[The problems involved in outcome control of non-industrial vocational rehabilitation measures].

The Social Code prescribes to the institutions financially responsible for rehabilitation to use their budgetary funds in accord with the principles of cost-effective and economical conduct, and to set up cost-benefit analyses in appropriate cases. This, however, presupposes a close and trusting working relationship between the financially responsible institutions and the rehabilitation facilities. It is possible to apply cost-benefit analysis to vocational rehabilitation; however, only those data should be included in the assessment that can in fact be clearly quantified. To ward off the danger of vocational rehabilitation missing labour market demands, improved statistical accessibility is recommended. The financially responsible institutions and the labour administration should draw up an overall statistical survey of rehabilitation--possibly in the form of a "rehabilitation report"--, containing break-downs of labour market statistics for comparison. This would facilitate vocational rehabilitation being accompanied and evaluated by research, the results obtained would be more obvious and clear. Improved prediction as regards employment opportunities of the rehabilitees could result.

Preparation of hormone-sensitive airway smooth muscle adenylate cyclase from dissociated canine trachealis cells.

Conventional homogenizing methods produced membrane preparations of canine trachealis airway smooth muscle which contained adenylate cyclase activity that was stimulated by fluoride but not by isoproterenol. We have devised methods using collagenase digestion of minced trachealis which destroy most of the tough connective tissues but leave dissociated canine trachealis cells in suspension. Gentle homogenization of these cells permitted preparation of a particulate fraction containing adenylate cyclase that was readily stimulated by beta-adrenergic agonist of prostaglandin E2. Isoproterenol stimulation was 2.34 +/- 0.58 (S.E.) times basal and 122 +/- 25% of the stimulation induced by NaF. The beta-adrenergic blocking agent propranolol prevented isoproterenol-induced stimulation of the cyclase but had no effect on prostaglandin E2 stimulation. Catecholamine order of potency was isoproterenol greater than epinephrine greater than norepinephrine. These methods enable demonstration of stimulatory effects of hormones in broken cell preparations of airway smooth muscle that are comparable to those when hormone-stimulated cyclic AMP formation is measured in intact muscle strips.

Cage culture turbidostat: a device for rapid determination of algal growth rate.

The present cage culture turbidostat consists of a growth chamber and a control unit. The microorganisms (photoautotrophic algae) are kept in the growth chamber by porous membranes (pore size 1 to 3 mum) which retain the algae but allow efficient exchange of the growth medium. Flow rate and composition of the medium can therefore be varied independently of algal population density. A reciprocating pumping mode of the medium is introduced to obtain more gentle clearance of membranes than that provided by rotation or stirring in other membrane fermentors. Pulsed light and a light-emitting diode/light-sensitive transistor couple are used to monitor the turbidity of the culture, independent of external light needed for growth. The control unit keeps the turbidity constant by frequent activation of the dilution pump. Theoretical analysis of growth in the turbidostat shows that integrated activation time of the dilution pump is proportional to the growth rate of the organism. Theoretical analysis was also used to determine minimum flow-rate and nutrient concentration of medium to cover the requirement of the algae. Experiments with three different marine diatoms were carried out, and they demonstrated that the growth rate could be determined every hour and that the cultures could be kept at constant turbidity over 10 to 14 days at least.

The difference between fetal transcutaneous pO2 and arterial pO2 during labour.

Sub partu measurements of the transcutaneous pO2 (tcpO2), the relative local skin perfusion, the fetal heart rate, and the amniotic fluid pressure were carried out with commercial equipment on 40 pregnant women at term. In 19 cases (47.5%), the tcpO2 could be monitored until the fetal head was delivered. The relationship between the tcpO2 and the arterialised capillary scalp blood was investigated in 20 patients on 42 micro-blood samples. Changes of the fetal heart rate pattern were analysed by means of a cardiotocogram scoring system with regard to its influence on the tcpO2 values. Two main groups of tcpO2 measurements could be differentiated. In 9 of 19 cases the tcpO2 remained stable over a prolonged period and decreased only during the final phase of birth to values between 6 and 18 mm Hg. In 10 of 19 cases the tcpO2 values began to decline to low levels in the course of labour as early as 90 min ante partum and reached values of 0-3 mm Hg at birth. In 9 of these 10 cases a tcpO2 'zero line' could be monitored in the last 2-90 min prior to birth. The tcpO2 did not correlate to either the pO2 of the arterialised fetal scalp blood or to the pO2 of the umbilical artery blood. Even with a tcpO2 of approximately 0 mm Hg, an arterial pO2 of 14.5-22.5 mm Hg in the scalp blood and of 10-20 mm Hg in the umbilical artery blood could be determined. In most of the cases a difference between the tcpO2 and the arterial pO2 developed.

[Diphenols and caramel compounds in roasted coffees of different varieties. II. (author's transl)].

Eleven diphenols and seven caramel compounds were extracted from roasted coffee with pentane/ether (1 + 1), silylated with BSTFA, characterized, and semiquantified by GC-MS. Seven phenols and five caramel components were identified for the first time in roasted coffee. Robusta coffee contained the highest amounts of phenols and maltol. Arabica showed higher amounts of furaneol (2.5-dimethyl-4-hydroxy-3(2H)-furanone), isomaltol, 5-hydroxymaltol, and 5-hydroxy-5.6-dihydromaltol.

Lysozyme activity in cerebrospinal fluid. Studies in inflammatory and non-inflammatory CNS disorders.

Lysozyme activity was measured in cerebrospinal fluid (CSF) from 114 patients with inflammatory (bacterial and serous meningitis, polyradiculitis, encephalitis) and non-inflammatory (multiple sclerosis, CNS tumors, cerebral vascular diseases) CNS diseases. Highly elevated values were found consistently in patients with bacterial meningitis. Elevated values were found also in patients with encephalitis, polyradiculitis, multiple sclerosis and CNS tumors, but a considerable overlapping between these groups and normal controls precludes the use of CSF lysozyme measurements as a diagnostic aid in the latter disease groups. Simultaneous measurements of lysozyme, albumin and IgG in CSF and serum suggested that the mechanism for increased CSF lysozyme values in bacterial meningitis is mainly a breakdown of the blood/brain barrier, whereas the increased CSF lysozyme values in the remaining groups of patients are more likely caused by production of lysozyme by cells within the meninges (neutrophilic granulocytes, monocytes?).

Neurofibromatosis of the bladder.

A case of neurofibromatosis of the bladder of a 19-year-old man is reported. If patients with cutaneous manifestations present urinary symptoms, bladder involvement should be suggested. When upper urinary tract dilatation is demonstrated urinary diversion above the bladder is recommended.

Plasma level and effect of carbamazepine in grand mal and psychomotor epilepsy.

In 117 patients with grand mal and psychomotor epilepsy the effect of carbamazepine alone or in combination with phenobarbital and diphenylhydantoin was registered and correlated to the plasma level of carbamazepine. Carbamazepine was found to be an effective anticonvulsant in the treatment of grand mal epilepsy. The therapeutic level was higher than 4 mg/1. Carbamazepine given alone was rather ineffective in the treatment of psychomotor seizures.
