The proximal promoter of the mouse loricrin gene contains a functional AP-1 element and directs keratinocyte-specific but not differentiation-specific expression.

Loricrin gene expression is limited to terminally differentiating keratinocytes of stratified squamous epithelia. To define the regulatory elements that mediate the expression of the loricrin gene, we replaced the loricrin coding sequences from a 6.5-kilobase genomic fragment with the chloramphenicol acetyltransferase gene and transfected this construct into cultured mouse keratinocytes. High expression levels were observed in both undifferentiated as well as differentiating cells. Transgenic mice bearing a similar construct, but with beta-galactosidase as the reporter gene, corroborated these in vitro findings and showed tissue- and cell type-specific, but not differentiation-specific expression. Deletion analysis of the promoter region determined that sequences up to -60 base pairs from the start of transcription could be removed without significant loss of promoter activity. Within these proximal 60 base pairs is an evolutionarily conserved AP-1 element that is recognized by both purified c-Jun and AP-1 factors from keratinocytes in vitro. Mutation of this AP-1 site abolished the activity of the loricrin promoter. These studies show that elements directing expression of the loricrin gene to the stratified squamous epithelia are contained within a 6.5-kilobase genomic fragment, and those elements required to restrict expression to differentiated keratinocytes lie outside this region.

Cooliatoxin, the first toxin from Coolia monotis (Dinophyceae).

Coolia monotis is a benthic dinoflagellate previously thought to be non-toxic. We describe a new toxin, named cooliatoxin, purified from cultures of a strain of C. monotis isolated from Australia. Cooliatoxin is likely a mono-sulphated, polyether toxin (M = 1,062; i.p. LD50 = 1 mg/kg in mice) that induces hypothermia and respiratory failure in mice after a pronounced delay period during which there are no obvious signs of intoxication. These signs in mice are similar to those reported for the shellfish toxin named yessotoxin and the molecular weight of cooliatoxin corresponds to the mono-sulphated form of yessotoxin, suggesting that cooliatoxin may be an analogue of yessotoxin. Cooliatoxin has no effect on the mouse phrenic nerve or diaphragm musculature in vitro but causes initial stimulation and subsequent block of unmylenated nerves in vitro. In isolated guinea pig left atria, cooliatoxin (above 20 nm) induced a slow developing concentration dependent sustained inotropic response. Propranolol or tetrodotoxin reversed the positive inotropic effects, indicating that the majority of the cooliatoxin induced response was mediated by stimulation of nerves associated with the atrial musculature, resulting in the release of noradrenaline. Cooliatoxin induced transient contractions in isolated guinea pig vas deferens preparations. Atria and vas deferens preparations were tachyphylactic to a second equivalent dose of cooliatoxin applied after the effects of the first dose had diminished. The observed in vitro effects of cooliatoxin on peripheral nerves are unlikely to account for the lethal effects in mice and a central action of this toxin is suspected.

The gastrointestinal handling and metabolism of [1-13C]palmitic acid in healthy women.

The gastrointestinal handling and metabolism of [1-13C]palmitic acid given as the free fatty acid was examined in six healthy women by measuring the excretion of 13C-label in stool and in breath as 13CO2. The gastrointestinal handling of [1-13C]palmitic acid was compared with the apparent absorption of dietary lipid by measuring lipid losses in stool. The variation both within and between subjects was determined by repeating the study in the same individuals on separate occasions. The time course for excretion of label in stool over the five-day study period followed a common pattern, with most of the label excreted over the first two days of the stool collection. 13C-Label excreted in stool over the five-day study period was 14.3 +/- 9.8% of that administered and on repeating the trial was 31.6 +/- 24.7% (not significantly different due to variability); there was poor agreement within subjects. Lipid excreted in stool expressed as a percentage of ingested lipid was 5.2 +/- 4.4% in Trial 1 and 5.9 +/- 4.0% in Trial 2, and was the same in each individual on repeating the trial. There was no clear relationship between the excretion of 13C-label and lipid in stool (Trial 1: R = -0.43, P > 0.40; Trial 2: R = -0.02, P > 0.97). On the first occasion, 22.0 +/- 4.5% of the administered label was excreted on breath over the 15-h study period and on repeating the trial was 15.8 +/- 9.5% (not significantly different) with poor repeatability in a given individual.(ABSTRACT TRUNCATED AT 250 WORDS)

Nicotinic acetylcholine receptors on the surface of the blood fluke Schistosoma.

Blood dwelling stages of schistosomes have acetylcholinesterase (AChE) on their teguments. As an initial step towards understanding the function of tegumental AChE, we have used specific ligand-binding assays to identify nicotinic acetylcholine receptors (nAChR) on the schistosome surface. AChR could not be detected on migratory stages using fluoroscein isothiocyanate-alpha-bungarotoxin binding but the amount of specific labelling increased on sexual pairing and as the parasites matured into egg-producing adults. Both AChE and nAChR were concentrated on the dorsal surface of the adult male. These results indicate a role for AChE and AChR associated with the transporting function of this membrane.

Variation in angiographic practice and technique amongst interventional radiologists in the UK: assessment by postal questionnaire.

In order to ascertain the national variation in practice and workloads amongst interventional radiologists a questionnaire was sent to all members of the British Society of Interventional Radiologists of three or more years standing. The questionnaire was designed to assess changes in workload and angiographic practice amongst members at the various institutions. Results indicate that, over the last five years, there has been a dramatic increase in both diagnostic and therapeutic procedures performed, with an overall doubling of workload. There are, however, wide variations in angiographic practice and technique and referral pattern.

A comparison of the investment in hospital-based obstetrical ultrasound in Wales and Washington state.

The purpose of this study was to examine differences in the way Britain and the United States invest in and deploy a new medical technology. We used structured interviews to obtain information on the technical sophistication and approximate replacement value of all hospital-based obstetrical ultrasound machines in every maternity hospital in Washington state and Wales. The supply of hospital-based ultrasound machines--approximately two machines per 1,000 births--was similar in both countries. Wales had fewer advanced ultrasound machines than Washington state, and they were based exclusively in high-volume district general hospitals; there were no obstetric ultrasound machines in the private sector. In Washington state, the majority of advanced machines were in small and medium-sized hospitals, and many private offices had ultrasound machines. The approximate replacement value of hospital-based machines was three times as high per birth in Washington state as in Wales. In the case of obstetrical ultrasound, centralization of facilities, a relatively small private sector, and global budgeting lead to lower expenditures per patient within the National Health Service without compromising access to care.

Community survey of home nebulizer technique by elderly people.

To determine the prevalence and nature of difficulties elderly subjects experience using home nebulizers we have studied all patients in the Leeds Health District aged 70 years and older loaned a nebulizer from the hospital for at least 6 months. Of 42 subjects with a nebulizer, 40 (95%) agreed to assessment. They had a mean age of 79 years (range 70-84 years) and mean duration of nebulizer use of 38 months (range 6-120 months). Twenty subjects had one or more problems with 14/40 (35%) rated as moderate or severe; difficulties with cleaning 16/40 (40%) and filling 7/40 (18%) were the most frequent; 13/40 (33%) were sometimes or always dependent on their carers to administer their nebulizers. Elderly subjects often experience practical difficulties, or require help from a carer, when using a home nebulizer.

Palmitoyl-acyl carrier protein (ACP) thioesterase and the evolutionary origin of plant acyl-ACP thioesterases.

Acyl-acyl carrier protein (ACP) thioesterases play an essential role in chain termination during de novo fatty acid synthesis and in the channeling of carbon flux between the two lipid biosynthesis pathways in plants. We have discovered that there are two distinct but related thioesterase gene classes in higher plants, termed FatA and FatB, whose evolutionary divergence appears to be ancient. FatA encodes the already described 18:1-ACP thioesterase. In contrast, FatB representatives encode thioesterases preferring acyl-ACPs having saturated acyl groups. We unexpectedly obtained a 16:0-ACP thioesterase cDNA from Cuphea hookeriana seed, which accumulate predominantly 8:0 and 10:0. The 16:0 thioesterase transcripts were found in non-seed tissues, and expression in transgenic Brassica napus led to the production of a 16:0-rich oil. We present evidence that this type of FatB gene is ancient and ubiquitous in plants and that specialized plant medium-chain thioesterases have evolved independently from such enzymes several times during angiosperm evolution. Also, the ubiquitous 18:1-ACP thioesterase appears to be a derivative of a 16:0 thioesterase.

Utilizing Peplau's psychodynamic theory for stroke patient care.

A theory which enables the recipient of care to identify health-care needs is paramount. Hildegard Peplau's (1952) theory of psychodynamic nursing enables this process to occur in collaborative identification and resolution of need. The psychodynamic approach can direct the user to focus on the psychosocial problems created by a stroke. The advantages and limitations of the theory are still to be investigated with the continued application to varied clinical situations. There is a need to generate an expanded research-based taxonomy of psychosocial interventions for this client group.

Reflective process in action: the uncovering of the ritual of washing in clinical nursing practice.

Reflection can be utilized to reveal aspects of clinical practice which remain shrouded in ritualistic origin. The underlying themes surrounding the action of washing patients can be traced to Victorian legacies and the nurse education process. The nurse-patient relationship serves as a template for the deliverance of nursing interventions. An incomplete awareness of the nurse-patient relationship persists as to its initiation, continuation and usefulness in clinical nursing. Following reflection, there has been an increased awareness of feelings and attitudes surrounding the activity of washing a patient.

An analysis of case management--the efficient utility of human resources, but to what end?

Case management has been credited as a modality to reduce the lengths of hospital admission and to utilize human resources more effectively. The approach concentrates on the process of delivering care to constantly improve the productivity levels of health care practitioners. The United Kingdom government policy is certainly progressing towards cost-containment and cost-effective service delivery. However, it needs to be considered if the culture of nursing would be able to adopt the strategy. Firstly, it needs to be questioned if nurses have the managerial and financial acuity to successfully perform the role. Secondly, the adoption of case management may potentially negate the value of some aspects of professionally skilled nursing care which has little cost-benefit analysis but provides the true essence of nursing. With a further advancement of cost-centered care, case management may become the anti-thesis of an inherently caring profession.

The organizational influence on counselling relationships in a general hospital setting.

Counselling is becoming increasingly important as a development of nursing practice. The Scope of Professional Practice has enabled the emergence of the nurse, midwife and health visitor as a hybrid practitioner, the nurse counsellor. This paper explores the nature of the 'nurse counsellor's' role. The paper makes a distinction between counselling skills as a feature of the nurses' role and counselling practice. It examines the influence exerted through the organizational context of the work setting. The author explores the issues as they arise in a general health setting and makes a case for organizational empathy toward what is essentially an embryonic and potentially misunderstood development of nursing practice.

Improving the skills of doctors in giving distressing information.

In response to the growing awareness of the need to convey distressing information effectively, four workshops were held for doctors across Britain with the aim of improving skills, knowledge and attitudes. From the unexpectedly large response of 441 nominations, 20 participants were selected for each workshop. Participants identified four major concerns: how to do it; where to give distressing information; how to deal with collusion; and how to survive mentally in the work. After sessions on these areas, participants interviewed a simulated patient and were recorded on videotape for subsequent feedback. The results showed that after brief training, 91% of all participants gave a warning of distressing information, 85% gave the information at the patient's pace, but only 5% attempted to 'pick up the pieces'. Further work is required on this key aspect of clinical work.

Evaluation of a breaking bad news course for medical students.

This paper presents the evaluation of a Breaking Bad News course run for three groups of medical students (fourth and fifth year from the London Hospital Medical College and fourth year from St Bartholomew's Hospital Medical College). The course, which is student centred, uses group discussion, videotape presentations and role-play including actors. All teachers, clinicians and human science tutors, had been through a staff training programme on teaching methods. At the end of the course, students' knowledge of important principles in giving bad news had increased, particularly in relation to interpersonal communication; they were more confident in their ability to break bad news well; and the course learning methods were highly rated. The course was just as well received by fourth year as by fifth year students and several said they would like more of this training. The evaluation shows that if reservations about role-play can be overcome then this experiential learning is highly valued by students.
