Use of values for calcium and protein in serum, and of a derived index obtained from a probability population sample.

We measured serum protein and calcium concentrations in 2340 individuals between 10 and 96 years of age from 900 families chosen by probability methods to give a representative population. These values were used to calculate an index, based on a regression analysis of serum protein on calcium, which was then treated as a new variable. Age-sex specific reference values and frequency distributions are presented for this index as well as for protein and calcium calculated by both parametric and nonparametric methods.

Do the right children have necropsies?. Analysis of selected determinants.

An analysis of the frequency of post-mortem examinations was conducted in the paediatric age group. Among the factors found to influence necropsy rates were (i) age at death, the frequency of necropsy decreasing with age; (ii) cause of death, necropsy frequency varying with major diagnostic groups; (iii) investigation conducted before death, necropsy frequency decreasing when the clinical picture was ill-defined; and (iv) place of death, necropsy being less frequent when death occurred outside hospital. These findings imply (a) that necropsy practices may be governed by habit and convenience as well as by desire to complete the clinical picture, and (b) that more equitable balances in such practices should be sought if there is to be improvement in the quality of mortality data.

Short-term assessment of 0.1% hydrocortisone 17-butyrate ('Locoid') in the treatment of skin disease.

An assessment was made of the local use of 0.1% hydrocortisone 17-butyrate in the treatment of 15 patients with a variety of skin conditions involving face and neck. Treatment was used on a short-term basis ranging from 3 to 22 days. There was marked clinical improvement of the lesions in 12 patients. The results of this short study are in accord with earlier published work and suggest that this new nonfluorinated steroid has promising potential.

Parasitic infection of man on Kar Kar Island, New Guinea.

The prevalence of protozoan and helminth parasites of the intestinal tract in a Papua New Guinea island with a population of 16,800 was assessed by surveys of a series of randomly selected villages. There were significant differences in the severity of infection with these parasites between the two linguistic groups inhabiting the eastern and western halves of the island. Blood examinations for malaria and microfilariae also differed. It is thought that these differences reflect obscure microenvironmental factors in what would appear to be an epidemiologically homogeneous population.

Acute abdominal complications of coeliac disease.

Two rare complications of coeliac disease are described in patients who presented as acute abdominal emergencies. One of the patients had both oesophageal and small intestinal obstruction produced by an ulcerative process involving these portions of the gastro-intestinal tract. The other, a patient with long standing dermatitis herpetiformis, perforated his small intestine at a site involved by both a lymphoma and partial villous atrophy.
