[Leiomyosarcoma of the vagina: a rare case].

Vaginal leiomyosarcoma is unfrequent. We report on a case in a 50-year-old multipara patient who had presented a posterior vaginal swelling since 6 months. The tumor was discovered at the occasion of pains and non hemorragic discharge. The histological pattern of the tumor was well-differentiated spindle cell sarcoma with pleiomorphic areas. The immunohistochemistry confirmed the smooth myogenic differentiation. The treatment consisted of posterior pelvic exenteration extended to the vagina. The patient is alive and free of disease at 20 months of fellow-up.

[Spermatocytic seminoma. Apropos of a case and review of the literature].

We report a case of sperm cell seminoma caused by trauma. The data in the literature indicate the frequency is less than 5% of all seminomas. This case was exclusively located in the gonads and was a pure form. Orchidectomy with high ligature of the cord and adjuvant radiotherapy at the dose of 25 Gy centered on the para-aortic and subdiaphragmatic chains is adequate treatment. When inguinoscrotal surgery is performed, this zone must be irradiated with 25 Gy. Prognosis is satisfactory: 5-year survival is 100% with this protocol.

Accidental overexposure to 192Ir source in industrial radiography: a follow-up study.

The consequences of an accidental overexposure from an 192Ir radiography source are being followed up to assess the long-term effects to the victim. The estimated dose equivalents resulting from the accident were approximately 24 Sv to the fingertips and 2-3 Sv to the body. Progressive tissue deterioration has set in and has led to multiple surgeries and successive amputations of the fingertips.

Short-term experiments on ion transport by seedlings and excised roots : technique and validity.

The absorption of K(+) by excised roots of barley (Hordeum vulgare L. cv California Mariout) has been systematically compared with that of entire, undisturbed seedlings. Some experiments have also been done with wheat (Triticum aestivum L.) and an amphiploid obtained from a cross between it and salt-tolerant tall wheatgrass (Lophopyrum elongatum Host Löve [syn. Agropyron elongatum Host]). For all three genotypes, the rate of K(+) absorption measured in a 20-min period was identical for entire 8-d-old seedlings and their excised roots within the experimental error. Manipulation gentler than root excision, viz. careful transfer of seedlings from one experimental solution to another, was also without effect on the rate of K(+) absorption. Absorption of K(+) measured by assay of its (86)Rb label in the tissue was identical with that measured by K(+) depletion of the experimental solutions assayed chemically. For the plant materials and conditions of these experiments, the excised root technique for studying ion transport into roots is validated. The advantages of the technique, and findings differing from the present ones, are discussed.

[Isolated retroperitoneal hydatid cyst. Apropos of a case].

This is a report of a rarely observed localisation of hydatid cyst: the retroperitoneal space. The hypotheses of pathogenesis advanced are reviewed and the interest of intravenous urography and ultrasonography in the diagnosis is emphasized. Surgery the only treatment, consists of partial resection of the cyst.

An overexposure in industrial radiography using an 192Ir radionuclide.

An industrial radiographer was accidentally exposed to a high dose of ionizing radiation from an 192Ir source during radiography of weldjoints in gas pipelines. Some symptoms of high radiation exposure occurred immediately after the incident. The clinical effect of skin erythema developed within 7 d, leading to progressive tissue deterioration. The dose to the body was estimated to be about 2-3 Gy, and the dose to the fingertips was approximately 24 Gy.

[Surgery of hydatid cysts of the liver: 581 patients, 952 cysts].

Over a period of 20 years, surgery was carried out in 581 patients for 952 hydatid cysts of the liver. Patients included 372 females and 209 males, aged from 6 to 70 years, with a mean age of 34 years. 52% of patients were aged between 20 and 40 years. The cyst was single in 324 patients, double in 102, triple in 22 and multiple in 133. Treatment most often consisted of resection of the projecting dome of the cyst (78%), and more rarely pericystectomy (5.5%), hepatic resection (3.5%) or external drainage (4.2%) via a median incision in 412 cases and a subcostal incision in 126 cases. Capitonnage of the residual cavity was combined with resection of the projecting dome, 176 cases included a cystobiliary fistula with migration occurring in 30 cases. Morbidity was 20% and included, in particular, 45 cases of subphrenic abscess. 19 patients showed signs of recurrence and 16 underwent further surgery. Overall mortality was 2.06% i.e. 12 patients. In endemic countries, resection of the projecting dome remains a method which gives good results and has a low mortality and morbidity.

[Recessive dystrophic epidermolysis bullosa (with esophageal involvement)].

We are presenting a young man with Epidermolysis Bullosa (Dystrophica recessive type) who had lesions in hands, feet and face; some plaques of alopecia were noted on the scalp. Three years ago the patient showed marked dysphagia due to some esophagus erosions, which have been appearing regularly. One year ago it was necessary to take the patient to the Hospital where a esophagectomy was performed replacing the esophagus by a piece of colon.

[2 cases of the diffuse form of Caroli disease].

Report of two cases. The authors report two observations in what concern the diffused form of the Caroli's disease, one pure, the other mixed associating the congenital dilatation of the intra-hepatic bileducts with lesion of congenital hepatic fibrosis. They emphasize the efficiency of the echographia and computed tomography in diagnosing Caroli's disease. The surgical treatment of these diffused forms often complicated by acute recurrent cholangitis used with some success various methods of biliary drainage. But the radical treatment seems to be obliged to turn itself more likely toward liver transplantation.
