[Synovial cyst of the hip. 2 cases presenting with vascular symptomatology].

Two cases of synovial cysts of the hip are reported beside any rheumatoid context. One presented as a pseudoaneurysmal inguinal mass, the other one as a venous compression of the lower limb, both of them were operative discovery. Through a review of the literature, pathogenic, diagnosis and therapeutic aspects of this rare disease are developed.

[Iatrogenic carotid-cavernous fistulas, complications of carotid thrombectomy].

The authors report four cases of carotid cavernous fistula at the C5 segment of the internal carotid artery following embolectomy by a 3F Fogarty Catheter. All patients were operated on a carotid bifurcation stenosis (pre-occlusive) and a thrombectomy with a 3F Fogarty Catheter was carried out in the same time in 3 patients and during re-operation in 1 patient who had post operative thrombosis. The carotid-cavernous fistula occurred the day after surgery in 2 cases, 1 week after in 1 case, and 2 months later in 1 case. Spontaneous recovery occurred in two patients (1 occlusion--1 re-stenosis), 1 patient died from controlateral carotid occlusion, and 1 patient was lost for investigation. There were no relationship between the number of "repeated passages" of the Fogarty Catheter and the interval time where the fistula occurred. However the location of fistula was constant. Overtension on the internal wall of the artery by the balloon could be advocated rather than a collateral torn of the intra cavernous internal carotid. Endoluminal treatment of various vascular lesion is increasingly used and iatrogenic problems as reported here should be kept in mind. Age and atheromatous lesion of carotid siphon appear to be the main risk factors.

Evolution of acetic Acid bacteria during fermentation and storage of wine.

Acetic acid bacteria were present at all stages of wine making, from the mature grape through vinification to conservation. A succession of Gluconobacter oxydans, Acetobacter pasteurianus, and Acetobacter aceti during the course of these stages was noted. Low levels of A. aceti remained in the wine; they exhibited rapid proliferation on short exposure of the wine to air and caused significant increases in the concentration of acetic acid. Higher temperature of wine storage and higher wine pH favored the development and metabolism of these species.

[Iliac-femoral artery involvement in relapsing polychondritis].

A patient presenting a relapsing polychondritis with ruptured iliac arterial aneurysm and lower limbs arteritis is described. The analysis of the results compared with the literature's cases confirm the no response for surgical and medical treatments.

[Leiomyosarcoma of the femoral vein. Apropos of a case].

A 51 years old woman developed an edema of the right lower limb. Phlebographic examination revealed an occlusion of the femoral vein. This was a tumor at operation. A leiomyosarcoma was diagnosed by microscopic study. The authors emphasize the difficulty of diagnosis, value of phlebography and discuss the treatment.

[Aorto-bi-iliac resection, of the 3rd part of the duodenum and the left colon, for recurrence of colonic cancer].

An aorto-bi-iliac resection was performed in a patient with recurrent cancer of the left colon. Implantation of a Dacron vascular prosthesis was combined with left iliac colostomy and resection of 3rd part of duodenum. Based on this case and a review of the literature, the possibility and value of resection of an invaded vascular axis is emphasized. The revascularization procedure, either by a prosthesis in situ or associated extra-anatomical by-pass, is discussed.

[Segment I of the liver or spigelian lobe. Anatomic study and surgical value].

Spigel's lobe, or segment I of the liver, currently represents the "smallest amount of hepatic parenchyma capable of regeneration". It is an autonomous lobe by virtue of its site and vasculo-biliary distribution, a true "accessory" liver connected to the "main" liver. Lesions of Spigel's lobe are rare, which is explained by the anatomy of the portal bifurcation. Hence the value of making every effort to preserve this segment during hepatectomy because of the possibilities of additional hepatic vein drainage. Segment I surgery is difficult and falls within the context of extended hepatectomies. Can Spigel's lobe be considered to be a "safety liver"? The possibility of subtotal hepatectomy, retaining only segment I, is being studied in the animal. It may be possible in man in the case of extensive hepatic lesions where only segment I is unaffected, as soon as effective artificial liver support is developed.
