Prognostic factors analysis in a therapeutic trail of acute lymphoblastic leukaemia (ALL).

It is common practice in therapeutic trials in Acute Lymphoblastic Leukaemia (ALL) to analyse the impact of prognostic factors on clinical outcome. In this study, 30 newly diagnosed ALL patients were enrolled and the median age was 14.5 years. 22 patients were male and 8 patients were female. Prognostic factors analysed include age, sex, lymphomatous presentations (bulky peripheral lymphadenopathy, mediastinal mass and enlargement of spleen and liver), initial leucocyte count, blast cell morphology and bone marrow response to chemotherapy. Remission induction and maintenance therapy with conventional combination chemotherapy and CNS prophylaxis with intra thecal methotrexate and radiotherapy were instituted to all patients for long term event free survival. Results of induction therapy and overall outcomes of treatment were observed. The impacts of analyzed prognostic factors on clinical outcomes were assessed. 3 patients (10%) failed to go into remission after 12 weeks of therapy. 27 patients (90%) obtained remission. Among 27 patients those obtained remission, 17 patients continued event (failure to remit, relapse and death) free survival, 7 patients suffered relapse of their leukaemia and 3 patients died during maintenance therapy. Events were observed more among patients age group > or = 20 yrs (84%), initial WBC count > or = 50 x 10(9)/L (58%) and FABL2 morphology (80%).

Expression of human inositol monophosphatase suppresses galactose toxicity in Saccharomyces cerevisiae: possible implications in galactosemia.

A suppressor of galactose toxicity in a gal7 yeast strain (lacking galactose 1-phosphate uridyl transferase) has been isolated from a HeLa cell cDNA library. Analysis of the plasmid clone indicated that the insert has an ORF identical to that of hIMPase (human myo-inositol monophosphatase). The ability of hIMPase to suppress galactose toxicity is sensitive to the presence of Li(+) in the medium. A gal7 yeast strain harboring a plasmid containing cloned hIMPase grows on galactose as a sole carbon source. hIMPase mediated galactose metabolism is dependent on the functionality of GAL1 as well as GAL10 encoded galactokinase and epimerase respectively. These results predicted that the UDP-glucose/galactose pyrophosphorylase mediated pathway may be responsible for the relief of galactose toxicity. Experiments conducted to test this prediction revealed that expression of UGP1 encoded UDP-glucose pyrophosphorylase can indeed overcome the relief of galactose toxicity. Moreover, expression of UGP1 allows a gal7 strain to grow on galactose as a sole carbon source. Unlike the hIMPase mediated relief of galactose toxicity, UGP1 mediated relief of galactose toxicity is lithium insensitive. Based on our results and on the basis of available information on galactose toxicity, we suggest an alternative explanation for the molecular mechanism of galactose toxicity.

The spinal manifestations of Stickler's syndrome.

STUDY DESIGN: A review of current knowledge, clinical publications, and recent concepts of the causes of Stickler's syndrome was correlated with a clinical review of the condition at the Children's Hospital of Eastern Ontario, Canada. OBJECTIVES: To acquaint orthopedic spine surgeons with the natural history, associated anomalies, and high incidence of spinal deformity and scoliosis in children with Stickler's syndrome. SUMMARY OF BACKGROUND DATA: Stickler's syndrome is a hereditary, progressive arthro-ophthalmopathy with an autosomal dominant inheritance pattern. The estimated incidence is 1 in 10,000 people, which is slightly more common than Marfan syndrome. METHODS: The experience with Stickler's syndrome was reviewed in seven children, 2-15 years of age, with particular attention to the spinal abnormalities secondary to the connective tissue dysplasia. RESULTS: Six of the children had kyphosis or scoliosis, and four had wedging or flattening of the vertebrae or platyspondylia. In general, the spinal changes became more prominent in the older children with Stickler's syndrome, with the spinal vertebrae affected by the generalized epiphyseal dysplasia. The treatment of scoliosis and kyphosis is no different in children with Stickler's syndrome. The most difficult aspect is in diagnosing the condition. CONCLUSIONS: The importance of recognizing the syndrome is to allow for the investigation and treatment of the many other associated connective tissue disorders associated with Stickler's syndrome, such as the high incidence of retinal detachment, mitral valve prolapse, and mandibular hypoplasia that may result in problems with anesthesia should the spine require surgical stabilization.

Acquired pure red cell aplasia--a case report.

A married female patient of 36 years with chronic anaemia, because of pure erythroid aplasia with a haemolytic component and hypothyroidism due to antithyroid auto-antibodies, was subsequently discovered as a case of systemic lupus erythematosus (SLE). She was treated with corticosteroid and immunosuppressive therapy and her anaemia was corrected. The response of erythroid aplasia to corticosteroid and other immunosuppressive agents suggests that immunological factors play a role in erythroid aplasia in SLE. The occurrence of red cell aplasia in association with a variety of immune phenomenon supports the concept that in SLE, erythroid aplasia may be of immune aetiology.

Is asthma documentation improved by computer-facilitated data entry?

The documentation of acute asthma in written medical records was compared with data entered into a Computer-Assisted Triage System (CATS) in 104 children who presented to the emergency department and subsequently admitted to the Royal Alexandra Hospital for Children, Sydney. A total of 65 items in 5 categories were analysed and satisfactory documentation was defined as the recording of a specific item in more than 80% of records (written or electronic). Satisfactory documentation was observed for all 6 items in visit details and 9 out of 10 items in triage details for both recording systems. Nursing observations were better documented in the medical record than in CATS (87 vs 25%; kappa = 0.63). Documentation of medical details was also worse in CATS (75 vs 25%; kappa = 0.24) and the documentation of asthma severity was poor in both systems (31 vs 0%; kappa = 0.31). Attempts to improve asthma documentation through the development of a computerized medical record have highlighted further barriers to documentation.

Relative abundance and species composition of some dung beetles (Coleoptera: Scarabaeinae) in Bangladesh.

1. The occurrence and community structure of dung beetle species (Coleoptera: Scarabaeinae) in the faecal deposits of man and some domestic vertebrates varied according to dung type, seasonal conditions and soil type. 2. Of the twenty-four species belonging to six genera, Onthophagus was the prevalent genus and O. ramosellus the most abundant species. 3. Cattle dung was the dominant food type.

Distribution of peanut agglutinin binding sites in rat lymphatic organs.

Distribution of peanut agglutinin binding sites was studied histologically with horseradish peroxidase labelled and fluorescein isothiocyanate labelled peanut agglutinin in terms of cell differentiation in rat lymphatic organs, (thymus, spleen, lymph nodes). In this study, alcohol-fixed paraffin-embedded tissue sections were used and proved to be useful for the histochemical study with peanut agglutinin. In the germinal center of the lymph node, cells were weakly positive for peanut agglutinin binding sites but not in the mantle zone of the lymph follicle. In the thymus, the cortical thymocytes were weakly positive for peanut agglutinin binding sites but not in the medulla. In the spleen, some cells on the periphery of the white pulp were weakly positive for PNA binding sites but cells around the central artery were not positive. Large cells with granular cytoplasma around the sinus of the spleen and lymph node, thought to be fixed macrophages, were strongly positive for PNA binding sites.

Cancer morbidity.

In the present study, 1736 cases of cancer has been analysed by major sites, sub-sites and mortality trend. The most common cancerous lesion in the male was in the lungs and in the female in the uterine cervix. The second common lesion in male and female were larynx and breast respectively. In children, retinoblastoma was the commonest type of cancer.
