Increased telomerase activity in hyperplastic nodules and hepatocellular carcinomas induced by a choline-deficient L-amino acid-defined diet in rats.

Activation of telomerase has been reported in several human cancers, including hepatocellular carcinomas (HCCs). We investigated telomerase activity during hepatocarcinogenesis induced by a choline-deficient L-amino acid-defined (CDAA) diet in rats. Male F344 rats were given a CDAA diet or a choline-supplemented L-amino acid-defined (CSAA) diet from 6 weeks of age for 75 weeks, and subgroups were killed 10 weeks, 50 weeks and 75 weeks after the beginning of the experiment. Hyperplastic nodules and HCCs were noted in rats fed a CDAA diet for 50 weeks and 75 weeks, respectively. Normal control liver specimens were obtained from 6-week-old rats. Telomerase activity was assessed by using a telomeric repeat amplification protocol (TRAP). Normal liver and background parenchyma of rats fed either of the diets for 10 weeks or 50 weeks showed weak telomerase activity. In contrast, markedly increased levels were demonstrated in hyperplastic nodules and HCCs. These results suggest that increased telomerase activity may be a biological feature of preneoplastic lesions that evolve to HCCs in rat liver.

[A device for rapid elevation of plasma methotrexate (MTX) concentration and its maintenance in high-dose MTX therapy].

For 5 osteosarcomas and one synovial sarcoma, 24 courses of high-dose methotrexate (MTX) therapy were performed. At MTX doses of 300 mg/kg with several infusion patterns, the plasma MTX concentrations were measured by fluorescence polarization immunoassay. In the various types of infusions, five hours oblique and five hours bolus infusions were well maintained at 1,000 micromol/l for several hours. In particular, five hours bolus infusion of MTX needed only 20 minutes to reach 822 micromol/l and maintained 1,000 micromol/l during almost the entire infusion period. Optimization of chemotherapy for sarcomas by MTX requires individual adaptation of the infusion pattern.

Group-specific component subtyping in semen and seminal stains by conventional isoelectric focusing.

Group-specific component (GC) subtyping in semen and seminal stains was carried out using isoelectric focusing in carrier ampholyte-generated pH gradients and immunoblotting. In serum samples the anodal bands of GC 1F and of GC 1S disappeared by neuraminidase treatment, but in semen samples these bands remained unchanged after such treatment. The GC 2 type in semen exhibited two bands: the main GC 2 band and another fast band which focused at the position of the cathodic band of GC 1F. These seminal GC bands were unaffected by enzyme digestion. Reliable subtyping was possible in seminal stains stored at 4 degrees C for up to 10 weeks, at room temperature for up to 8 weeks, and at 37 degrees C for up to 5 weeks. The GC subtyping by conventional isoelectric focusing after neuraminidase treatment is simple, economical and useful in medicolegal examination of seminal stains.

Nuclear DNA content and nucleolar organizer regions in colorectal cancer.

The prognostic value of nuclear DNA content and argyrophilic nucleolar organizer regions (AgNOR) is still controversial in colorectal cancer. Sixty patients with colorectal cancer were studied by flow cytometric DNA analysis and AgNOR measurement, and their prognostic significance was tested. DNA index was closely linked to depth of invasion and lymph node metastasis, while AgNOR count did not correlate with such parameters. The survival curve was strongly influenced by depth of invasion, lymph node metastasis, and Dukes' stage but was not affected by DNA ploidy and AgNOR count. These results indicate that neither DNA ploidy nor AgNOR count correlates with survival of patients, although DNA ploidy is linked to progression of colorectal cancer.

Malignant lymphoma of the rectum treated preoperatively with hyperthermia and radiation.

A 58-year-old Japanese woman with primary malignant lymphoma of the rectum was treated preoperatively with radiation and intraluminal hyperthermia, after which abdominoperineal rectal amputation (Miles' operation) was done. The rectal tumor disappeared and there were no lymphoma cells in the resected specimens. The postoperative course was smooth and she is being followed in the outpatient department. At this writing, five years after the surgery, she remains well.

Prevention by methionine of enhancement of hepatocarcinogenesis by coadministration of a choline-deficient L-amino acid-defined diet and ethionine in rats.

The effects of methionine on hepatocarcinogenesis induced by coadministration of a choline-deficient L-amino acid-defined (CDAA) diet and ethionine were examined. F344 male rats were divided into 4 experimental groups. Groups 1 and 2 received the CDAA diet and a choline-supplemented L-amino acid-defined (CSAA)++ diet, respectively. Group 3 received the CDAA diet containing 0.05% ethionine, and group 4 the CDAA diet containing 0.05% ethionine and 0.47% methionine. Animals were killed after 12 weeks of treatment. Histologically, the CDAA diet induced intracellular fat accumulation and foci. In contrast, ethionine caused not only foci, but also hyperplastic nodules, cholangiofibrosis and the proliferation of oval cells without such fat accumulation. Methionine abolished the development of all of the liver lesions induced by coadministration of the CDAA diet and ethionine. To investigate the effects of methionine on induction of c-myc and c-Ha-ras expression, as well as generation of 8-hydroxyguanine (8-OHGua) and 2-thiobarbituric acid-reacting substances (TBARS), by coadministration of the CDAA diet and ethionine, subgroups of 3 to 5 animals were killed at 2, 4, 8, or 11 days after the beginning of the experiment. Coadministration of the CDAA diet and ethionine markedly enhanced the level of expression of c-myc and c-Ha-ras, 8-OHGua formation and TBARS generation as compared with the CDAA or CSAA diet within 11 days, and methionine blocks these actions. These results indicate that addition of methionine prevents the induction of c-myc and c-Ha-ras expression, 8-OHGua formation and TBARS generation, as well as hepatocellular lesions, by coadministration of the CDAA diet and ethionine in rats, and suggest a possible involvement of oxidative stress and gene expression in hepatocarcinogenesis by these agents.

[An analysis of factors related to recurrence of superficial bladder cancer after transurethral resection].

A total of 205 patients with primary superficial bladder cancer (Ta, T1) followed more than 3 years were retrospectively analyzed for factors related to recurrence of tumors after transurethral resection. Patients age were 25 to 90 years old, average 61 years old, and there were 160 males and 45 females. Initial tumor grades were G0 in 4 patients, G1 in 48, G2 in 134 and G3 in 19. Seventy four patients had Ta tumor and 131 had T1. Initial treatments were transurethral resection (TUR) alone in 137 patients. TUR with intravesical chemotherapy in 64, with BCG therapy in 7 and others in 7. Factors examined included age, sex, chief complaint, shape, size, and number of tumors, tumor distribution (single area or multiple area), histological grade, stage and intravesical chemotherapy. Overall non-recurrent rate were 81.7% at 1 year, 60.7% at 3 year, 53. 8% at 5 year and 44.2% at 8 year. Five-year non-recurrent rate according tumor factors, showed significant difference regarding tumor size (< 1 cm or 1 cm <: P = 0.027), tumor number (single or multiple: P = 0.004), tumor distribution (single area or multiple area: p = 0.002), histological grade (< G1 or G2 < : p = 0.001) and stage (Ta or T1: p = 0001). However, there were no significant difference regarding factors of age, sex, chief complaint, tumor figure and presence or absence of intravesical chemotherapy. This results suggested that the tumor factors of size, number, tumor distribution, grade and stage were highly related to intravesical tumor recurrence of superficial bladder cancer.

[A study of tumor location and prognosis in renal pelvic and ureteral cancer].

A total of 245 patients with renal pelvic and ureteral cancer (transitional cell carcinoma) were retrospectively analysed for tumor location and prognosis. In 133 renal pelvic cancer patients, 34 patients (25.6%) had tumor in lower calyx, 33 patients (24.8%) in renal pelvis, 31 patient (23.3%) in upper calyx, 21 patients (15.8%) in whole renal pelvis and 7 patients (5.2%) in middle calyx, respectively. In 128 ureteral cancer patients, 60 patients (46.9%) had tumor in lower ureter, 27 patients (21.1%) in distal end of ureter, 26 patients (20.3%) in middle ureter, 12 patients (9.4%) in upper ureter and 3 patients (2.3%) in whole ureter. In combination of tumor location, 101 patients (41.2%) had tumor in only renal pelvis, 94 patients (38.4%) had in only ureter, 14 patients (5.7%) had in renal pelvis and ureter, 19 patients (7.8%) had in renal pelvis and bladder, 12 patients (4.9%) had in ureter and bladder, and 5 patients (2%) had in renal pelvis, ureter and bladder. Five year survival rate of renal pelvic cancer according to tumor location were 55.9% in upper calyx tumor, 60.8% in middle calyx tumor, 63.8% in lower calyx tumor, 60.2% in renal pelvic tumor and 63.8% in PUJ tumor, respectively. There were no significant difference between those 5 groups. Five years survival rate of ureteral cancer according to tumor location, 90% in upper ureteral tumor, 60.8% in middle ureteral tumor, 66.5% in lower ureteral tumor and 52.6% in tumor of distal end of ureter, respectively. Efficacy of the angiogenesis inhibitor O-(chloroacetyl-carbamoyl)fumagillol (AGM-1470) on osteosarcoma growth and lung metastasis in rats.

The efficacy of the anti-angiogenic agent, O-(chloroacetyl-carbamoyl)fumagillol (AGM-1470), against primary tumor growth and spontaneous lung metastasis was evaluated experimentally using a transplantable osteosarcoma line in rats previously established in our laboratory. Male Fischer 344 rats bearing the tumor with a high potential for metastasis received intermittent or continuous subcutaneous administrations of AGM-1470. Both treatment regimens resulted in significant inhibitions of spontaneous lung metastasis and primary tumor growth in a dose-dependent manner, with continuous administration of AGM-1470 exerting the most pronounced inhibitory effects on both parameters.

ITI (inter-alpha trypsin inhibitor) phenotyping by immunogold staining.

Polymorphism of Inter-alpha-Trypsin Inhibitor (ITI) was investigated in 685 unrelated Japanese individuals by isoelectric focusing followed by electroblotting and immunogold silver staining. The allele frequency estimates were ITI*1 = 0.455 +/- 0.013, ITI*2 = 0.516 +/- 0.013, ITI*3 = 0.028 +/- 0.0045 and ITI*4 = 0.001 +/- 0.001 and the phenotype frequencies fitted the Hardy-Weinberg equilibrium. The ITI system could be regarded as a potential genetic marker in paternity testing and anthropological studies.

The distribution of lymph node metastases in right-sided colon cancer.

We reviewed the surgical results of 57 patients with right-sided colon carcinoma (RCC) with special reference to lymph node metastasis. In 45 cured patients, 25 showed no lymph node metastasis (n0), 19 showed a low or moderate degree of lymph node metastasis (n1, n2), and only one showed a high degree of lymph node metastasis (n3). As for the remaining 12 cases, the reasons that operation did not lead to a cure were liver or lung metastasis (66%), peritoneal spread (17%), or extensive lymph node metastasis (17%). Among these patients, only one may have died from insufficient lymph node dissection (R2 < n3). The presence of lymph node metastasis was predicted by the serum carcinoembryonic antigen level (> 3.0 ng/ml) and histologic type (other than well differentiated). These results indicate that complete removal of the group 1 and 2 nodes (R2 dissection) is a feasible operation for RCC.

Polymorphism of factor H (beta 1H globulin) in Japanese: description of a new allele.

Polymorphism of factor H (HF) was investigated in 1060 unrelated Japanese individuals using isoelectric focusing and immunoblotting. Besides 6 different HF types a null type and an unusual type were observed. The family analysis suggested the hereditary occurrence of a new variant allele HF*C. The population data fitted the Hardy-Weinberg equilibrium, assuming that the null allele HF*QO occurs commonly. The allele frequencies were HF*A = 0.407 +/- 0.011, HF*B = 0.491 +/- 0.011, HF*A1 = 0.011 +/- 0.002 and HF*QO = 0.091 +/- 0.006. The HF polymorphism in Japanese was shown to be controlled by the above 4 common alleles.

GC subtyping in dental pulps.

By means of non-immobilized isoelectric focusing followed by immunoblotting, group-specific component (GC) subtypes were detected in dental pulp tissues. The amount of GC in dental pulp was quantitated using rocket immunoelectrophoresis and the minimum amount of the protein necessary for the typing of GC was estimated to be about 0.02 microgram in 10 microliters of the sample. The results of GC typing in dental pulp tissues were in agreement with the phenotypes obtained from serum samples of the same individuals. Subtyping by the present method was also possible in dental pulp tissues obtained from the teeth stored for 4 weeks at room temperature. The GC subtyping by non-immobilized isoelectric focusing is of practical use for medicolegal investigation of teeth.

Transferrin subtyping in dental pulps.

Serum transferrin (TF) subtypes were also found in dental pulps by isoelectric focusing and immunoblotting. The types observed in dental pulps completely agreed with those in serum samples from the same individuals. The allele frequencies in 105 samples were TF*C1 = 0.757 and TF*C2 = 0.243. Reliable subtyping was possible for 4 weeks following extraction of the teeth. The TF system can provide a useful genetic marker for the medicolegal individualization of teeth.

A clinicopathologic study of mucinous gastric carcinoma.

A clinicopathologic study of 42 mucinous gastric carcinomas (MGC) and 73 nonmucinous gastric carcinomas (NGC) was done. The tumor was defined as MGC when more than one half of tumor area had mucin pools. The 5-year survival rate for curatively treated patients was almost the same in MGC (58%) and NGC (56%), and clinicopathologic features, except for lymphatic permeation, showed no significant difference between MGC and NGC. Findings in MGC patients who died of a recurrence within 3 years included total gastrectomy, upper location, large size, infiltrative growth, extraserosal invasion, positive lymph node metastasis, more advanced stage, and a noncurative operation. There was no significant correlation between the degree of mucin content and other data, including the prognosis. Histologically, MGC were divided into well-differentiated and poorly differentiated types, according to the degree of glandular formation of the tumor cells. In patients with well-differentiated MGC, the age of onset was older, tumor growth was localized, and there were metastases to the liver. In patients with poorly differentiated MGC, the age of onset was younger, tumor growth was infiltrative, and there was peritoneal dissemination. These results show that the biologic behavior of MGC is similar to that of NGC and that the lesion basically is determined by the histologic subtype, not by the mucin content.

Polymorphism of coagulation factor XIII B subunit: further occurrence of FXIIIB*15 in Japanese and phenotyping in bloodstains.

The polymorphism of FXIIIB was investigated in 555 unrelated Japanese individuals using isoelectric focusing and immunoblotting. Five common phenotypes and a rare variant type FXIIIB 15-3 were observed. The allele frequencies were FXIIIB*1 = 0.3063, FXIIIB*2 = 0.0162, FXIIIB*3 = 0.6766 and FXIIIB*15 = 0.0009. Phenotyping was also possible from bloodstains stored at 37 degrees C for up to 4 months and from bloodstains stored at room temperature and at 4 degrees C for over 6 months. The FXIIIB system can provide a new powerful genetic marker for the medicolegal grouping of bloodstains.

Multiple colorectal neoplasms in a young adult with hypogammaglobulinemia. Report of a case.

We report on a 22-year-old man with congenital hypogammaglobulinemia who developed multiple colorectal neoplasms. An immunodeficiency had been diagnosed in the patient since two years of age, and this time many tumors of the sigmoid colon and rectum were detected by barium enema and fiberscopy. Abdominoperineal resection was performed, and the resected specimen revealed 29 polyps, including 9 adenocarcinomas and 20 adenomas. The carcinomas, measuring 0.8 to 11.0 cm in size, showed various depths of invasion, and the adenomas, measuring 0.2 to 1.5 cm in size, showed various degrees of epithelial atypia. DNA analysis demonstrated that the tumors were heterogeneous, showing different DNA index and ploidy patterns. The pathogenetic relation between malignancy and immunodeficiency is also reviewed.
