[Effect of high-dose preoperative intracavitary radiotherapy on uterine size and early operability in patients with corpus carcinoma].

22 patients with corpus carcinoma were treated by intracavitary irradiation by the after-loading technique (Cathetron) during three sessions separated by one-week intervals. The weeks after radiotherapy, hysterectomy and bilateral salpingo-oophorectomy were performed. Changes in uterine size were observed during and after radiotherapy with the help of an uterine probe and by ultrasound-B-examination. The uterus was found to increase in size during radiotherapy. Measurement with the probe showed an average longitudinal increase of 1.2 cm, whereas ultrasonic examination revealed an increase in length by 1.7 cm and an anteroposterior distension of the fundus uteri by 0.9 cm. No technical difficulties were encountered during early surgery preceded by radiotherapy. Ehin, readily removable adhesions were found in six cases, and symptoms of irritation were seen in the rectal wall and in the peritoneum in one. No postoperative complications occurred in any of the patients.

Human placental lactogen levels during and after labor.

In order to estimate the human placental lactogen (HPL) level and its value as an indicator of fetoplacental function during labor, we determined HPL levels (N equals 225) before, during, and after labor in normal (N equals 16) and preeclamptic (N equals 14) subjects or in patients with benign intrahepatic cholestasis of pregnancy (N equals 5). During labor, greater decreases in this value were found in preeclamptic than in normal subjects and similarly in mothers with fetoplacental dysfunction than with normal fetoplacental function. The rupture of the membranes had no effect on the level of HPL, which was not related to parity, oxytocin infusion, time interval from rupture of the membranes to delivery, nor to relative placental weight. The half-life of HPL varied in the range of 20-23 minutes immediately after delivery and in the range of 30-39 minutes some time later. During labor, greater decreases in HPL level in cases of preeclampsia or fetoplacental dysfunction may be caused by relative uteroplacental ischemia during uterine contractions, but from this finding it is hard to expect any advantage of HPL as a monitor of fetoplacental function during labor.

Observations on patients two years after legal abortion,.

The patients who underwent legal abortions in 1971 were invited to a follow-up examination 2 years later. 25% of the 562 patients invited came to be examined. Anamnestic data were obtained on 60%. Early complications had occurred in 10% of the cases. During the 2-year period which had elapsed, the 338 patients had had 73 new pregnancies (22%), of which 60% had terminated in delivery. No spontaneous abortions in the second trimester had occurred in the new pregnancies. 15% of the patients had resorted to repeated legal abortion. 44% of the patients were using oral contraceptives or IUCD 2 years after the abortion. A gynecological examination gave rise to a suspicion of cervical insufficiency in 10% of the cases examined. Hysterosalpingography suggested tubal pathology in 18%. In laparoscopy no occurrences of bilateral tubal occlusion were found. The need of new follow-up examination later is obvious.
