Hydrocolpos, vaginal atresia, and urethrovaginal fistula in a neonate: abdomino-perineal-vaginal pull-through.

The combination of high maternal estrogenic stimulation of the secretory glands of the reproductive tract of the newborn female and vaginal obstruction results in an abdominal mass due to vaginal distention (hydrocolpos). Prior experience with two infants with hydrocolpos (LG) led us to perform abdomino-perineal-vaginal pull-through, as advocated by Ramenofsky and Raffensperger, in the newborn period. So far as we can ascertain this is only the second infant so reported in the literature.

Detection of anti-red blood cell antibodies by treatment of the cells with neuraminidase.

Neuraminidase treated RBC were used to detect incomplete unbound anti RBC antibodies in the plasma of patients with immune diseases. 80% (48 out of 60) of the examined patients gave positive results with this method while all the 50 normal controls were negative. The indirect antiglobulin test was negative in all the patients. In the direct antiglobulin test only two RBC samples, whether treated with neuraminidase or not, gave positive results. The present study indicates that the assay of plasma on neuraminidase treated RBC may serve as a sensitive method for the detection of the presence of anti RBC antibodies in various immunological diseases.

An automated radiology information system.

An automated radiology department information system, serving the diagnostic radiology and nuclear medicine divisions, has been developed. Radiology applications are part of a centralized data processing operation which incorporates clinical, administrative, research, and teaching subsets. The radiology subset includes remote on-line ordering/reporting, automated examination census and statistics, an on-line audit trail/workload control program, and financial reports. The software is linked to other hospital applications. This system improved report turn-around times, decreased clerical loads, and provided better financial management.

The relationship of a serum protein, C1t, to a common nonfibrillar constituent of amyloid (P component) as revealed by immunohistochemical studies.

C1t, a serum protein isolated by affinity chromatography on Sepharose bears a remarkable ultrastructural and physiocochemical resemblance to P component, a common constituent of amyloid. This study provides further evidence for their similarity by the demonstration of immunologic identity and by the presence of C1t in amyloid deposits of various types using immunoperoxidase and immunofluorescent techniques. In addition, subcomponents of C1, as well as C3, C4, C5, and properdin, were demonstrated to a limited extent. The possible role of C1t/P component in amyloidogenesis is discussed in the light of recent advances in our knowledge of the nature of amyloid substance.

Immunopathologic study of herpes gestationis in mother and infant.

In vivo bound C3 was demonstrated by immunofluorescence in lesions of both maternal and infant skin in an immunopathologic study of herpes gestationis. Immunoglobulins and other complement components of both classical and alternative pathways were not found. The serum was also negative for circulating antibody to basement membrane of skin. Maternal serum had a factor present that was able to deposit C3 from fresh normal serum onto the basement membrane zone of skin. Hemolytic complement titrations showed a decrease of CH50 levels in serum of both classical and alternative pathways in the mother and the classical pathway in the infant, with a decrease of C2 and C4 in both. To our knowledge, this is the first reportof a decrease in serum complements in herpes gestationis and indicates that complement may not be activated locally.

Intensive plasma exchange, complement dependent microcytotoxicity and renal transplant rejection.

Intensive plasma exchange (IPE) was used to treat 13 rejection episodes in eight renal transplant recipients with biopsy evidence of humoral rejection. Prior to IPE, each patients had several rejection episodes treated with high dose steroids. The IPE-treated rejections had not responded to conventional anti-rejection therapy and all patients appeared likely to lose their grafts. IPE reversed 7 of the 13 rejections (5 of 8 patients responded). Two of the 8 grafts continue to have adequate function 6 and 8 months after IPE. IPE temporarily reverses rejection but not increase long term graft survival.

Carcinoma of the colon in a child.

A case of mucinous adenocarcinoma of the colon in a 12-year-old girl is reported. There was no evidence of pre-existing familial polyposis or ulcerative colitis, conditions which predispose to colon carcinoma in childhood. A review of the literature revealed 103 previously reported cases. The salient features of the disease are discussed.

Clinical and laboratory findings in primary generalized and multiple-myeloma-related amyloidosis.

Thirty-four patients with primary generalized amyloidosis (PGA) and 14 with multiple-myeloma-related amyloidosis (MRA) were studied. The commonest clinical manifestations in PGA were nephrotic syndrome, hepatomegaly and congestive heart failure, and in MRA, low back pain, plasmacytoma and rheumatoid-arthritis-like syndrome. Eight patients with PGA had limited clinical expression of the disease, such as involvement of only kidneys, joints, parotid glands or gastrointestinal tract; in one patient amyloidosis was limited to lymph nodes. Low serum concentrations of total protein and albumin were common. M components were detected in the serum of 91% of patients with PGA and 92% of patients with MRA: 70% of the M components in PGA and 25% of those in MRA had lambda light chains. Bence Jones proteinemia was detected in 56% of the patients with PGA and in 77% of those with MRA. The serum concentration of immunoglobulins was decreased substantially in more than two thirds of the patients with PGA. Proteinuria (greater than 250 mg/24 h) was observed in 78% of patients with PGA and in 93% of patients with MRA. Bence Jones proteinuria was noted in 75 and 77% of patients, respectively. Plasmacytic infiltration of the bone marrow was found in 90% of the patients with PGA. The mean survival time of the patients with PGA was 28 months and of those with MRA, 29 months from the time of diagnosis.

Glomerulonephritis with mesangial deposits of IgA unassociated with systemic disease.

Typical features of IgA-associated nephritis were found in renal biopsies from 16 of 355 consecutive patients. Generalized segmental mesangial proliferation was noted in biopsies from most patients, and dense deposits were detected by electron microscopy in mesangial regions of approximately 50% of biopsies. Immunofluorescent studies showed IgA to be the predominant immunoglobulin in glomueruli; IgG was present in less than 50% of biopsies and IgM in only 12%. The serum IgA value was significantly increased (P les than 0.001) in 50% of patients and the mean IgA/IgG ratio was significantly increase (P less than 0.001) for the patient group as a whole, which suggests a selective increase in IgA. Mesangial deposits of C3 were present in 15 of 16 biopsies and properdin was noted in all biopsies tested; C4 was not demonstrated in any biopsy. This suggests activation of the alternative complement pathway. The results of this study support the concept that IgA-associated nephritis is a unique condition that in some patients gives rise to idiopathic recurrent hematuria. Although the prognosis is good in the majority of patients, the renal disease may progress.

Fat emboli syndrome.

A review of the medical records of 30 patients with major trauma was undertaken. One third had fat emboli syndrome characterized by hypoxemia, confusion, and petechiae. Fluid balance was similar for all patients in the study. Hematologic indices had no prognostic value in identifying patients who were to become symptomatic from fat embolization. The serum albumin concentrations were significantly lower in the symptomatic group. The free fatty acid binding capacity of albumin is discussed.

The clinician's view of diagnostic electron microscopy.

From the clinician's standpoint, it has become increasingly evident that use of the electron microscope should no longer be confined to research institutes, but should be applied as an adjunct to clinical diagnosis, a guide to therapy, and a means of elucidating pathogenetic mechanisms. The usefulness of electron microscopy is stressed in relation to hematology, nephrology, virology, gastroenterology, and the study of miscellaneous metabolic conditions such as the storage diseases.

IgD deposition in glomerulonephritis. An immunopathologic study.

Diffuse deposits of IgD were found in glomerular basement membranes in eight of 263 consecutive renal biopsies by the direct immunofluorescent technic. In five biopsies a granular staining pattern was found and in three a linear pattern was found. IgG and beta1C were present in glomeruli of all eight patients, and IgM in four. Two patients had elevated titers of antithyroglobulin antibodies in the serum, and one patient had anti-glomerular basement membrane antibodies of the IgG class in both serum and renal eluate. This study suggests that IgD may play a role in immunologically mediated glomerular disease. (Key words: IgD; Glomerular deposits; Nephritis.)
