Acute rupture of the lateral ligaments of the ankle joint, diagnosis and surgical treatment.

A useful method for the diagnosis of acute rupture of the lateral ligament of the ankle joint is presented. Twenty-three patients with ruptures of the lateral ligament of the ankle joint were treated surgically with suture of the lateral ligaments of the ankle. Accurate preoperative diagnosis with stress films and arthrography under local anesthesia of the ankle joint is imperative. No instances of infection or other complications were observed. All patients achieved a normal range of ankle function within three to six months. Radiograhs of the injured ankle in the forced inversion position following operation showed full stability of the ankle joint. In our opinion, all ankle injuries should be examined roentgenographically with forced inversion of the foot. Tilting of the talus of at least 10 degrees must be followed by arthrography of the ankle joint under local anesthesia. If there is leakage of contrast material along the peroneal tendon sheaths and around the joint cavity, the lateral ligament of the ankle must be sutured immediately in order to regain stability of the ankle joint.

Dynamic fixation of the avulsed clavicle.

A new operative approach is presented for treatment of the fractured distal end of the clavicle, which is associated with a disruption of the coracoclavicular ligaments. Three cases are presented in which open reduction and the clayicle held in the corrected position by transfer of the coracoid process with its attached muscles onto the clavicle was followed by complete recovery.

Camptodactyly, with muscular hypoplasia, skeletal dysplasia, and abnormal palmar creases: Tel Hashomer camptodactyly syndrome.

A syndrome characterized by camptodactyly, distinct facial features, multiple musculoskeletal defects, and unique dermatoglyphic changes is described in two sisters born of consanguineous parents. In 1972 this same constellation of findings was first reported in two sibs from a different ethnic origin. This heritable disorder of connective tissue termed the Tel Hashomer camptodactyly syndrome is thought to be transmitted as an autosomal recessive trait. The basic defect is unknown.

Dynamic repair of acromio-clavicular dislocation.

Twenty patients with complete acromio-clavicular dislocation treated between 1967 and 1973 were reviewed with a follow-up time of from 1-6 years, the average being 3 years. They were treated by transposition of coraco-brachialia, short head of biceps and a third of pectoralis minor from their origin in the coracoid process to the clavicle together with repair of the acromio-clavicular joint. The results were evaluated according to pain, motion, deformity and x-rays with 90 per cent excellent results, 5 per cent fiar and 5 per cent poor. Discussion of the other types of treatment is presented.
