Long term follow-up of percutaneous transluminal renal angioplasty with special reference to aorto-arteritis.

Percutaneous transluminal renal angioplasty (PTRA) was attempted in 96 patients of renovascular hypertension (RVHT) admitted during the period 1986 to 1992. The patients' age ranged from 14-70 (mean: 38.7 +/- 18.8) years. There were 42 (43.8%) males and 54 (56.2%) females. The cause of renal artery stenosis (RAS) was aorto-arteritis in 44 (45.8%), atherosclerosis in 28 (29.2%) and fibromuscular dysplasia in 24 (25%). Bilateral RAS was found in 16 (16.7%). PTRA was angiographically successful in 92 (95.8%) patients. The mean pressure gradient decreased from 82.6 +/- 8.2 to 11.2 +/- 3.6 mm Hg in aorto-arteritis, 75.2 +/- 13.2 to 9.6 +/- 6.4 mm Hg in atherosclerosis and from 86.4 +/- 10.6 to 13.2 +/- 8.2 mm Hg in fibromuscular dysplasia respectively. The patients were followed up for 43.2 +/- 24.1 (range: 6-77) months. Remission or satisfactory lowering of blood pressure was achieved in 80 (86.9%) patients. Clinical success rate (remission or satisfactory lowering of BP) at the end of follow-up period was 75.3 percent. Randomly selected repeat angiography was done in 45 out of 92 (49.1%) patients at the end of 24 months of follow-up. Restenosis was detected in 12 (26.7%) patients and was commonest in the atherosclerotic group (42.8%), followed by fibromuscular dysplasia (14.3%) and least common in aorto-arteritis (11.8%). Repeat angioplasty was done successfully in 10 (83.3%) patients.(ABSTRACT TRUNCATED AT 250 WORDS)

Insulin release in type 2 diabetes mellitus.

Impaired insulin response is a characteristic feature of Type 2 diabetes. Overt diabetes develops when beta-cells fail to release enough insulin to compensate for decreased insulin sensitivity. However, a subgroup of normal weight patients demonstrates a pronounced beta-cell secretory defect and a normal insulin sensitivity. The molecular basis behind the impaired insulin response in Type 2 diabetes is not clear. Our studies in two animal models of this disease (GK rat and ob/ob mouse) suggest that an impaired glucose metabolism may be a primary defect in the stimulus-secretion coupling in the beta-cells in Type 2 diabetes. In the GK rat, three major alterations in the islet metabolism of glucose have been demonstrated: 1) increased glucose utilization but unchanged glucose oxidation; 2) increased glucose cycling and 3) decreased activity of the glycerol phosphate shuttle. In ob/ob animals we have found an increased rate of glucose cycling. These derangements might result in an incomplete closure of ATP-sensitive K(+)-channels with a decreased insulin response as a consequence.

Arias-Stella reaction associated with cervical pregnancy. Report of a case with a cytologic presentation.

We report the case of an Arias-Stella reaction of the cervix presenting in the cervical smear from a 35-year-old woman who complained of heavy menstrual periods. The smear was originally reported as inconclusive. Colposcopy revealed an abnormal transformation zone extending out of range. Cervical conization was performed. Histologic examination revealed an ectopic cervical pregnancy associated with a cervical Arias-Stella reaction. The cytologic manifestations of the Arias-Stella reaction continue to cause diagnostic confusion with benign lesions and adenocarcinoma. We believe this to be the first reported case of a cervical Arias-Stella reaction associated with a cervical pregnancy; it should alert cytologists to the diagnostic implications.

An unusual cause of dysphagia.

We report a rare case of a bronchogenic cyst in an adult presenting with dysphagia. The benign nature of the lesion was established preoperatively by a series of radiological investigations. This enabled surgical planning and prediction of prognosis. The presentation, radiological findings and its differential diagnosis are discussed.
