Is retrieval of bone material from multiorgan donors effective enough to cover demand for biostatic bone tissue grafts in Poland?

In Poland there is growing demand for biostatic allogeneic bone transplantation mostly for traumatologic operations and orthopedic reconstructions. The bone material is primarily harvested during postmortem examinations in forensic and pathology laboratories. Nevertheless, the collected amounts are not sufficient, so that material needs to be acquired from alternative sources, such as multiorgan donors. Between 1998 and 2003, 2331 potential donors were registered by the Transplantation Coordinating Center in Warsaw, which was adjusted to 1794 donors who would have been accepted as donors of the bone tissue. Unfortunately, due to denials from family members and public prosecutors, the sample was only 1416 donors, which would cover about 40% of the clinical orthopedic demand in Poland.

Factors determining the contamination of bone tissue procured from cadaveric and multiorgan donors.

There is an increasing demand for bone allografts that are widely used in orthopedic reconstructive procedures. The bone tissue may be harvested from two sources: cadavers and multiorgan donors. Providing safe and valuable bone allografts is of paramount importance. Contamination of allografts during bone retrieval seems to be one of the most important problems since pathogenic microorganisms might be responsible for postoperative infections and complications in the healing process. The purpose of our study was to identify all factors contributing to bacteriological contamination of harvested bones. Therefore, we have considered factors such as harvesting environment, explantation techniques, storage and preparation of allografts, number of preceding procurements from the same donor, procurement duration, and time interval between death and tissue procurement. The microbiological evaluation of allografts has been performed by taking cultures from all collected bones. Our study revealed significantly greater contamination rates of bone allografts harvested from morgue than from multiorgan donors. According to this observation, we suggest that orthopedic surgeons should pay particular attention to obtain more bones of the highest quality, personally participating in multiorgan procurements.

Analysis of neurological complications in children transplanted due to fulminant liver failure.

Treatment of patients with fulminant liver failure is a challenge of contemporary medicine. Liver transplantation, in this group, is presently the only reasonable alternative, but in many patients the disastrous condition of the patient results in serious life-threatening complications, including neurological sequelae, which may influence the quality of life after transplantation, and in some cases even cause death. From 1990 to 2004, we performed 241 liver transplantations in children, including 20 transplanted due to fulminant liver failure (8.2%). Serious neurological complications followed liver transplantation in five cases (20%), three of which were fatal. The analysis revealed that the duration of pretransplant coma (grade III or IV) strongly correlated with the incidence of neurological complications (P < .05). Also a suboptimal quality of the donor liver and poor early graft function may contribute to these posttransplant complications.

The surgical approach to hyperinsulinism and hypoglycemia in children.

Authors present 19 children aged from 2 weeks to 16 y with severe hypoglycemia. In 3 oldest children a single nesidioma was found. In other 16 children mainly infants nesidioblastosis has been recognised, and in those cases the subtotal pancreatectomy was performed. In 4 infants the additional resection of the pancreas was done because after the primary procedure hypoglycemia persisted. All patients survived and were euglycemic. Severe hypoglycemia leads to brain damage and therefore should be treated as an urgent state.

[Results of the treatment of neurosyphilis and seroresistant latent syphilis by intravenous administration of potassium salt of penicillin G].

Potassium penicillin G was administered intravenously 4 times daily in doses of 6 million u for 10 days to 49 patients, 36 with central nervous system syphilis and 13 with latent seroresistant syphilis. Forty six patients were followed up after treatment. In 16 patients with central nervous system syphilis treated by this method another cerebrospinal fluid examination was done. Cerebrospinal fluid changes regressed completely in 13 patients, in three cases the improvement was incomplete. The titres of VDRL and FTA tests decreased in 23 patients with central nervous system syphilis and in 11 with latent seroresistant syphilis. In eight patients treated by this method seroresistance persisted and in four cases the titre fluctuated. The results of the study demonstrate the effectiveness and advantages of the treatment of central nervous system syphilis and latent seroresistant syphilis with crystalline penicillin administered intravenously.
