[Second-trimester abortions induced by dinoprost].

The authors evaluated the usefulness of prostaglandin F2 alpha in inducing second trimester abortions in two consecutive groups of patients. Progressive and fractionated intracervical instillation of 10 ml of Tylose gel containing 10 mg of Prostine F2 alpha is the least traumatic method of uterine evacuation in the majority of cases (18 out of 24 cases). Advantages of this method consist of the ease with which it can be performed, that it may be repeated on subsequent days, and that there have been no serious complications. In case of failure, the authors recommend using an intraamnionic injection (usually performed only once) of 40 mg of Prostine F2 alpha diluted in 20 ml of normal saline solution. This more involved method requires an experienced physician and ultrasonography, but is very effective (31 cases out of 3). Exceptionally, this can be repeated after a several day interval if the initial injection fails (2 out of 3). These methods avoid recourse to surgery (except in the case of uterine scarring) in the induction of second trimester abortions.

Amplification and transmission of the EEG.

In long-term monitoring of the EEG in epilepsy, the recording conditions are usually such that a technique must be used which allows EEG recording from patients who are engaged in behavioral activities. The technique employed basically consists of the use of a miniaturized EEG amplifier at the site of the patient and the transmission of the amplified EEG to the recording apparatus. As regards the quality of the recording, the use of miniaturized amplifiers involves various problems, particularly those related to the choice of the fixed amplifier characteristics. Depending on the recording conditions, the transmission is commonly carried out whether by cable telemetry or by radio telemetry. Various techniques for multi-channel data transmission can be used for both telemetry methods. The choice of any particular transmission technique will mainly depend on the characteristics of the EEG data transmitted and on specific requirements on the quality of the recording.

[Mycoplasma infection: a disease of reproduction? Apropos of 271 cases].

We investigated by culture the presence of mycoplasmal infection in all reproductive abnormalities. 271 charts were studied, comprising problems of sterility with abnormal sperm, cystitis, urethritis, spontaneous abortions, fetal death in utero, premature deliveries and small for gestational age fetuses. Antimycoplasmal therapy lead to recovery in cases where this infection was the only cause, notably in sterility. Is Mycoplasma a risk factor or does it simply represent current infection?

[Pregnancy in a patient with an isotopic pacemaker placed for surgical atrioventricular block following treatment of tetralogy of Fallot. Apropos of a case. Review of the literature].

We report on a patient with an isotopic pacemaker who had a subsequent normal pregnancy. This device was implanted because of a surgically-induced atrioventricular block which occurred during an operation for tetralogy of Fallot. The pregnancy and postpartum course were normal and without complications.

[9 years of systematic cytologic examinations of expressed galactophorous discharge. Detection of cancerous and precancerous conditions].

Nine years of experience performing cytologic examinations on expressed breast discharge has enabled the identification of precancerous and cancerous conditions of the breast. Three thousand women were monitored. The only abnormal findings were cytologic (clinical and radiological examinations were normal). One case of atypical hyperplasia, 3 cases of cancer in situ, and 2 invasive cancers were identified upon histologic examination. Three women were lost to follow-up and subsequently developed invasive cancer.

[The "white" cervix: a new anatomo-clinical entity?].

The authors describe a very particular histologic pattern of the cervical epithelium. Many types of histologic lesions are involved and are almost always found in association one with another. This particular histologic pattern is characterized principally by well-differentiated epithelium, immature, with early necrosis of the superficial layers, associated with metaplastic endocervicitis. In addition to this particular association, there is a remarkable finding: this histologic pattern was found consistently in fifteen cases selected uniquely from their appearance at colposcopy. Could this be a new anatomic-histologic entity?

[Improvement in feto-neonatal prognosis in twin pregnancies. Comparison of 2 series of 158 pregnancies before and after establishing a therapeutic protocol].

We undertook certain measures in twin pregnancies: early screening during the second month of gestation with routine ultrasonography, cerclage of the cervix, cessation of work, with rest, hospitalization at 7 3/4 month's gestation for daily cardiotocographic monitoring, identical indications for cesarean section as for a single fetus. The outcome was clearly improved in 158 pregnancies when compared to an equal number of twin pregnancies from an earlier period.

Long-term supervised domiciliary EEG monitoring in epileptic patients employing radio telemetry and telephone telemetry. I. Telephone telemetry system.

A PCM system provides digital transmission of 8 EEG channels over conventional voice graded telephone lines. Transmission of the EEG from the patient's residence to the clinical centre enables the use of existing monitoring facilities for carrying out continuously supervised long-term domiciliary EEG monitoring. Secondary channels simultaneously enable exchange of information for quality and performance control. The ease of use and the flexibility of the public telephone system enable recording from almost any place desired.

Long-term supervised domiciliary EEG monitoring in epileptic patients employing radio telemetry and telephone telemetry. II. Radio telemetry system.

A miniaturized 8-channel EEG amplifier- submodulator unit is used in combination with a standard radio transmitter/receiver system for long-term ambulatory EEG recording at the patient's residence. The EEG frequency response (0.16-30 Hz) and amplitude range (1:1000) of the system are in accordance with the object in view, the recording of seizures. The submodulator characteristics conform to regulations for radio telemetry.

Hippocampal EEG and motor activity in the cat: the role of eye movements and body acceleration.

In cat the relation between various behaviours and the spectral properties of the hippocampal EEG was investigated. Both EEG and behaviour were quantified and results were evaluated statistically. Significant relationships were found between the properties of the hippocampal EEG and motor acts (walking, sitting, eating, stepping and eye movements). These results were compared with those obtained in dog under similar experimental circumstances. Species differences were found particularly regarding the fact that in the cat a dissociation between frequency and amplitude parameters was obtained for some behaviours; this may explain why appreciable differences in the visual interpretation of EEG records of different species are often reported. A simple model of the modulation of hippocampal EEG by brainstem inputs is presented. Particular attention is paid to species differences regarding the anatomy and physiology of the pathways involved in this modulation. It is concluded that in cat a strong relation exists between the modulation of spectral properties of hippocampal EEG and vestibular inputs and/or eye movements. The effects of body acceleration on hippocampal EEG are put in evidence and related to theories of hippocampal function.

Improved telemetric EEG monitoring in epileptic patients.

A 20-channel EEG telemetry system provides EEG monitoring with standard EEG recording performance for use in epileptic patients. The EEG activities of 20 electrodes are separately amplified and transmitted employing PCM cable telemetry. A standard EEG apparatus is used, allowing montages to be freely selected and changed during recording.

[Homogeneous triploidy in 2 premature infants (69 XXY)].

Two new cases of 69 XXY triploidy in live-born neonates are reported. As in 40 others cases of literature observed after 24 weeks of gestation, this chromosome abnormality was lethal. The clinical features are: a large posterior fontanelle, low set ears, syndactylies of hands and feet, and genital abnormalities in the presence of a 69 XXY karyotype. The first patient present a macrocytosis of red blood cells. Macrocytosis, large polymorphonuclear leukocytes and platelets can evoke the diagnosis of triploidy in a malformed newborn.

Telemetric EEG and video monitoring in epilepsy.

We report a clinical evaluation of EEG and video monitoring. In 181 consecutive records, the most common clinical indications were differentiation of epileptic and nonepileptic events (99), seizure recordings for locating a possible focus (23), seizure frequency determination (32), and investigation of possible trigger factors (19). Overall, useful information was obtained in 72% and the clinical question was answered in 67% of records. Prolonged EEG and video monitoring is therefore an important diagnostic aid in patients with proven or suspected epilepsy.

The spectral properties of hippocampal EEG related to behaviour in man.

In an epileptic patient with depth electrodes placed in the hippocampal formation we observed the following: (1) There existed a significant peak in the theta band of the hippocampal EEG of this subject under a number of behavioural circumstances. (2) Four different behaviours were performed at two degrees of intensity. Whenever the values of a certain spectral parameter associated with a low or a high intensity behaviour showed a significant difference, the higher value corresponded with the more intense version of the behaviour. (3) The frequency and rhythmicity of the hippocampal theta component during writing was consistently higher than during sitting or walking. The amplitude was smaller. (4) In a word association task the amplitude, frequency and rhythmicity showed a significant rise during the period of silence immediately following the question and preceding the answer.

Hippocampal EEG and behaviour in dog. I. Hippocampal EEG correlates of gross motor behaviour.

It was shown that rewarding spectral shifts (i.e. increase in amplitude or peak frequency of the hippocampal EEG) causes a solitary dog to show increased motor behaviour. Rewarded spectral shifts concurred with a variety of behavioural transitions. It was found that statistically significant modulations occur in the spectral properties of the hippocampal EEG correlated with: (1) the transition from walking to standing; (2) the transition from standing while eating to walking away from the food dish; (3) the increase in speed of a walking dog, caused by rewarding the animal; and with (4) each head movement in a learned series of head movements. Thus behavioural transitions to a more active state are accompanied by an increase of amplitude, frequency and rhythmicity in the theta band of the hippocampal EEG; behavioural transitions to a less active state show the inverse relationship with the hippocampal EEG. A close relationship between modulations of the dog's hippocampal EEG activity and elementary motor acts is stressed. The hypothesis is put forward that the spectral properties of the hippocampal EEG reflect the degree to which a number of motor and sensory structures in the limbic midbrain and brain stem are active.

Hippocampal EEG and behaviour in dog. II. Hippocampal EEG correlates with elementary motor acts.

A positive correlation has been shown between the speed of forced stepping on a conveyor belt and the amplitude and frequency of the concomitant hippocampal EEG. Significant modulation in the spectral properties of the dog's hippocampal EEG has been found in relation to 3 elementary motor acts: stepping, respiratory movements and a reflex movement in reaction to linear acceleration of the animal. The findings support the idea that within a certain experimental situation the hippocampal EEG reflects the intensity of motor behaviour in a non-specific but predictable way. However, some experimental results indicate that sensory inputs may modulate the hippocampal EEG as well.
