Case report: a surgical and prosthetic approach to combination syndrome presenting in a patient with craniofacial dysostosis (Crouzon syndrome).

A case of combined local surgical and prosthetic treatment in a patient with craniofacial dysostosis (Crouzon Syndrome) is described. The patient had atrophy and anterior fibrous replacement of the edentulous maxilla opposing natural mandibular anterior teeth and a lower removable partial denture. Initial prosthetic treatment alone was unsuccessful. The surgical management comprised augmentation of the maxilla with resorbable hydroxyapatite, in conjunction with a guided tissue regeneration technique and vestibuloplasty. This enabled conventional prosthetic management postoperatively to achieve an acceptable functional and aesthetic result.

Recurrent parotitis in children.

Recurrent parotitis in children (RPC), also known as juvenile recurrent parotitis, is a salivary gland inflammatory disease seen in the young. Intermittent episodes of parotid swellings, over a period of years, usually associated with a sialographic pattern of sialectasis, serve to differentiate RPC from other inflammatory diseases, such as mumps (epidemic parotitis). The etiology of RPC remains unknown, although such causative factors as heredity, immunology, infection, allergy and congenital duct malformation have all been considered. The manifestations of this unusual disease entity, illustrated by a case report, are the subject of this paper.

[Treatment of hypertension--the value of non-drug measures].

METHOD: To investigate the usefulness of non-pharmacological measures in the treatment of essential hypertension, 91 subjects comprising patients attending a physical-medical department and the inhabitants of an old people's home, were asked to complete a questionnaire. RESULTS: Almost all patients were receiving drug therapy (n = 89); in two-thirds treatment had been initiated when the diagnosis was first established. Although two-thirds were well informed about non-pharmacological measures (physical exercise, sodium restricted diet, reduced caloric intake, changes in dietary habits), more than one-half of the patients had neither been informed about, nor instructed in the use of, any of these alternatives by their general practitioners. Some 40% were aware of the benefit of relaxation techniques, but only one in six practiced them. CONCLUSIONS: Although this study revealed a high level of knowledge of non-pharmacological anti-hypertensive therapy, the care-providing doctors clearly attach more importance to prescribing drugs. In view of the potential value of alternative forms of treatment, greater consideration of this point by the physician would be desirable.

The effect of screw size and insertion technique on the stability of the mandibular sagittal split osteotomy.

The effect of screw size and insertion technique on the stability of sagittal split osteotomies was evaluated on 10 cadaver mandibles. The mandibles were osteotomized, and stabilized with either 2.0- or 2.7-mm stainless steel screws inserted as lag screws or position screws. All specimens were tested on an MTS servohydraulic testing machine (MTS Systems, Inc, Minneapolis, MN) to determine the strength and stability of the fixation. There were no statistically significant differences between the strengths achieved with the different screw sizes or fixation techniques. The primary variable that determined stability seemed to be the quality of the underlying bone.

Pneumoparotid: a case report.

Pneumoparotid is an unusual condition resulting from air being forced into the parotid ductal system. When it develops, it is a result of increased intraoral air pressure. Diagnosis is based on the etiologic factor, palpatory evidence of tissue emphysema, the escape of frothy saliva from the involved duct, and the sialographic finding of retained ductal air and ductal infection.

Polycystic parotid disease: a case report.

A case report of a 14-year-old girl, presumed to have PPD, is presented. Those manifestations of this patient's disease that can be added to the classic picture of PPD include a decreased salivary flow rate and a sialochemistry that reflects a mild chronic parotitis. In addition, cysts and sialoliths were demonstrated grossly rather than microscopically.

Masseteric hypertrophy.

The etiology and symptomatology of benign MH were reviewed. Three cases were presented in order to illustrate and emphasize the clinical appearance and variations in the causation of benign MH. The history and clinical examination are the keys to differentiating MH from parotid and dental pathology.
