of the colon.
The results of abdominal and pelvic pouch operations are described in surgical and urological material. The lateral ileal pouch (Fonkalsrud) was used in 8 patients with diffuse colonic polyposis and in 4 cases of colitis ulcerosa. The second step of this two-phase operation was completed in 7 patients. Five complications were observed. One patient died of faecal peritonitis. Some sort of pouch operation was carried out in 22 patients after radical cystectomy in urological practice. The indication was tumour of the urinary bladder in 18, tuberculosis in 2 and interstitial cystitis in 2 cases. The distribution of the different types of reconstructions was as follows: Kock's pouch in 11, Mainz-pouch I in 5, and Mainz-pouch II in 6 patients. Early complications were observed in 3 cases with kock's reservoir, in 2 with Mainz I and in 2 with Mainz II intervention. Overnight incontinence appeared as a late complication in 30-60% of the cases. One of these patients died of a suture-line insufficiency after a Mainz-I substitution.
