[Penetrating keratoplasty in congenital corneal opacities].

The clinical course and histopathological findings in 17 cases of congenital corneal opacity treated by penetrating keratoplasty are described. On the basis of the histopathological findings 9 of the cases of corneal opacity were diagnosed as Peter's anomaly, 5 cases as sclerocornea or mesenchymal dysgenesis, 2 as choristoma, and 1 case as leukoma. Only 2 of the corneal grafts performed in these infants remained clear. The underlying disease in the 2 cases where surgery was successful was Peter's anomaly, with a clear corneal periphery. From the analysis of the corneal buttons the authors deduce that graft failure in most such cases is due to severe structural changes of the host cornea. Three findings represented consistently unfavorable signs: (1) absence of Bowman's layer; (2) absence of Descemet's membrane and (3) corneal vascularization at the cut edge of the button. Immunoreaction or infection of the graft are rare causes of graft failure. Despite the extremely poor prognosis of bilateral congenital corneal opacities, penetrating keratoplasty should be attempted. The histopathological findings may be used as prognostic criteria for graft survival or the extent of anterior segment dysgenesis.

[Proliferative vitreoretinal reaction].

Epiretinal and vitreal membrane formation may be considered as a uniform reaction to various etiological factors. As a comprehensive term for the various stages of this intraocular tissue proliferation the term "proliferative vitreoretinal reaction" is proposed. Derived from a clinical and ultrastructural analysis of such membrane formations 3 stages may be differentiated. From findings it is induced that the clinically so important tractional phenomena have their morphologic correlate primarily in cellular contraction.

Vitreous morphology after carbon dioxide laser irradiation.

The immediate vitreous changes produced by a continuous-wave carbon-dioxide laser beam in freshly enucleated bovine and pig eyes were studied by gross examination with a dissecting microscope, and histopathologically by light microscopy and transmission electron microscopy. The lesions produced by the laser beam on the vitreous in an air tissue interface technique were remarkable for their ring-shaped vitreous condensations. The diameter and density of these condensations increased with irradiance. With the same irradiation parameters the lesions at the anterior hyaloid membranes showed greater size and density than those in the central area of the vitreous. These vitreous condensations were demonstrated to consist of heat-denatured collagen fibrils. For light microscopic evaluation of the vitreous a modified method of tissue processing according to Szent Györgi has proven optimal in reducing vitreous shrinkage from dehydration. This method of fixation and tissue processing appeared to be useful in evaluating lesions of the vitreous and their relationship to their surroundings.

[Argon laser trabeculoplasty. Initial results of a prospective study].

This paper presents the results of a prospective study on argon laser trabeculoplasty with clinical application in 166 cases up to now. So far, patients have been followed up for between 2 and 12 months. Low-intensity coagulation was performed over 360 degrees of the trabecular meshwork. The coagulation site within the trabecular meshwork was chosen according to the outflow facility as measured by tonography. In eyes with high outflow facility (C greater than 0.08) the unpigmented, so-called nonfunctioning trabecular meshwork was coagulated. Where the outflow facility was lower (C less than 0.03), a pigmented area of the trabecular meshwork was aimed at. Where the facility was intermediate (0.08 greater than C greater than 0.03) the border area between pigmented and nonpigmented meshwork was coagulated. Three groups of patients were treated: (1) older patients with insufficient compliance; (2) patients whose intraocular pressure (IOP) was poorly controlled by drugs and was under 35 mm Hg; (3) patients suffering from glaucoma in aphakia or status post trabeculectomy and poor control of IOP. Applying these criteria , IOP was controlled in 26.8% without additional drugs and in 68% in combination with local medication after 6 weeks. Very few complications were seen (8 cases), and they did not lead to deterioration or loss of vision. In the light of theoretical considerations and initial histopathologic findings in human eyes, the authors assume that low-intensity coagulation causes a denaturation and shrinkage of the trabecular collagen, leading to stretching of the surrounding trabecular structures.(ABSTRACT TRUNCATED AT 250 WORDS)

[Vitrectomy in endophthalmitis].

The authors report on 14 cases of fulminant and delayed forms of endophthalmitis, in which a diagnostic and therapeutic vitrectomy, were performed. By diagnostic processing of the excised vitreous the chances of obtaining positive findings were increased, especially with a combination of bacteriological and histopathological methods. As advantages of a diagnostic and therapeutic vitrectomy versus purely antibiotic treatment in endophthalmitis the authors discuss the following factors: (1) improved diagnostic possibilities in combination with vitrectomy; (2) a reduction in the quantities of infective organisms and toxins; and (3) improved penetration of antibiotic into and out of the vitreous body cavity.

[Oncocytoma of the ocular adnexa].

The authors report on the clinical and pathological findings in 8 oncocytic lesions of the ocular adnexa. These tumors may involve the caruncle, the conjunctiva, the lacrimal gland and the lacrimal sac. They are often clinically mistaken for haemangiomas, nevi or cysts. In the present authors series the tumor involved the caruncle in 4 cases, the lacrimal gland in 3 and the conjunctiva in one. The clinical and histopathological findings are presented. The occurrence of a malignant oncocytoma (oxyphilic adenocarcinoma) in a 58-year-old man, an oncocytoma (oxyphilic adenoma) in 18-month-old girl and a cystic oncocytoma in a 76-year-old woman are particularly remarkable findings. They are compared, on the basis of a review of the literature, with 50 previously reported oncocytic lesions of the ocular adnexa, of which only 3 involved the lacrimal gland.

Blindness from cataract formation in leprosy.

Of 744 leprous patients, 61 (8.2%) had cataract-induced blindness; 46 patients (6.2%) were unilaterally blind and 15 (2.0%) were bilaterally blind. The mean age of patients with cataracts was 63.4 years in the tuberculoid-type leprosy and 56.4 years in the lepromatous type. The appearance of cataracts in lepromatous patients at an earlier age than in tuberculoid patients is significant (p less than 0.005). Of 41 cataractous eyes with posterior iris synechiae, 42.1% were observed in patients with tuberculoid-type leprosy and 59.1% in patients with the lepromatous type. Histopathological examination of 24 lenses showed that irregularities of the lens epithelium were present in 79.2%. Posterior iris synechiae were seen in 66.7% and fibrous pseudometaplasia in 41.7%. Posterior migration of the lens epithelium in the posterior subcapsular area was observed in 54.2%. In view of these clinical and histopathological findings, we discuss the probability that most cataracts in leprous patients are complicated in nature.

[Clinical and pathological aspects of epiretinal membrane formation].

In the light of an analysis of epiretinal membrane formations, both clinical and histopathological, a stage classification of these membranes is proposed. Three stages are distinguished, characterized by their clinical picture and their morphology. Simple epiretinal membranes, the so-called cellophane macula, may be distinguished from more complex membranes with and without major traction on the retina. Whereas, histopathologically, the simple epiretinal membranes contain one cell system, the more complex membranes are composed of two or three cell types. Membranes with major traction are remarkable for the presence of contractile intracytoplasmic elements. On the basis of the authors' findings epiretinal membrane formation seems to be a uniform reaction caused by several etiologic factors; therefore, the term proliferative vitreoretinal reaction is proposed for epiretinal membrane formations.
